Book excerpt: L’imagerie de diffusion est utilisée aux différents âges post-nataux, mais n’a pas encore été étudiée chez le foetus. Afin de déterminer sa faisabilité, nous avons étudié de manière prospective à l’Hôpital Robert Debré, 48 IRM de diffusion avec mesure du coefficient apparent de diffusion (ADC) et de la fraction d’anisotropie (FA) dans dix-neuf sites anatomiques. Vingt-cinq examens de foetus normaux âgés de 31 SA à 37 SA et 3 jours sont inclus dans l’étude. Parmi les examens exclus, quatre examens l’ont été pour des artéfacts dûs à un dysfonctionnement des gradients de diffusion et/ou aux mouvements du foetus. Les résultats de notre étude, comparés à ceux de la littérature chez des prématurés d’âges équivalents, valident la mesure de I’ADC in utero. Ainsi on retrouve des différences significatives de l’ADC selon les régions anatomiques et selon la micro-architecture de substance blanche qui est fonction de son organisation spatiale (centre semi-ovale versus corps calleux, respectivement 1,8 μm2 versus 1,2 μm2 et de sa myélinisation (centre semi-ovale versus pons postérieur, respectivement 1,8 μm2 versus 1,1 μm2 La maturation cérébrale se traduit par une décroissance de I’ADC en fonction du terme dans les sites en cours de myélinisation (pons postérieur, pédoncule cérébral). L’ADC de la substance blanche frontale, du centre semi-ovale et de la substance blanche occipitale, non myélinisés dans l’intervalle des termes étudiés, varie peu. La précision et la reproductibilité de la mesure de la FA est moindre que chez le nouveau-né. Cependant elle montre des corrélations intéressantes avec le terme au niveau des faisceaux organisées de substance blanche, et cela avant le début de toute myélinisation visible. L’utilisation des nouvelles séquences d’IRM et de nouvelles antennes pourraient permettre de surmonter ce problème. Cette étude est un préalable à l’application de la technique à la pathologie foetale.

Book excerpt: In Volume 5 of Surgical Neuroangiography we will discuss indications and approaches for endovascular treatment of disorders affecting the spine and spinal cord circulations. Our approach is similar to those discussed in volume 2 and 4, it requires integration of knowledge concerning functional vascular anatomy, lesion angioarchitecture, and the relationship between normal and pathological circulations. In AVMs these features are considered as they relate to the clinical presen tation, progression, and natural history of the lesion. All these factors must then be combined with an understanding of technical capabilities to for mulate a pretherapeutic plan designed to favorably affect the long term out come in the individual patient. Endovascular surgery of spinal cord AVMs has become the treatment of choice, while microsurgery is reserved for those cases where embolization fails. Endovascular surgery may be the sole form of treatment, or may be combined with other therapeutic modalities including surgery, chemothera py, or radiation in cases of neoplasms, or may be palliative, to decrease pain or spinal cord compression. As in all other areas a multidisciplinary ap proach is essential for decision making to best accomplish the desired ob jectives.
