Book excerpt: This book addresses a topic that has received surprisingly little attention to date--the health and social needs of young adults with physical disabilities. Following an extensive review of the contemporary literature on the subject, the authors report an in-depth study on a large sample of physically disabled 18- to 25-year olds living in two survey areas, one urban and one semirural. The study, which included pediatric examinations and interviews by a social psychologist, found that a large proportion of subjects face social and medical problems, for instance: They have serious health needs that are not being met by the statutory services; they left school with less than adequate skills to enable them to lead an independent adult life; they are without a daytime occupation or attend day centers that are described as "unstimulating" places; they do not receive all the financial benefits to which they are entitled; they are isolated from their peers and lack the social skills to interact with others in a positive and successful way. Parents and guardians were also found to receive little physical or emotional support. These findings underline the need for a better system of providing and coordinating services for this client group, and the authors call for the formation, within each local health authority area, of an "Adult Disability Service" that would have a clinical and planning role in terms of health care and that would act as a linchpin in the coordination of health, educational, social, and voluntary services. Although this work relates specifically to service provision in the United Kingdom, it is felt that the conclusions and recommendations are also broadly applicable to the situation in other countries.

Book excerpt: In the current era of cost awareness and the pressure to free hospital beds as rapidly as possible, coupled with the move towards community care, there is increasing emphasis on returning and main taining disabled people in the community. Often, many health and welfare workers are involved in this process. Occupational therapists however have a particular role to play. Their training is such that they are able to assess and treat the physical, psychological and social aspects of a disabling condition. This enables them to help disabled people to achieve their maximum ability in the environment in which they live. Whether the occupational therapist is hospital or community based, employed by the health or social service depart ment, her caseload is often vast. The occupational therapist may specialize in a particular age group or disability, but it is more likely that she will have to work with a wide age range from the very young to the very old. Assessment of needs and the subsequent provision of equipment and adaptations is the greater part of the OT's job. Other aspects include counselling the disabled person and his family and planning future intervention must often, however reluctantly, be given a low priority. In spite of staff shotages occupa tional therapists are frequently called on to provide the client and other groups of workers with advice and information on all aspects of living with any disability.

Book excerpt: Improved housing conditions can save lives, prevent disease, increase quality of life, reduce poverty, and help mitigate climate change. Housing is becoming increasingly important to health in light of urban growth, ageing populations and climate change. The WHO Housing and health guidelines bring together the most recent evidence to provide practical recommendations to reduce the health burden due to unsafe and substandard housing. Based on newly commissioned systematic reviews, the guidelines provide recommendations relevant to inadequate living space (crowding), low and high indoor temperatures, injury hazards in the home, and accessibility of housing for people with functional impairments. In addition, the guidelines identify and summarize existing WHO guidelines and recommendations related to housing, with respect to water quality, air quality, neighbourhood noise, asbestos, lead, tobacco smoke and radon. The guidelines take a comprehensive, intersectoral perspective on the issue of housing and health and highlight co-benefits of interventions addressing several risk factors at the same time. The WHO Housing and health guidelines aim at informing housing policies and regulations at the national, regional and local level and are further relevant in the daily activities of implementing actors who are directly involved in the construction, maintenance and demolition of housing in ways that influence human health and safety. The guidelines therefore emphasize the importance of collaboration between the health and other sectors and joint efforts across all government levels to promote healthy housing. The guidelines' implementation at country-level will in particular contribute to the achievement of the Sustainable Development Goals on health (SDG 3) and sustainable cities (SDG 11). WHO will support Member States in adapting the guidelines to national contexts and priorities to ensure safe and healthy housing for all.

Now, as a new book with a radical new vision, comes his Designing for the Disabled: The New Paradigm. Goldsmith's new paradigm is based on the concept of architectural disability. As a version of the social model of disability, it is not exclusively the property of physically disabled people. Others who are afflicted by it include women, since men customarily get proportionately four times as many amenities in public toilets as women - and women have to queue where men do not - and those with infants in pushchairs, because normal WC facilities are invariably too small to get a pushchair and infant into. To counter architectural disability, Goldsmith's line is that the axiom for legislation action has to be 'access for everyone' - it should not just be 'access for the disabled', as it presently is with the Part M building regulation and relevant provisions of the 1995 Disability Discrimination Act. In a 40-page annex to his book he sets out the terms that a new-style Part M regulation and its Approved Document might take, one that would cover alterations to existing buildings as well as new buildings. But architects and building control officers need not, he says, wait for new a legislation to apply new practical procedures to meet the requirements of the current Part M regulation; they can, as he advises, act positively now. This is a book which will oblige architects to rethink the methodology of designing for the disabled. It is a book that no practising architect, building control officer, local planning officer or access officer can afford to be without.
