In recent years, despite the development of prenatal ultrasound technology, mortality in cardiac anomaly has reached 10% and this rate covers 50% of the deaths related to malformation (1,2). The birth of a baby is one of the most powerful human experiences and creates a great hope and expectation in families. The effects of the disease of the child on the parents vary by disease being a threat to his/her life, having a significant impact on the daily life of families, arousing a feeling of uncertainty for the future and being diagnosed after birth. Hopelessness that families experience adversely affects the treatment compliance of the individuals, their efforts, motivation and coping mechanisms . In this study, hopelessness levels of the parents of children with congenital heart disease (CHD) and influencing factors were determined.The universe consists of parents applied to Ege University Department of Cardiovascular Surgery and whose children had CHD surgery. The sample includes parents applied to the hospital between August and December 2015, agreed to participate and with children aged 0-6 had surgery. Sample size was 100 parents (50 males, 50 females) according to priori power analysis. Patient identification form for sociodemographic characteristics and The Beck Hopelessness Scale were used to collect data. Descriptive statistics, the Kruskalu2013Wallis test, the Mann-Whitney U and the Independent Samples t test were used.It was determined that gender and age of the parents and children and diagnosis during or did not affect hopelessness level (p>0.05) unlike educational background (p

Book excerpt: Little is known about the mental health burden or the risk and protective factors that have impacted mental health outcomes in parents of children with congenital heart defects (CHD) during the COVID-19 pandemic. Exposure to COVID-19 related stressors plus CHD care related stressors may have had an additive effect on this parent population's mental health. The purpose of this study was to examine risk and protective factors associated with anxiety symptoms, depression symptoms, and perceived stress in parents of young children with CHD during the COVID-19 pandemic. The study used a cross-sectional, nonexperimental survey design that focused on parents of children aged newborn to five years with CHD one year into the COVID-19 pandemic. The findings reveal CHD parents experienced significant anxiety, depression, and stress during the COVID-19 pandemic at levels higher than the US general population norms. Regression analyses were conducted to examine associations between COVID-19 stressors, CHD care related factors, parental resilience, external support, and mental health outcomes. Increased levels of anxiety symptoms, depression symptoms, and perceived stress were associated with 1) Exposure to a greater number of COVID-19 related stressors, 2) Distress from family visitation restrictions during healthcare encounters, 3) Worry related to the perceived risk of their CHD child's exposure to COVID-19 during healthcare encounters, and 4) Worry about their CHD child's risk of death or serious illness from COVID-19. Parental resilience, emotional support, and informational support were shown to be key protective factors for anxiety, depression, and stress. However, resilience was remarkably low in CHD parents. Further research is needed to expand our understanding of resilience in CHD parents and to explore strategies that may support resilience in this population.

Book excerpt: Depression is a widespread condition affecting approximately 7.5 million parents in the U.S. each year and may be putting at least 15 million children at risk for adverse health outcomes. Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Congenital Heart Disease (CHD) is the most commonly diagnosed birth abnormality in the US. With great advances in surgery and medicine, however, survival rates have improved by 75% since the 1940s. Welcome news, of course, as only a few decades ago these birth defects were considered a death sentence, but as with any chronic condition, survival does not mean the issue is cured. With better medical care, babies born today with CHD have a good chance of surviving, but throughout their entire lives they can face surgeries, invasive treatments, lifelong monitoring, frequent medical check-ups, and significant limitations on physical activity, in addition to poor public awareness which can have an impact on social inclusion and understanding. Much attention has rightly been focused on the medical needs of these children and on providing their parents and caretakers with resources and information to navigate the complexities of this chronic condition. Little attention, however, has been paid to the psychosocial impacts on these individuals, especially as they grow, mature, and become adults living with a serious, chronic medical condition. Prevalence rates for anxiety, depression, and PTSD are significantly higher (2-3x in some cases) for people living with CHD than the general adult population. From feeling self-conscious about scars and limitations on physical activity and sense of loss around so many of life's little normalcies, to frequent hospital visits and living in constant fear of an emergency, this condition is ever-present. Liza Morton and Tracy Livecchi are both mental health professionals who have developed a specialty in working with clients who have chronic medical conditions, and they are both themselves living with CHD. In this book they set out to provide the resources and support they have been looking for their entire adult lives. While their powerful personal stories are woven into the narrative, the book is focused on providing evidence-based coping and self-care skills for adults living with CHD.
