Download or read book Block Granting Medicaid written by Edward Alan Miller and published by Routledge. This book was released on 2013-11-12 with total page 252 pages. Available in PDF, EPUB and Kindle. Book excerpt: Medicaid is the largest grant-in-aid program in the United States. Reform in this area, therefore, provides a unique opportunity to study the intersection between federal and state policy making in an area recently characterized by substantial uncertainty deriving from the lingering effects of the Great Recession, ongoing debate over the federal budget, and implementation of the Patient Protection and Affordable Care Act. Invariably states reform the way health care is delivered, regulated, and financed within broader parameters established by federal statutes and regulations. It is critical therefore that effective strategies be put into place if both current and future health and long-term care reform efforts are to have their greatest chances at success. Rhode Island is the first state to receive permission to operate its entire Medicaid program under a global cap. As a consequence, it has entered the national consciousness as a key data point potentially supporting the block grant approach to Medicaid reform. In this book, Edward Alan Miller identifies factors that either facilitated or impeded the design and implementation of Rhode Island’s Global Consumer Choice Compact Medicaid Waiver in order to draw broader lessons for the Medicaid block grant debate and health and long-term care reform more generally. Evidence gathered from archival sources and in-depth interviews with key stakeholders exposes the role that provider capacity has played in the implementation process, including adult day care, assisted living, home maker, and other home- and community-based services. The impact of the Global Waiver on the nursing home sector is examined as well, in addition to new authority to obtain federal matching dollars for previously state-only funded programs. By providing a sophisticated understanding of factors enhancing or impeding state health reform, this book will contribute to improvements in the development and administration of policy development at both the state- and federal-levels.

Book excerpt: This Second Edition of the Handbook addresses the evolving interdisciplinary health care context and the broader social work practice environment, as well as advances in the knowledge base which guides social work service delivery in health and aging. This includes recent enhancements in the theories of gerontology, innovations in clinical interventions, and major developments in the social policies that structure and finance health care and senior services. In addition, the policy reforms of the 2010 Patient Protection and Affordable Care Act set in motion a host of changes in the United States healthcare system with potentially profound implications for the programs and services which provide care to older adults and their families. In this volume, the most experienced and prominent gerontological health care scholars address a variety of populations that social workers serve, and the arenas in which they practice, followed by detailed recommendations of best practices for an array of physical and mental health conditions. The volume's unprecedented attention to diversity, health care trends, and implications for practice, research, policy make the publication a major event in the field of gerontological social work. This is a Must-Read for all social work social work educators, practitioners, and students interested in older adults and their families.

Available in PDF, EPUB and Kindle. Book excerpt: In the past, when people with disabilities and older adults needed help with activities of daily living and navigating their communities, they rarely had any choice about who helped them, when that support was delivered, or what the worker would or would not do. The self-direction movement changed all that by offering people the option to select their own workers and even create an individualized budget to help them live more independently. Written by experts who played a key part in the growth, evaluation, and dissemination of this revolutionary approach, Self-Direction describes the development of this movement through the authors' personal accounts. Also included are stories from actual participants in the movement who benefitted from this approach and from policymakers who saw how self-direction could help address states' problems. The book's conclusion discusses recommendations that can improve the way self-direction is delivered and how to spread its message so that all people with disabilities can have this choice.
