Book excerpt: BACKGROUND: Recent advances in biomedical HIV prevention - including pre-exposure prophylaxis (PrEP) and treatment as prevention (TAsP) - are key to ending the HIV epidemic. The analysis examined how social factors that are strongly related to HIV incidence and treatment outcomes (e.g., substance use, access to healthcare, age, race/ethnicity, gender, and geographic location) may affect the HIV prevention continuum. METHODS: Chapters 2 and 3 use data collected as part of clinical care at the Los Angeles LGBT Center. In Chapter 2, a cross-sectional study of HIV-negative men who have sex with men (MSM) and transgender people who have sex with men (TGSM) who visited the Center between August 2015 and April 2017 examined how sexual history, substance use, and demographic factors were associated with initiating PrEP. In Chapter 3, records-based longitudinal study of patients prescribed PrEP at the Center evaluated discontinuation, HIV incidence, and loss to follow-up. Chapter 4 uses data from the mStudy to analyze the relationship between methamphetamine use (urine drug screen and self-reported frequency) patterns and viremia among HIV positive MSM of color. RESULTS: Use of sex drugs, but not alcohol use, was associated with PrEP initiation among MSM and TGSM. Key demographic risks were associated with lower odds of PrEP initiation (Black or Latino race/ethnicity, younger age). About half of patients who started PrEP at the Center discontinued or were lost to follow-up. HIV incidence among those who discontinued was 1.4%, compared to 0.3% among those who were actively attending follow-up appointments. Persistence was highest for those receiving PrEP through a low-cost program, and lowest for younger people. Longitudinal patterns of frequent and/or recent methamphetamine use were associated with a detectable pattern of viremia. CONCLUSION: Because younger people had lower PrEP initiation and more discontinuation compared to older people, strategies to support youth are key to PrEP's success for HIV prevention. The findings that methamphetamine was an obstacle to secondary HIV prevention but not necessarily to PrEP use highlight how facilitating PrEP use among people who use methamphetamine and other substances may be key to HIV prevention.

Book excerpt: Between 2011 and 2019, WHO has developed and issued evidence-based policy recommendations on the treatment and care of patients with DR-TB. These policy recommendations have been presented in several WHO documents and their associated annexes, including the WHO Consolidated Guidelines on Drug Resistant Tuberculosis Treatment, issued by WHO in March 2019. The policy recommendations in each of these guidelines have been developed by WHO-convened Guideline Development Groups, using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach to summarize the evidence, and formulate policy recommendations and accompanying remarks. The present WHO Consolidated Guidelines on Tuberculosis, Module 4: Treatment - Drug-Resistant Tuberculosis Treatment includes a comprehensive set of WHO recommendations for the treatment and care of DR-TB. The document includes two new recommendations, one on the composition of shorter regimens and one on the use of the BPaL regimen (i.e. bedaquiline, pretomanid and linezolid). In addition, the consolidated guidelines include existing recommendations on treatment regimens for isoniazid-resistant TB and MDR/RR-TB, including longer regimens, culture monitoring of patients on treatment, the timing of antiretroviral therapy (ART) in MDR/RR-TB patients infected with the human immunodeficiency virus (HIV), the use of surgery for patients receiving MDR-TB treatment, and optimal models of patient support and care. The guidelines are to be used primarily in national TB programmes, or their equivalents in Ministries of Health, and for other policy-makers and technical organizations working on TB and infectious diseases in public and private sectors and in the community.

It serves as an update to the previous edition of the consolidated guidelines on HIV. These guidelines continue to be structured along the continuum of HIV care. Information on new combination prevention approaches, HIV testing, ARV regimens and treatment monitoring are included. There is a new chapter on advanced HIV disease that integrates updated guidance on the management of important HIV comorbidities, including cryptococcal disease, histoplasmosis and tuberculosis. The chapter on general HIV care, contains a new section on palliative care and pain management, and up to date information on treatment of several neglected tropical diseases, such as visceral leishmaniasis and Buruli ulcer. New recommendations for screening and treating of cervical pre-cancer lesions in women living with HIV are also addressed in this chapter. Importantly, this guidance emphasizes the need for differentiated approaches to care for people who are established on ART, such as reduced frequency of clinic visits, use of multi-month drug dispensing and implementation of community ART distribution. The adoption of these efficiencies is essential to improve the quality of care of people receiving treatment and reduce the burden on health facilities, particularly in resource limited settings.
