Book excerpt: Nursing programs are increasingly facing issues with access to appropriate clinical sites, access to properly credentialed nursing faculty, and concerns regarding graduates readiness to practice safely. These issues have led to increased use of high fidelity simulation (HFS) experiences to replace traditional clinical rotations (Benner, Sutphen, Leonard & Day, 2010; Lasater, 2007a; Orsolini-Hain & Malone, 2007). Previous research has shown that traditional clinical rotations are instrumental in development of clinical reasoning skills and HFS experiences are valuable in developing technical skills. However, it does not provide insight into the impact of HFS experiences on development of clinical reasoning skills (Lapkin, Levett-Jones, Bellchambers & Fernandez, 2010). The purpose of this quasi-experimental pilot study was to determine the relationship between HFS clinical experiences and development of clinical reasoning skills in nursing students. Findings will influence the use of HFS to replace traditional clinical experiences and provide a foundation for future research on the efficacy of HFS in development of clinical reasoning skills. Analysis of data showed that there was no significant statistical difference in development of clinical reasoning skills between prelicensure nursing students attending traditional clinical experiences and those participating in simulated clinical experiences. These abilities are used continually, enabling nurses to provide safe nursing care to increasingly complex patients in a variety of healthcare settings. The challenge that faces baccalaureate nursing programs as they prepare nursing students to function competently upon graduation, is how best to facilitate the development of these skills in our students. The purpose of this capstone project was to test the effect of the middle range theory of the Developing Nurses' Thinking (DNT) Model on clinical judgment in nursing students. Tanner's Clinical Judgment Model formed the conceptual framework for this project. An experimental, pretest/posttest study was conducted using a convenience sample of 44 senior students at one southeastern baccalaureate nursing program. Two clinical groups received the intervention in post conferences, while three groups served as the control group. Student clinical judgment was measured in high fidelity patient simulation, using the Lasater Clinical Judgment Rubric. Paired t-tests evaluated the differences between clinical judgment scores and an independent t-test was utilized to evaluate the difference between groups. Both groups showed statistically significant improvements on the posttest, but there was no statistically significant difference between groups. Therefore, the findings of this study did not support the use of the DNT Model to facilitate the development of clinical judgment in nursing students. However, small sample size and inadequate exposure to the intervention were likely contributors to these outcomes.

Book excerpt: Can clinical judgment be increased because of certain methods implemented within nursing programs? The results of this project provided educators an alternative teaching method that enhances the students' level of clinical judgment. Through the use of simulation, nursing students would be able to develop the knowledge to notice, interpret, respond and reflect on the situation that has occurred within the simulation scenario. With the use of a high-fidelity simulator, simulation can offer a realistic form of clinical experience. Using simulation, students' were able to increase not only their clinical judgment, but their focused observational skills, recognizing deviations from patients, information gathering techniques as well prioritizing data that is obtained during the simulation. It is these aspects that have shown significant increase within this study. Using the tool of the Lasater Clinical Judgment Rubric (LCJR), educators can determine if students are developing an increase level of clinical judgment that would be needed to take care of the patients in today's health system. This project used a non-experimental non-equivalent comparative effectiveness design to compare scores for each student using the LCJR. Second-term nursing students will have a change in the level of clinical judgment based on the scores from the rubric. Nurses need clinical judgment to be able to perform at a competent level because skilled observation and interpretation of deviations from normal are actions much needed by nurses today.

Book excerpt: Problem The problem investigated in this study was the lack of empirical evidence available regarding the effectiveness of electronic interactive simulation (EIS) for developing critical thinking disposition and clinical judgment skills in the senior baccalaureate nursing student. Aim The aim of this study was to identify an effective method of experiential learning simulation that may be independently accessed by the learner with a goal of enhancing critical thinking disposition and clinical judgment skills of senior baccalaureate student nurses (BSN). Purpose The purpose of this experimental study was to compare the effects of EIS to traditional paper case studies on the critical thinking disposition and clinical judgment skills, measured by accuracy and efficiency of situational decision making, of senior nursing students enrolled in baccalaureate nursing programs in the United States. Methods One hundred and seventeen senior nursing students completed the randomized control study by using either the EIS or paper case study learning intervention. Repeated measures ANOVA and nonparametric tests were used to test the hypotheses that senior BSN who participate in EIS of real-life clinical scenarios over a period of two weeks will experience significant increases in clinical judgment and critical thinking disposition compared to students who receive traditional paper case study simulation. Findings Results showed that participants who used EIS over a two-week period increased their scores for critical thinking disposition overall and on three of the subscales. Results also indicated a positive trend, greater than the comparison group, on the remaining subscales. It is noted that many scores for the Case Study group actually decreased, suggesting that this method had a stifling effect on the development of critical thinking disposition. Retention and application of learned information was apparent for both groups, however, there was a trend for a greater change in the EIS group compared to the Case Study group. Additional research is needed to explore the effectiveness of this emerging pedagogy to add to what is known about the effects of experiential learning in the healthcare professions.

Book excerpt: Nurses are challenged to use critical thinking skills and make sound clinical judgment in achieving optimal patient outcomes and improving patient safety (Alfaro-LeFevre, 2017). Clinical judgment is the ability to interpret patient data, and develop appropriate and timely interventions (Bussard, 2018). The effect of simulation on the development of clinical judgment has been investigated extensively in traditional nursing students. The rapid development of second-degree nursing students to address the nursing shortage (Sedgwick, Kellet, & Kalischuck, 2014), requires further investigation of the effect of simulation on the development of clinical judgment in second-degree nursing students. Opportunities for simulation experiences promote the development of clinical judgment in second-degree nursing students (Mariani, Cantrell, Meakin, & Jenkinson, 2015). This study encompassed a regularly scheduled simulation experience for students enrolled in a gerontology course in a second-degree nursing program. The study used a cross sectional design and encompasses an educational preparation video and demographic survey prior to the simulation. The Lasater Clinical Judgment Rubric (Lasater, 2007) was used post debriefing and reflection to assess the effect of the simulation on the development of clinical judgment. Statistical analysis using SPSS software was used for data analysis, results, and understanding the implications for translating the evidence into practice.

Book excerpt: Due to the high acuity of patients today, it is imperative that nurses quickly and appropriately respond to patient deterioration. Appropriate clinical judgment is key to early detection of deterioration. However, nurse managers indicate that the majority of new graduates lack clinical judgment skills. To overcome this critical barrier to providing safe patient care, nurse educators suggest better training of student nurses regarding clinical judgment skills. One way to teach clinical judgment is to deliberately provide cues to guide the student into making the correct decision. The purpose of this randomized control pilot study was to evaluate the effectiveness of using the National Early Warning Score (NEWS) as a deliberate set of cues during high-fidelity simulation to guide the student in the development of clinical judgment, and the early detection of patient deterioration. A single convenience sample of 57 junior level baccalaureate nursing students was recruited. Twenty-eight students were randomized into the treatment group and 29 into the control group. The students were then randomly placed into teams of 2-3 students each, resulting in an N of 14 in both the treatment and control groups. The treatment group was educated on the use of the NEWS tool. The research evaluated the differences in clinical judgment scores and speed of detection of patient deterioration during high-fidelity simulation between students trained in the use of the NEWS tool versus those who were not. No significant differences in elapse of time or clinical judgment were detected between the treatment and control groups; however, the data was suggestive of a trend towards increasing clinical judgment. Findings from this study confirm the need for further research to be done in this area to determine if utilizing the NEWS tool during high-fidelity simulation can enhance clinical judgment, resulting in early detection of patient deterioration.
