High fidelity simulation is important to nurse clinical educators as an alternate method to provide experiences that assimilate, expand, and enhance their self-confidence. National League for Nursing states that nurses' self-confidence improves the performance level in life saving conditions (National League for Nursing, 2013). The problem statement of this Direct Practice Improvement (DPI) project was the following: It is not known to what extent using high fidelity simulation in adult cardiac life support (ACLS) renewal courses increase the self-confidence of nursing staff. The purpose of this quantitative project was to determine if high fidelity simulation increases nurses' confidence level while performing ACLS. The clinical question was the following: Does using high fidelity simulation in ACLS renewal classes increase nursing staff's confidence level and improve their performance at work? The theoretical foundation for this project incorporated Benner's Novice to Expert model. The design of this DPI project was a pre- and post- survey. A paired t-test was used to analyze the data. The population was 64 registered nurses who self-enrolled for an ACLS class using simulation. Statistical findings of this project suggest that high fidelity simulation increases the self-confidence level of the nurses.

Book excerpt: Nursing educators are increasingly utilizing high-fidelity simulation to provide realistic educational experiences to prepare students to enter the workforce. High-fidelity simulation is utilized with the belief when students encounter real-life patients they will be more confident in their ability to care for them. This quantitative study examined the effects of high-fidelity simulation on senior nursing students perceived self-efficacy and perceived practice readiness. The sample (N=48) consisted of senior associate degree nursing students at a community college in the Southeast. The findings showed high-fidelity simulation had a positive effect on students perceived self-efficacy and perceived practice readiness at certain points in the study, while other times not demonstrating a positive correlation. Implications for use of simulation as a teaching strategy in nursing education and recommendations for future research were proposed.

High-fidelity simulation (I-IFS) is often used in nursing schools, as well asthe hospital education setting, as it provides a safe environment forparticipants to practice clinical skills. The purpose of this Doctor of Nursing Practice (DNP) Evidence-Based Practice (EBP) project was to evaluate the impact of a high-fidelity mock code simulation intervention on NGN's self-confidence and anxiety while responding to a deteriorating patient. The I-IFS interventionwas conducted with 10 new graduate nurse participants at the project facility using the Nursing Anxiety and Self-Confidence with Clinical Decision Making Scale (NASC-CDM) © created by Dr. Krista A. White. The pre- and postself-confidence for Dimension 2 was statistically significant (alpha = 0.05, (9) = -2.43, p =-038) and the pre- and post-anxiety for Dimension 3 was statistically significant (alpha = 0.05, (9) = 3.11, p = .013). While the participants felt additional clinical experience and HFS was needed to improve self confidence andanxiety when responding to a deteriorating patient, HFS intervention can beused to improve clinical performance. The implications for practice indicate more hands-on practice is needed to increase self-confidence and decrease anxiety. The implications for education include more high-fidelity simulation for new graduate nurses and additional clinical practice to increaseself confidence and decrease anxiety. Implications for research indicate more research is needed using high-fidelity simulation related to training new graduate nurses to decrease anxiety and increase self-confidence.

Book excerpt: Nursing programs are increasingly facing issues with access to appropriate clinical sites, access to properly credentialed nursing faculty, and concerns regarding graduates readiness to practice safely. These issues have led to increased use of high fidelity simulation (HFS) experiences to replace traditional clinical rotations (Benner, Sutphen, Leonard & Day, 2010; Lasater, 2007a; Orsolini-Hain & Malone, 2007). Previous research has shown that traditional clinical rotations are instrumental in development of clinical reasoning skills and HFS experiences are valuable in developing technical skills. However, it does not provide insight into the impact of HFS experiences on development of clinical reasoning skills (Lapkin, Levett-Jones, Bellchambers & Fernandez, 2010). The purpose of this quasi-experimental pilot study was to determine the relationship between HFS clinical experiences and development of clinical reasoning skills in nursing students. Findings will influence the use of HFS to replace traditional clinical experiences and provide a foundation for future research on the efficacy of HFS in development of clinical reasoning skills. Analysis of data showed that there was no significant statistical difference in development of clinical reasoning skills between prelicensure nursing students attending traditional clinical experiences and those participating in simulated clinical experiences. Research has added to the knowledge regarding simulation development, evaluation, and implementation. The purpose of this quasi-experimental study was to add to the knowledge base regarding the influence of high-fidelity simulation on gain and retention of knowledge. Forty-four junior level baccalaureate nursing students participated in either a lecture only (n = 19) or lecture with simulation (n = 25) experience. The students were tested pre-intervention, post-intervention, and at 4 months utilizing a multiple choice knowledge evaluation test that was developed by the researchers. The results of this study demonstrate that although there was significant learning with the simulation and lecture experience over the lecture only experience (t = -3.39, df = 29, p = .002), there was not significant impact on the 4 month retention of knowledge between the groups (t = -.30, df = 42, p = .766). Simulation design was tested through the National League for Nursing simulation tools, including the: Simulation Design Scale, Educational Practices in Simulation Scale, and the Student Satisfaction and Self-Confidence in Learning Scales. The scenario was found to be well developed as compared to other pulished studies.

Book excerpt: The purpose of this study was to evaluate and describe how a realistic and evidence-based simulation training on clinical skills impacts nursing students' proficiency, self-assessment of proficiency, confidence, enjoyment, and anxiety. The study explored the impacts this simulation training program had on FSU's nursing students enrolled in 2nd and 3rd semesters. A total of 170 nursing students participated in this study. Different modalities of simulation equipment (task trainers and high fidelity mannikins) were utilized to achieve the learning objectives of each skill taught during this training. The design of this skills training follows the Simulation Design standard of best practice by INACSL (Watts and others, 2021). Questionnaires with Likert scale and open-text responses were used to collect data on the students' self-reported proficiency, confidence, enjoyment, and anxiety. Achievement of skill proficiency was measured by faculty using checklists for each skill taught. I served as both the researcher and the designer of the boot camp skills course.The results of this study indicated that most nursing students who participated in the simulation training achieved proficiency in the clinical skills taught. Results also revealed that self-reported proficiency, confidence, and enjoyment significantly increased for all skills taught in the training program. However, the findings did not reveal a statistically significant difference between the pre and post-simulation means for self-reported anxiety. Introducing a simulation boot camp for skills practice before students start clinical rotations seems like a good adjunct that supplements clinical practice. Students enjoyed the training, receiving feedback, and practicing in a simulated environment resembling reality. However, it remains to be seen whether the skills learned in this boot camp transfer into real-world practice. More research is needed to evaluate the transferability of skills acquired through simulation into clinical practice.
