Book excerpt: The Laboratory of Hyperbaric Physiology of the Medical Clinic of the University of Zurich came into existence in 1960 thanks to private initiative and a readiness to undertake risks; the success ful start was made possible with help from the French Navy and the United States Navy. A prerequisite for the development of the laboratory was also the benevolence of the authorities of the University of Zurich toward a research project from which scarcely any practical use could be expected for the land-locked country of Switzerland. The development of the laboratory and the systematic research were supported generously from 1964 by Shell Intemationale Petroleum Maatschappij of The Hague. The basic theme of the research was always the well-being and functional ability of the human being in an atmosphere of abnor mal pressure and or abnormal composition. Many connections became obvious with respiratory physiolo gy, circulatory physiology, and physiology at great heigts, and close contact with other special laboratories of the Medical Clin ic proved very valuable. With a relatively small number of steady collaborators it was possible to master an extensive experimental program. Special thanks are due to Mr. Benno Schenk, who as technical head was responsible for the exact performance of all the hyperbaric experiments.

Excerpt: The decompression of a diver is the reduction in ambient pressure experienced by the diver during ascent, and also the process of elimination of dissolved inert gases from the diver's body, which occurs both during the actual ascent, during pauses in the ascent known as decompression stops, and after surfacing, until either the gas concentrations reach equilibrium, or another dive is started. When a diver descends in the water the hydrostatic pressure, and therefore the ambient pressure, rises. Breathing gas is supplied at the same pressure as the surrounding water, and some of this gas dissolves into the diver's blood, and is transferred by the blood circulation to other tissues. Inert gas continues to be taken up until the gas dissolved in the diver is in a state of equilibrium with the breathing gas in the diver's lungs, (see: "Saturation diving"), or the diver moves up in the water and reduces the ambient pressure of the breathing gas until the inert gases dissolved in the tissues are at a higher concentration than the equilibrium state, and start diffusing out again. Divers breathing gas at high pressure may need to do decompression stops, but a diver who only breathes gas at atmospheric pressure when free-diving, does not usually need to do decompression stops. However, it is possible to get taravana, thought to be a form of decompression sickness, from repetitive deep free-diving with short surface intervals. Divers who only use a snorkel near the surface or use an atmospheric diving suit will not need to decompress. Book excerpt: The additional decrease in ambient pressure which occurs when a compressed air diver flies in an aircraft within a short time after diving may be sufficient to precipitate decompression sickness, even though the dive itself was in accordance with the U.S. Navy decompression tables. The current practice by both military and civilian divers of using air transportation after compressed air diving suggests the need for specific instructions regarding the decompression required before flying after diving. In order to quantitate the importance of this problem, an experiment was designed in which large dogs were exposed to compressed air for 7 hours at their 'no-bends' pressure threshold as determined after the method of Reeves and Beckman. After pressurization, the animals were decompressed within 2-3 minutes to sea level. A sea level decompression interval of 1, 3, 6, or 12 hours was given prior to further decompression to a simulated altitude of 10,000 feet. The incidence of decompression sickness at altitude was 92.9% for the 1 hour surface decompression interval, 30% for the 3 hour interval, 27.8% for the 6 hour interval and 0% for the 12 hour interval. From these large animal studies it may be postulated that a surface decompression interval of at least 12 hours should be allowed before flying after compressed air diving of a depth and duration to require the use of diving tables.

Book excerpt: River otters (Lontra canadensis) are highly active, semi-aquatic mammals indigenous to a range of elevations and represent an appropriate model for assessing the physiological responses to diving at altitude. In this study, we performed blood gas analyses and compared blood chemistry of river otters from a high-elevation (2357 m) population at Yellowstone Lake with a sea-level population along the Pacific coast. Comparisons of oxygen dissociation curves (ODC) revealed no significant difference in hemoglobin-oxygen (Hb-O(2)) binding affinity between the two populations - potentially because of demands for tissue oxygenation. Instead, high-elevation otters had greater Hb concentrations (18.7 g dl(-1)) than sea-level otters (15.6 g dl(-1)). Yellowstone otters displayed higher levels of the vasodilator nitric oxide (NO), and half the concentration of the serum protein albumin, possibly to compensate for increased blood viscosity. Despite compensation in several hematological and serological parameters, theoretical aerobic dive limits (ADL) were similar between high-elevation and sea-level otters because of the lower availability of O(2) at altitude. Our results suggest that recent disruptions to the Yellowstone Lake food web could be detrimental to otters because at this high elevation, constraints on diving may limit their ability to switch to prey in a deep-water environment.
