While a considerable volume of research has analysed the possible significance for health infrastructures and individuals for common conditions in the elderly such as cardiovascular disease and diabetes, the impact of an increase in hearing impairments amongst an ever increasing ageing population has been under studied in this country. Prior research in this field has shown a significant positive relationship between the severity of hearing loss and the experience of disability and hearing handicap (1-4). Negative consequences of hearing loss in the elderly are known to include depression (5), social isolation and withdrawal (6). Restrictions to participation and disadvantages in participation due to hearing loss have been shown to affect overall quality of life (2, 4). Rehabilitation with amplification can greatly improve the negative effects of hearing handicap in this population (1, 7). However hearing aids are to a large extent under utilised by this age group (8, 9). The current study used a qualitative approach to investigate the effects of hearing loss on a group of elderly living independently in the Auckland community. The study was looking to understand the ways that hearing loss is a part of elderly people's lives, as seen from their perspective. Also of interest were the coping mechanisms that people in this group employ to help them with hearing difficulties. It is hoped that gaining information and understanding of elderly people's perspectives on their hearing handicap and disability will contribute to enhancing a hearing health professional's ability to better treat this age group. Interviews and hearing screens were carried out in the participant's home, and later analysed using a general inductive approach (10) to qualitative analysis, which categorizes data into themes within a hierarchy. Results indicated that complex interactions between hearing loss and other aspects of a persons daily life influence the experience of hearing impairment and the effect that it can have on self perceived quality of life. Future research is needed in this area to further understand the experience of elderly with hearing loss both in the community and in care settings, and to address the significant under use of hearing aids that can reduce disability in this age group.

Book excerpt: The loss of hearing - be it gradual or acute, mild or severe, present since birth or acquired in older age - can have significant effects on one's communication abilities, quality of life, social participation, and health. Despite this, many people with hearing loss do not seek or receive hearing health care. The reasons are numerous, complex, and often interconnected. For some, hearing health care is not affordable. For others, the appropriate services are difficult to access, or individuals do not know how or where to access them. Others may not want to deal with the stigma that they and society may associate with needing hearing health care and obtaining that care. Still others do not recognize they need hearing health care, as hearing loss is an invisible health condition that often worsens gradually over time. In the United States, an estimated 30 million individuals (12.7 percent of Americans ages 12 years or older) have hearing loss. Globally, hearing loss has been identified as the fifth leading cause of years lived with disability. Successful hearing health care enables individuals with hearing loss to have the freedom to communicate in their environments in ways that are culturally appropriate and that preserve their dignity and function. Hearing Health Care for Adults focuses on improving the accessibility and affordability of hearing health care for adults of all ages. This study examines the hearing health care system, with a focus on non-surgical technologies and services, and offers recommendations for improving access to, the affordability of, and the quality of hearing health care for adults of all ages.
