Book excerpt: A cross sectional study looked at the association between sociodemographic factors, HIV/AIDS clinical history and health-related quality of life (QOL) in HIV/AIDS persons. It used the WHOQOL-BREF-THAI questionnaire completed by 3,596 patients during 2008 in three Thai hospitals. The questionnaire explored five QOL domains: Physical, Psychological, Social, Environment and Overall and it was for the first time administered to many HIV/AIDS patients in Thailand. Descriptive statistics and inferential statics (bivariate and multivariate : one-way ANOVA, Kruskal-Wallis Test, Pearson Correlation and independent ttest)) were used to test association between independent factors and QOL. Results: The following associations were all statistically significant. Males had higher QOL than females in Physical, Psychological and Overall domains. Younger age was associated with higher QOL in Physical and Social domains, older age in Psychological and Environment domains. Married had higher psychological QOL than single and separated. Good adherence to, more than 3 year duration of ARV treatment and not suffering AIDS related events were associated with higher QOL in all except social domains. VL

Book excerpt: The introduction of new medical treatments in recent years, commonly referred to as highly active antiretroviral therapy, has greatly increased the survival of patients with HIV/AIDS. As patients with HIV/AIDS continue to live longer, other important health-related outcomes, such as quality of life (QOL), should be thoroughly studied. There is also evidence that racial/ethnic minorities are disproportionately affected by HIV/AIDS, but potential health disparities among individuals already infected with HIV/AIDS have not been adequately examined in ethnically diverse populations. The purpose of this dissertation was to: (1) examine the impact of both demographic and behavioral variables on functional status and overall QOL among a population of ethnically diverse and economically disadvantaged HIV/AIDS patients; (2) examine the psychometric properties of a functional status measure--the Household and Leisure Time Activities questionnaire (HLTA); and (3) assess a proximal-distal theoretical framework for QOL using a full structural equation model in a population of patients with HIV/AIDS. Analyses were performed using data collected in the fall of 2000 from the project, Health and Work-Related Quality of Life and Health Risk Behaviors in a Multiethnic HIV-positive Population . Investigators from The University of Texas M.D. Anderson Cancer Center, The University of Texas-Houston Medical School, and The University of Texas School of Public Health conducted this project. The study site was the Thomas Street Clinic (TSC), a comprehensive HIV/AIDS care facility funded by the Harris County Hospital District (HCHD). TSC provides HIV/AIDS care to a diverse population of approximately 4000 medically indigent residents of Harris County. A systematic, consecutive sampling procedure yielded a sample size of 348 patients. Findings suggested that overall QOL, work-role functioning, household functioning, and leisure time functioning were impaired in this patient population. Results from the psychometric evaluation indicated that the HLTA was a reliable and valid measure of household and leisure time functioning status in a low-income multiethnic HIV-positive population. Finally, structural equation modeling of the proximal-distal QOL model suggested that this model was not a viable representation of the relationship between the study variables in this patient population.

Book excerpt: Background: Clinical care for people living with HIV (PLWH) increasingly incorporates quality of life considerations due to advances in HIV treatment and decline in HIV-related mortality. Men who have sex with men (MSM) and transgender women (TW) constitute the majority of new HIV infections in Lima, Peru, and there is currently limited research evaluating health-related quality of life (HRQOL) determinants in these populations. Methods: We analyzed differences in health-related quality of life between HIV-positive participants from the Sables study who were randomized to either initiate antiretroviral therapy (ART) immediately or to defer ART for 24 weeks after HIV diagnosis. Items from the Short Form (SF)-12 survey were evaluated as individual HRQOL outcomes, and bivariate and multivariate logistic analyses were used to assess the relationship between HRQOL, demographic characteristics and baseline CD4 count. Results: The results of bivariate analyses indicated that participants who initiated ART immediately were more likely to report better general health at 48 weeks post HIV diagnosis (prevalence ratio (PR): 1.11, 95% confidence interval (CI) [1.01, 1.21]). Multivariate analyses revealed participants who reported higher HIV-related aversive coping at 48 weeks were more likely to report more limitations with moderate physical activities (adjusted prevalence ratio (APR): 0.86, 95% CI [0.77, 0.97]) and with climbing several flights of stairs (APR: 0.89, 95% CI [0.80, 0.99]), increased limitations with social functioning (APR: 0.70, 95% CI [0.58, 0.85]), elevated psychological distress (APR: 0.73, 95% CI [0.58. 0.92]), and more role limitations due to physical health (APR: 0.89, 95% CI [0.80, 0.99]). Baseline CD4 count of 500 cells/uL or higher was found to be associated with less psychological distress (APR: 1.36, 95% CI [1.01, 1.83]) and less interference on normal work due to pain (APR: 1.37, 95% CI [1.01, 1.88]). Conclusions: Among HIV-positive MSM and TW participants in Lima, Peru, self-perceived general health status at 48 weeks was greater in the immediate ART arm compared to those who deferred ART for 6 months after HIV diagnosis. One year after HIV diagnosis, higher HIV-related aversive coping was associated with worse HRQOL outcomes. Higher CD4 count at diagnosis was associated with less psychological distress 1 year after HIV diagnosis.

All HIV-related stigma scale scores were inversely associated with social capital indicators and with physical and mental component summary scores, after controlling for potential confounders. These results provide little support for mediation of the relationship between formal employment status and HRQoL among PLHIV by social capital and HIV-related stigma. Both social capital and HIV-related stigma have independent relevance to, but formal employment accounts for the largest effect on the health and well-being of PLHIV.
