Book excerpt: The Health Coverage Tax Credit (HCTC) pays 65 percent of health insurance premiums for about 16,000 trade-displaced workers, early retirees receiving payments from the Pension Benefit Guaranty Corporation, and their families. It is the country's only use of refundable federal income tax credits to cover the otherwise uninsured. Its success or failure matters, not just to the households depending on the program for health coverage, but also to broader national trade and health policy. Offering tax credits to the uninsured is often cited as one way to provide more Americans with health coverage. The concept is popular with many lawmakers; several Republican and Democratic presidential candidates have proposed such tax credits. Yet, HCTC faces significant challenges, including limited participation due to affordability, liquidity, complex enrollment and limited coverage and high administrative costs. Just 15 percent of eligible workers and their families participate. In this Urban Institute policy brief, health policy expert Stan Dorn analyzes how these tax credits can be restructured to reach more workers who qualify and how future tax credits could be designed to serve millions of uninsured Americans more effectively. Congress is considering changes to HCTC as part of Trade Adjustment Assistance reauthorization. This brief describes these congressional proposals and suggests additional approaches both to address HCTC's problems and to prevent similar difficulties with future tax credits aimed at a larger group of uninsured Americans.

Book excerpt: "Why GAO Did This StudyThe HCTC pays 72.5 percent of health plan premiums for certain workers who lost their jobs due to foreign import competition and for certain retirees whose pensions from their former employers were terminated and are now paid by the Pension Benefit Guaranty Corporation. A small share of individuals who are potentially eligible for the HCTC participate. In 2010 there were 43,864 participants and 469,168 nonparticipants. The HCTC program will expire at the end of 2013 when premium tax credits and cost-sharing subsidies become available to eligible individuals who purchase health plans through health insurance exchanges under PPACA. PPACA also expands Medicaid eligibility to nonelderly individuals who meet specific income requirements to the extent that states choose to implement this provision. Book excerpt: As part of the National Evaluation of the Trade Adjustment Assistance (TAA) Program, this report provides information about the characteristics and early program experiences of workers eligible for TAA under the 2002 Trade Act. Information was collected via a telephone survey of a nationally representative sample of workers in 26 states (covering 90 percent of TAA eligible workers). Interviews were conducted both with participants in TAA and with "nonparticipants" who were eligible for, but did not receive, TAA services or benefits. Compared with other displaced manufacturing workers, TAA-eligible workers on average were more highly paid and were more likely to be full-time workers with long tenure at their previous job. Only half of TAA-eligible workers participated in the program with participation tending to be higher among women, older workers, and workers with less education. Almost all (98 percent) TAA participants received a Trade Readjustment Allowance (TRA) providing weekly income support, but only 60 percent of TAA participants enrolled in training. Receipt of the Health Coverage Tax Credit, a new provision under the 2002 Act, was uncommon. Only 14 percent of TAA participants used this credit. The participation rate was also low for another new component of the 2002 bill, the Alternative Trade Adjustment Assistance (ATAA) program, which provided a subsidy for individuals taking a job paying less than their former job. Only four percent of participants age 50 and older used ATAA, due primarily to difficulties in finding a job and its less attractive benefits as compared to TRA.
