As such, though not exhaustive, the book does represent an anthology of contemporary neurosurgeons. From the preface: At the very beginning of the project, our intention was to make a “poetbook”. But month after month it became obvious that the work would be much more expansive; ultimately we produced three volumes. Nevertheless we hope that all the three volumes together will remain easily accessible and a daily companion. The pocket has to be more like a travel bag! We would like to thank all of the contributors; they have sacrficed their valuable time to deliver sound and critical views, and above all useful guidelines.

The Atlas is based on the Handbuch der Neurochirurgie, Vols. I and III (Springer 1956, 1959) but, whereas this is a comprehensive reference work, the present book is intended to give the practicing neurosurgeon, neuroradiolo gist, neuropathologist and neurologist the concise information they need for diagnostic purposes concerning the aspect, site, and ma lignancy of tumors and other space-occupying lesions in the brain. The schematic diagrams showing the sites of predilection of these tumors, as well as a piOgnosis based on the degree of malignancy, will be most useful here. The early chapters discuss the general rules governing displace ments due to space-occupying lesions and the manifestations of brain herniations. Other neurosurgical conditions, such as localized inflammatory processes, edema and obstructive hydrocephalus, are dealt with in brief chaptets; in this case I have chosen to show some of the rarer conditions rather than all the common lesions. In spite of probable future changes in terminology and classification, we have retained the classification used in the Atlas of Histology of Brain Tumors.

Of 150 papers submitted, the program com mittee of the Society chose 69 for presentation. I would like to thank Professors Bock, Brock, Jensen, Wenker, and Wlillenweber for their as sistance in the selection. It was the intention of the President of the Congress at this meeting to lay special scientific emphasis on discussion. For this reason the number of papers had to be limited even more than usual in order to give all participants the opportunity for questions and the presenta tion of their own experience. The main topic of the first day was Extra-Intracranial Anastomoses. Here the operative experience and the long-term results of these operations in the German-speaking countries were reviewed. This discussion was preceded a day earlier by a satellite symposium on the possibilities of the prophylaxis and treatment of ischemic neurological deficits fol lowing subarachnoid hemorrhage by means of calcium antagonists. Microsurgery at the Edge of the Tentorium was the second main topic. In co operation with the SOCiety's microneurosurgery working group, only a small number of lectures were planned, but the topic was covered thor oughly in a round-table discussion.

Book excerpt: The cerebello-pontine angle has always posed a challenge to the neurosurgeon, the otoneurosurgeon, and the neuroradiologist. Angle masses which are very small and difficult to detect frequently produce symptoms, but may remain silent while growing to exceptional size. The neuroradiologist must have firm knowl edge of the clinical manifestations of the diverse angle lesions in order to tailor his studies to the patients' needs. The majority of angle lesions are benign; thus successful surgery has the potential for complete cure. Angle lesions typically arise in conjunction with vital neurovascular structures, and often displace these away from their expected positions. Large lesions may attenuate the vestibulocochlear and facial nerves and thin them over their dome. Since the nerves often remain functional, the surgeon then faces the need to separate the tumor from the contiguous nerve, with preservation of neurological function. Depending on the exact location and extension of the lesion, resection may best be attempted via otologic or neurosurgical approaches. The neuroradiologist must determine - precisely -the presence, site, size, and extension( s) of the lesion and the displacement of vital neurovascular structures as a guide to selecting the line of surgical attack. Since the arteries, veins, and nerves that traverse the angle are fine structures, the neuroradiologist must perform studies of the highest quality to do his job effectively.

This 25th annual meeting will deal with two examples of diseases that present common problems to the neurologist and to the neurosurgeon, namely meningiomas and multiple sclerosis. In view of their long histories often going back over many years, both of these lesions lead to diagnostic errors and indequate treatment. And yet it should be possible to recognize meningiomas at an early date and to initiate the only possible treatment, the operation,if all diagnostic measures are repeatedly carried out. The diagnosis MS, on the other hand, with the multiplicity of symptoms which are peculiar to this disease, should continue to be re-examined until every other lesion has been excluded with certainty. The increasing number of legal proceedings because of diagnostic and therapeutic measures as well as the doctor-patient talk preceding the written consent for these measures are further problems in need of discussion. For this reason, the topic "medical liability in special reference to the neurosurgeon" was chosen for this meeting. Many questions necessitate many answers.

Book excerpt: The basic principles of the management of cerebral arteriovenous mal formations were established during the first phase of the neurological attack on these problems between 1930 and 1960. The leaders were CUSHING, BAILEY and DANDY, but principally OUVECRONA, and in Ger many TONNIS. The experience gained showed that complete excision of the arteriovenous angioma was the only certain cure, and therefore was the procedure of choice. In the present second phase important advances should be made and indeed are occurring. New diagnostic techniques such as total angio graphy, selective and superselective angiography, intraoperative and fluorescein angiography, and the EMI-scanner have been developed. The pathophysiological aspects have been further investigated by indirect and direct measurement of local and general cerebral blood flow. Parallel with these developments operative technique itself has been improved and modified by new methods. A more aggressive attitude has been stimulated towards those angiomas, which had to be regarded as inoperable only a few years ago. Among these many im provements and technical advances include microsurgical techniques, combined stereotactic and microsurgical procedures, artificial emboliza tion of different kinds and the cryosurgical management. Multiple variables such as the age of the patient, the type, localization, and size of the angioma, its clinical picture and the possible complica tions, such as hemorrhage have been analysed and are understood better. These factors influence the indication for, and choice of, the appropriate procedure to a great extent.

I truly believe that the articles presented in this book cover so many interesting subjects concerned with peripheral nerve lesions that the book will serve the interested and dedicated physician involved with such cases as a reference work for the basics and also provide him with the therapeutic guidelines to assist him in his daily work.

I wish to acknowledge once again the excellent translation of the former two editions by Dr. ALAN B. ROTHBALLER and the late Dr. JERZY OLSZEWSKI. With this edition I have followed the general theme of the original German edition published in 1951. However, I have tried to consider modern techniques and the many new publications on the subject of brain tumors. Meanwhile, an early desire of mine has been fulfilled by the completion and publication of a classification which can be understood worldwide and hopefully be used widely, namely, the classi fication of the World Health Organization: Histological Typing of Tu mours of the Central Nervous System (1979). The classification which I used in the 1951 edition is very close to the final pattern of that accepted by the World Health Organization (WHO), since both follow the line of the BAILEY and CUSHING classifica tion of 1926/1930. To consolidate our old concepts and experiences we have reclassi fied our collection of 9000 cases with the assistance of my co-workers Dr. M. FUKUI, Dr. A. SATO. Dr. E. SCHARRER, Dr. E. SIMON, and Dr. J. SZYMAS. In the last decade two large atlases have been published, one called an Atlas of the Histology of Brain Tumors 1 (in six languages) and a second one called an Atlas of the Gross Neurosurgical Pathology 2.

Since at recent meetings of the German Neurosurgical Society central pathophysiological problems such as "central dysregulation" and "brain edema" had been discussed extensively, it seemed appropriate to choose another major area of cerebral patho physiology for the meeting in Heidelberg. CEREBRAL HYPOXIA is, as LANGFITT once emphasized, "the final common denominator" of various cerebral lesions with which the neurosurgeon is confronted every day. Raised intracranial pressure, respiratory disorders and disturbances in systemic arterial blood pressure, etc. may lead, if not treated, to a focal or global lack of oxygen in the brain tissue. Anoxia finally results in cell death and thus in irreversible cerebral damage or even death. Main interest has therefore been focussed on disturbances in cerebral perfusion pressure ("ischemic hypoxia") and in arterial oxygenation ("hypoxic hypoxia"). The importance of cerebral autoregulatory mechanisms protecting the brain against tissue hypoxia, of patho morphological alterations of the cerebral vessels (e. g. the "no-reflow-phenomenon") in the course of severe hypoxia, and of changes in brain metabolism have been discussed on a large scale. The organizing committee was particularly happy to have obtained internationally well-known scientists who presented their work in the field of cerebral hypoxia.
