Book excerpt: Numerous claims of illnesses from Gulf War veterans and inconsistent reports on veterans' exposure to risk factors have prompted a number of government studies on the nature, extent, and treatment of Gulf War illnesses. In June of last year, we reported to you and other members of Congress that government research efforts may not be able to provide conclusive answers about the causes of veterans' illnesses and that several risk factors such as chemical and biological agents had been ruled out prematurely as possible explanations for the reported illnesses. We also reported that many veterans were unhappy with the quality of health care treatments they had received from the Department of Veterans Affairs (VA) and that enhanced monitoring is needed to determine if veterans are getting any better or worse over time.

Book excerpt: Based on our completed study, we found that none of the data sources providing information on the health of Gulf War veterans can be used to reliably estimate the incidence of tumors or other illnesses. Existing government data systems are generally limited by poor coverage of the Gulf War veteran population and problems of accuracy and completeness. As a result, it is not known how many Gulf War veterans have tumors or whether they have a higher incidence of them than other veterans. DOD and VA have begun efforts to improve these data systems but have not developed the capability to effectively link information from different sources to assess tumors or other illnesses among Gulf War veterans. DOD and VA also recently funded a few research studies that should provide additional information on tumor cases in the future. However, these studies are not the product of a systematic effort to study the incidence of tumors, and limitations to the studies will prevent them from providing reliable and valid estimates of Gulf War veterans' tumors.

Book excerpt: Many individuals, groups, and federal agencies have a strong interest in finding answers to the numerous and complex questions regarding the health of Gulf War veterans. Various types of research and health measurement are needed to address these diverse issues. The Institute of Medicine (IOM) was asked by the Department of Veterans Affairs (VA) and the Department of Defense (DoD) to undertake a study to identify important questions concerning the health of Gulf War veterans and then to design a study to answer those questions. The committee determined that it is of fundamental importance to ask how healthy are Gulf War veterans? Are they as healthy as others? What characteristics are associated with differences between the health of Gulf War veterans and the health of others? To address these questions, it will be necessary to measure not only the health status of those who served in the Gulf War, but also to compare Gulf War veterans with other groups. Further, one must continue to follow these groups through time to determine whether the groups differ in the way their health status is changing. As the committee began to develop a design that would address the fundamental questions identified, it realized that such a study could have important implications for understanding not only the health of Gulf War veterans, but also the health of veterans of other conflicts.
