Book The Implementation of a Palliative Approach in Residential Aged Care

Download or read book The Implementation of a Palliative Approach in Residential Aged Care written by Bridget Mary Joan Yalden and published by . This book was released on 2010 with total page 444 pages. Available in PDF, EPUB and Kindle. Book excerpt: Data was generated from multiple sources in the cyclical process of the inquiry. It involved members of the aged care team, residents and families and decision-makers at different levels of the health care organisation in which the facility was located. Reflexive analysis using thematic mapping resulted in the translation of knowledge from the evidence-based guidelines, participants and literature that shaped the inquiry and creative and reflective methods that helped make sense of the meaning of a palliative approach and how it was incorporated into practice. The results demonstrate how the emergent strategies created the underlying conditions that enabled the implementation of a palliative approach to care and the translation of the guidelines as part of the inquiry and transformative processes. The key strategies included an integrative approach and a model of active learning that enabled teamwork, communication and holistic end-of-life care. The study methods raised awareness of the underlying philosophical and practical intent of a palliative approach within the normative pattern of care and living and dying trajectories of residents. Through practice development facilitation, they empowered participants to build trusting relationships that resulted in enlightened ways of working as a multidisciplinary aged care team. The findings are significant because they reveal the underlying conditions that are crucial to enable sustainable change in this environment. They inform practitioners, policy makers, strategic planners and transformative leaders because they describe strategies and methods that enable successful implementation and translation of evidence into practice that are in harmony with the values, beliefs, resources and policies in this context. They contribute to practice development knowledge and implementation science in explicating selected methodological principles that support critical, creative and participative inquiry methods. Overall, the findings add to the body of knowledge about implementing quality palliative care, making it accessible to residents and their family in residential aged care.

Book Living with Ageing and Dying

Download or read book Living with Ageing and Dying written by Merryn Gott and published by OUP Oxford. This book was released on 2011-05-12 with total page 551 pages. Available in PDF, EPUB and Kindle. Book excerpt: Ageing populations mean that palliative and end of life care for older people must assume greater priority. Indeed, there is an urgent need to improve the experiences of older people at the end of life, given that they have been identified as the 'disadvantaged dying'. To date, models of care are underpinned by the ideals of specialist palliative care which were developed to meet the needs of predominantly middle-aged and 'young old' people, and evidence suggests these may not be adequate for the older population group. This book identifies ways forward for improving the end of life experiences of older people by taking an interdisciplinary and international approach. Providing a synergy between the currently disparate literature of gerontology and palliative care, a wide range of leading international experts contribute to discussions regarding priority areas in relation to ageing and end of life care. Some authors take a theoretical focus, others a very practical approach rooted in their clinical and research experience. The issues covered are diverse, as are the countries in which discussions are contextualised. Those working in both palliative care and gerontology will find the issues and advice discussed in this book hugely topical and of real practical value.
