Book excerpt: Surgical site infections are caused by bacteria that get in through incisions made during surgery. They threaten the lives of millions of patients each year and contribute to the spread of antibiotic resistance. In low- and middle-income countries, 11% of patients who undergo surgery are infected in the process. In Africa, up to 20% of women who have a caesarean section contract a wound infection, compromising their own health and their ability to care for their babies. But surgical site infections are not just a problem for poor countries. In the United States, they contribute to patients spending more than 400 000 extra days in hospital at a cost of an additional US $10 billion per year. No international evidence-based guidelines had previously been available before WHO launched its global guidelines on the prevention of surgical site infection on 3 November 2016, and there are inconsistencies in the interpretation of evidence and recommendations in existing national guidelines. These new WHO guidelines are valid for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.

Book excerpt: Infections in surgery, commonly known as surgical site infections (SSIs), are complications that may occur after a surgical procedure. SSIs represent a serious problem leading to increased morbidity, mortality, and healthcare costs, highlighting the need for continued efforts to improve surgical practices and reduce their incidence. Several factors can contribute to the development of SSIs: patient-related factors (such as obesity, advanced age, diabetes, immunosuppression, pre-existing infections etc.); preoperative preparation-related factors (such as incomplete skin antisepsis or failure to administer appropriate antibiotic prophylaxis); microbial contamination (despite sterile techniques, microorganisms can infect the surgical site); surgical procedure-related factors (duration and complexity of the surgeries); postoperative care-related factors (such as inadequate wound care or infection control measures). To prevent SSIs, healthcare facilities implement various strategies, including: optimization of patients’ conditions before surgery; preoperative antibiotic prophylaxis; strict sterile technique; postoperative wound care; surveillance and monitoring to identify and address SSIs early, preventing their spread and complications. Despite the evidence supporting the effectiveness of best practices, many clinicians fail to implement them, and evidence-based practices that optimize both the prevention and treatment of SSIs tend to be underused, highlighting the importance of ongoing research and improvement in surgical techniques and infection control practices. This open access book provides a practical toolkit for surgeons and intensivists to improve their daily clinical practices in order to reduce the risk of SSIs.

Book excerpt: The first ever Global guidelines for the prevention of surgical site infection (SSI) were published on 3 November 2016, then updated in some parts and published in a new edition in December 2018. They include a list of 29 concrete recommendations on 23 topics for the prevention of SSI in the pre-, intra and postoperative periods, which are based on 28 systematic reviews of the evidence. For the 2018 update, the membership of the guidelines development group (GDG) was broadened to include an additional eight anaesthesiology experts. The 2018 edition of the guidelines includes the revision of the recommendation regarding the use of 80% fraction of inspired oxygen (high FiO2) in surgical patients under general anaesthesia with tracheal intubation and the update of the section on implementation. Between 2017 and 2018, WHO re-assessed the evidence on the use of high FiO2 by updating the systematic review related to the effectiveness of this intervention to reduce SSI and commissioning an independent systematic review on adverse events potentially associated with it. Based on the updated evidence, the GDG decided to revise the strength of the recommendation from strong to conditional.

Les infections du site opératoire (ISO) sont fréquentes (prévalence : 4,5%), graves (létalité : 2,5% à 5%), coûteuses (environ 1 semaine d'hospitalisation supplémentaire, environ 3 000 dollars) et en partie évitables (Senic Project). La législation française, l'évolution jurisprudentielle des responsabilités, l'accréditation et les contraintes budgétaires font de la surveillance épidémiologique des ISO un enjeu de management des risques nosocomiaux. Dans un établissement de santé, l'organisation de la surveillance nécessite une bonne définition préalable des objectifs, des moyens nécessaires et des méthodes envisagées pour les atteindre. L'efficience de la méthode de surveillance choisie dépend en partie du suivi des patients après leur sortie de l'hôpital. La seconde partie de ce travail est une étude descriptive des IS0 diagnostiquées lors de ce suivi. Matériels et méthodes : Nous avons utilisé les données recueillies par INCISO, réseau de surveillance des ISO coordonné par le C-CLIN Paris Nord. Résultats : 15 085 interventions de chirurgie digestive ont été étudiées. La durée moyenne de séjour était de 9,5 jours. 759 ISO ont été diagnostiquées, 35,4% après la sortie de l'hôpital. 57% des ISO à NNIS 0 l'ont été après la sortie. La proportion d'ISO après sortie était inversement proportionnelle à la durée d'hospitalisation et le délai médian de diagnostic était de 12,5 jours. La plupart des facteurs de risque classiques d'ISO étaient associés après analyses unifactorielles, au risque d'ISO après sortie (NNIS index, classe de contamination, score ASA, durée d'intervention, expérience du chirurgien, urgence, chirurgie coelioscopique). Conclusions : Le taux d'incidence des ISO à NNIS 0 est un indicateur de qualité des soins chirurgicaux. Le suivi après sortie des patients opérés est le gage d'une surveillance épidémiologique de qualité procurant des taux d'incidence d'IS0 à NNIS 0 fiable.

Book excerpt: Not everyone is a friend of the manifold abbreviations that have by now beCome a part of the scientific language of medicine. In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
