Or, il faudra attendre 2017 avec la parution du Guide du diagnostic en ergothérapie pour qu'il soit formalisé. Déceler les bénéfices et les freins potentiels à l'utilisation et à la mise en place du diagnostic ergothérapique dans un contexte de réinsertion professionnelle des personnes adultes atteintes de schizophrénie, puis mettre en avant les pistes d'amélioration à explorer. Une étude exploratoire qualitative a été réalisée sous forme d'entretiens semi-directifs guidés par un guide d'entretien auprès de cinq ergothérapeutes. Les informations ont été recueillies dans une grille d'analyse. Les ergothérapeutes mettent en avant la place non négligeable que joue le diagnostic ergothérapique dans la pratique. Toutefois, plusieurs freins sont émis quant à son utilisation, conduisant alors à la mise en lumière de potentiels axes d'amélioration à explorer. De fait, le diagnostic ergothérapique est un élément majeur de la profession. Toutefois, il semble manquer de retours d'expérience sur le terrain qu'il s'agirait d'étudier en vue de la validation du diagnostic ergothérapique.

Pour pallier à cette malformation congénitale, une des solutions est la pollicisation. Suite à cette opération, les enfants sont accompagnés de différentes manières en fonction des institutions. Il n'y a pas de protocole actuellement défini qui fasse consensus en France. L'ergothérapeute a les compétences pour évaluer les conséquences fonctionnelles des déficits de la main et certains travaillent avec des enfants ayant eu une pollicisation. Cependant, en France actuellement seulement 10% des rééducateurs de la main sont des ergothérapeutes. Montrer comment un ergothérapeute peut accompagner un enfant ayant eu une pollicisation à l'aide du diagnostic ergothérapique pour prévenir les répercussions négatives sur l'indépendance de l'enfant. Un essai d'accompagnement en ergothérapie avec pour moyen le diagnostic en ergothérapie basé sur le Modèle Canadien du rendement et de l'Engagement Occupationnel (MCREO) est réalisé dans ce travail. Le diagnostic ergothérapique suivant les trois dimensions, physique, cognitive et affective du MCREO permettrait de proposer un plan d'intervention pour l'enfant afin de prévenir les répercussions sur son indépendance. Pour élaborer la proposition de modélisation, un recensement théorique sur le diagnostic ergothérapique, le MCREO et l'accompagnement des enfants ayant eu une pollicisation a tout d'abord été effectué. Ensuite, 4 ergothérapeutes ont été interrogés. 2 utilisant le MCREO pour réaliser leur diagnostic et 2 travaillant avec des enfants ayant eu une pollicisation. Le but du diagnostic en ergothérapie consiste à faire le lien entre la personne, ses occupations et son environnement avec ou sans modèle conceptuel. Ce diagnostic permet d'avoir une vision holistique de la personne grâce à la diversité des informations prise en compte et sa réalisation en collaboration avec la famille. C' est donc un moyen de répondre à l'accompagnement ponctuel d'un enfant ayant eu une pollicisation bien qu' aujourd'hui ce ne soit pas une approche systématiquement proposée et mise en place. Le diagnostic ergothérapique permet d' accompagner un enfant ayant eu une po11icisation. Sa coconstruction avec l'enfant et ses parents ainsi que sa vision global permettent de compenser l'accompagnement ponctuel de cette population. Cependant cela n'est pas exclusif au MCREO. L' hypothèse est donc à moitié confirmée.
