La restitution du volume osseux par sinus lift, est aujourd'hui la technique de référence pour la réhabilitation implanto prothétique dans les secteurs maxillaires postérieurs, ces greffes sinusiennes sont des opérations pratiquées de manière régulière de nos jours. La très grande diversité des matériaux disponibles permet de ne pas nécessiter un second site opératoire, cependant les auteurs n'arrivent pas à se mettre d'accord sur le choix d'un matériaux de référence. Bien que la fréquence des complications soient faibles, celles-ci sont présentes et trop peu développées de manière générale. L'objectif de cette thèse était de faire un état des lieux des différentes complications et leur gestion, des mécanismes y aboutissant et leur compréhension afin de mieux maitriser les traitements et d'éviter leur répétition. Pour ceci, une connaissance accrue de l'anatomie, des fonctions du sinus, des indications et contre-indications au sinus lift, ainsi que la notion bénéfice risque sont extrêmement recommandés. La correcte gestion des complications assure la survie de la greffe, des implants et la réussite du traitement prothétique.

C'est une chirurgie fiable et reproductible dont les bénéfices ne sont plus à démontrer. Bien que les complications soient rares, il est intéressant de savoir les gérer dans notre exercice hebdomadaire. Tout acte chirurgical va demander un suivi post-opératoire et une gestion des complications qui seront d'autant mieux acceptés par nos patients, lorsqu'une information préopératoire (aboutissant sur le consentement éclairé), un examen clinique et radiologique ont été mené. L'objectif de ce travail est de faire une synthèse des complications pouvant survenir en pré per et postopératoires lors de greffe sous sinusienne par abord latéral, et de mettre en avant la prévention, l'anticipation et la gestion des différentes complications. Grace aux progrès constant de la chirurgie assistée par ordinateur et l'évolution des concepts alternatifs aux greffes sous sinusiennes, cela nous permet aujourd'hui de répondre à la plupart des situations cliniques en évitant la greffe. Chaque patient est unique, nous devons prendre en compte la balance bénéfice/risque au titre individuel à chaque moment, le traiter donc dans sa singularité

Cette vacuité pourra être comblée par un matériau de greffe, d'origine naturelle ou non. Les techniques d'élévation et de comblement sous sinusien sont devenues indispensables dans la pratique quotidienne de l'implantologie orale. Elles ont augmenté les indications et les succès thérapeutiques, avec un recul de 30 ans. Cependant les indications doivent être clairement posées et la sélection des patients doit être stricte, afin de limiter les risques d'échecs.
