Book excerpt: Excerpt from The Thyroid Gland in Health and Disease A period of sick leave from active service has afforded me an Opportunity to incorporate into one volume the results of some fourteen years of study of the thyroid apparatus and its diseases. When, in 1902, I undertook the investigation of the aetiology of its maladies a maze of hypotheses and uncertainties surrounded the subject. Knowing nothing, premising nothing, I determined to examine it from the combined epidemiological, clinical, experi mental and pathological points of view. Some years of extensive epidemiological research cleared the ground of many false hypo theses, resolved some uncertainties, and brought to light certain facts which suggested an infectious or toxic origin for some of the thyroid's most common disorders. Clinical experience during the same period added further facts which strengthened this supposition; observations on man and experimental work on a large scale on animals converted the supposition into what I believe to be fact, and pathological studies have since confirmed my belief. Meanwhile it became necessary to revert to the study of the normal thyroid gland in order fully to understand the various processes of abnormality. As a result of this study a number of important points relating to the gland's normal range of physio logical action were elucidated, whereby it became possible to interpret the pathological changes to which various experimental procedures had given rise. The experimental physiology and pathology has been dealt with in exhaustive fashion in the excellent work of A. Biedl. The present volume is concerned with the clinical aspects of the diseases of the ductless glands. Of the results of experimental pathology I have set forth only as much as seemed to me necessary for the explanation of the clinical manifestations. For the rest of these results the reader is referred to the work of Biedl. In most of the chapters I have tried to describe the clinical symptomatology on the basis of my own observations, such as I had occasion to make, for the most part, at the first medical clinic in Vienna. Prof. Dr. L. von Frankl-Hochwarl and Privat-Docent Dr. R. Stern I thank cordially for the revision of the neurological part, Prof. Dr. Stoerck for the revision of the pathologico-anatomical part, and the latter also for numerous microscopical preparations. Macmillan 85 Co., a small portion of the articles on The Disorders of Function written for the New System of Gynaecology. In the Arris and Gale Lectures probably the first definite attempt was made to demonstrate that the reproductive functions are directed and controlled by all the organs of internal secretion acting in conjunction, rather than by the gonads alone, as formerly thought. Earlier investigators had adduced evidence, it is true, of a connexion between the genital organs and some of the distant hormonopoietic glands but the observations made do not appear to have led to the conclusion that the association is not incidental, but, rather, that it indicates the existence of a definite genital system, in which probably all the internal secretions play a part.
