Book excerpt: La pratique de l'odontologie pédiatrique dans les cabinets d'omnipratique est à ce jour limitée par plusieurs facteurs, dont l'anxiété dentaire que l'on retrouve souvent chez les jeunes patients. Cette dernière va être influencée par des facteurs extrinsèques, tels que les expériences vécues lors de soins dentaires, les informations reçues par l'enfant, sa culture ou encore son environnement. De même, des caractéristiques intrinsèques entrent en jeu (état de santé, perception de soi, sensibilité à la douleur). Cette anxiété se révèle être un obstacle lorsqu'elle limite la réalisation des soins, de par des comportements perturbateurs, allant jusqu'au refus de soins. Pour remédier à cela, de nombreuses techniques comportementales ont été développées. Le Tell-Show-Do est la principale technique utilisée au cabinet dentaire, en y ajoutant des jeux inspirés par les soins dentaires, elle se révèle très efficace. On retrouve ensuite la distraction, qui peut être auditive, audio-visuelle ou kinesthésique. La seule de ces méthodes ayant fait ses preuves est la distraction audio-visuelle, encore améliorée avec l'arrivée de la réalité virtuelle. De même, la modélisation, qu'elle soit directe, filmée ou interactive permet de gérer l'anxiété des enfants. On préférera les modélisations filmée ou interactive, dont les résultats sont plus fiables. Une nouvelle technique commence à trouver sa place en odontologie pédiatrique, il s'agit de l'hypnose dont les résultats sont pour le moment très encourageants. En dernier recours, il existe une thérapie cognitivo-comportementale, peu développée en France et difficile à mettre en place, mais très efficace dans la gestion de l'anxiété dentaire chez les jeunes patients.

Book excerpt: Pour de nombreux patients, les soins dentaires peuvent représenter une source d'anxiété, d'origine souvent multifactorielle et se manifestant de façons diverses, pouvant conduire ainsi à l'évitement des soins, donc à la dégradation de la santé bucco-dentaire. La sédation consciente par inhalation de MEOPA constitue une aide précieuse pour lutter contre l'odontophobie, notamment dans la prise en charge d'enfants opposants, et permet souvent de limiter le recours à l'anesthésie générale. Toutefois, même si cette méthode est en général bien acceptée et peu contraignante, son taux de succès est souvent évalué en référence aux soins effectivement réalisés par rapport aux actes prévus dans le plan de traitement, mais ils ne transcrivent pas vraiment les conditions dans lesquelles ils ont été conduits (par exemple : a-t-il été nécessaire de maintenir l'enfant au cours de la séance ?). Certaines techniques de relaxation non médicamenteuses, notamment la musicothérapie, ont fait l'objet d'études récentes, mais peu d'entre elles font part de leur association au MEOPA. Il serait donc intéressant d'évaluer l'intérêt d'adjoindre ces deux procédés au potentiel anxiolytique lors des soins dentaires. Ainsi, après avoir détaillé ces notions d'odontophobie et de musicothérapie, nous avons réalisé une revue de littérature scientifique, dont l'objectif était de déterminer l'influence potentielle de la musique sur la gestion de l'anxiété liée aux soins dentaires chez l'enfant. Ceci nous a conduit à proposer un protocole de recherche clinique visant à étudier l'impact d'une association entre musicothérapie et sédation consciente au sein du service d'odontologie pédiatrique du CHU de Nice.
