For most people, eyesight is part of everyday communication, social activities, educational and professional pursuits, the care of others, and the maintenance of personal health, independence, and mobility. Functioning eyes and vision system can reduce an adult's risk of chronic health conditions, death, falls and injuries, social isolation, depression, and other psychological problems. In children, properly maintained eye and vision health contributes to a child's social development, academic achievement, and better health across the lifespan. The public generally recognizes its reliance on sight and fears its loss, but emphasis on eye and vision health, in general, has not been integrated into daily life to the same extent as other health promotion activities, such as teeth brushing; hand washing; physical and mental exercise; and various injury prevention behaviors. A larger population health approach is needed to engage a wide range of stakeholders in coordinated efforts that can sustain the scope of behavior change. The shaping of socioeconomic environments can eventually lead to new social norms that promote eye and vision health. Making Eye Health a Population Health Imperative: Vision for Tomorrow proposes a new population-centered framework to guide action and coordination among various, and sometimes competing, stakeholders in pursuit of improved eye and vision health and health equity in the United States. Building on the momentum of previous public health efforts, this report also introduces a model for action that highlights different levels of prevention activities across a range of stakeholders and provides specific examples of how population health strategies can be translated into cohesive areas for action at federal, state, and local levels.

Book excerpt: Astigmatism is a common refractive error, which occurs alone or accompanies other refractive errors (e.g. myopia) and affects a considerable part of the world-wide population. The aetiology of astigmatism is complex, caused by the combined effects of multiple genes and environmental risk factors. The main objectives of this study are to understand the sources of phenotypic variation for refractive error, as well as identify and characterise genetic susceptibility loci for astigmatism. -- In this study, the broad-sense heritability for astigmatism was 67% which suggests a strong genetic contribution for astigmatism. A susceptibility locus, in a region of high linkage disequilibrium, upstream to the promoter region of COL13Al showed strong association with astigmatism in the TwinsUK cohort. Meta-analysis of seven different cohorts identified a different polymorphism for this region and highlighted two potential novel susceptibility loci in CHL1 and IGF1R genes associated with astigmatism. -- COL13A1 is found to be widely expressed in the human cornea and the organisation of which is largely responsible for transparency and refraction of the eye cornea. COL13A1 is regulated by the complex VitD3-VDR-RXR. Retinoic acid signalling pathways are complex and play an important role in eye development, including the cornea. -- Genome-wide expression studies have identified the CHL 1 gene as the most strongly expressed individual gene in the human cornea. During hyperopia defocus, cell adhesion molecules are down-regulated, suggesting the possibility of some involvement of this gene in eye development. -- The IGF-1 gene was found to be significantly associated with high myopia and implicated in eye growth control, and may also be associated with astigmatism development.
