The pharmaceutical industry s innovative power has significantly contributed to the improvement of the quality of health care. Medical innovations have completely transformed the treatment paradigm, have dramatically increased individuals chances of surviving certain diseases such as cancer and heart disease, and have reduced the likelihood and impact of diseases such as HIV/AIDS or arteriosclerosis. From a business perspective, the pharmaceutical industry has been the most profitable one during the last decade. With a median profit margin of 17 percent compared to 3.1 percent for all other industries on the Fortune 500 list, and representing 20 percent of all global research and development (R&D) investments as well as generating revenues of over USD 700 billion, the pharmaceutical industry has visibly shaped the global business world. However, the pharmaceutical industry is facing an increasingly volatile and uncertain environment. Evolving challenges such as an increase in regulatory state interference including the cost containment measures of health care reform, decreasing R&D productivity, and many blockbusters going off-patent are just some examples of the complexity and upheaval the industry is exposed to. Due to the increasing complexity and volatility, traditional planning tools are no longer suitable to adequately support conventional decision-making processes, since they insufficiently take uncertainty into account. This problem can be resolved by implementing scenario-based planning. This tool is applied to depict possible future scenarios, i.e., to identify a wide range of possible developments, which makes it a suitable tool in a volatile and complex environment. Hence, the objective of this thesis is to develop four plausible scenarios and secondly, to determine a core strategy, as well as strategic options for the pharmaceutical industry in Germany. First, an overview of the pharmaceutical industry in Germany is presented and major industry-related opportunities and challenges examined. Second, the theoretical foundation of scenario-based planning and its methodology is discussed. The HHL scenario-based approach to strategic planning is presented and briefly explained. Book excerpt: In the last three decades, several health care reform acts have targeted the pharmaceutical sector of the German Statutory Health Insurance System. But so far, the established regulatory framework has failed to stop the growth of drug expenditure. This book examines explanations for the cost development, and it provides suggestions for future reforms. By means of principal component and cluster analysis, groups of pharmaceuticals whose members are characterized by similar demand attributes have been built. Each group has been analyzed by considering the impact of age-dependency and technological progress on consumption. Based on these results, an outlook on future expenditure development, until the year 2050, is delivered. Although the main cost drivers are exogenous, politics are able to control the development of disbursements. Econometric tests show that health policy was characterized in the past by politicians' selfish interests, such as partisan and opportunistic behavior. The results of a vertical product differentiation model suggest the introduction of Medical Savings Accounts for achieving more efficiency in drug provision.

At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. The STG (Stuurgroep Toekomstscenario's Gezondheidzorg) was established in 1983 as an independent advisory group to the State Secretary for Welfare, Public Health, and Cultural Affairs (WVC) to assist in long-range health planning efforts. Thus far, STG commissions have examined cardiovascular disease, cancer, aging, and life styles as issues of importance to the health of the Dutch population in the future. Obviously, health care technology is of great concern to the government. On the one hand, technology is one of the major tools to promote a healthy population. On the other hand, the costs of health care have been rising at an alarming rate in recent years. It was these two facts, along with the social consequences of certain technologies such as genetic screening, that led the STG to establish the Commission on Future Health Care Technology in 1985. The European Office of the World Health Organization (EURO) cosponsored the project. The Health Council (Gezondheidsraad) agreed to cooperate with the project by furnishing space and intellectual and logistical support. The goal of the Commission's work is to develop sufficient information on future technological developments in health and health care to assist planning for their consequences. Subsequent reports will give more detail on future health care technologies and will provide in-depth assessments on a few specific technological areas.
