Available in PDF, EPUB and Kindle. Book excerpt: In 1998, the Government of Belize outlined its Health Sector Reform Program under which it signaled health financing as one of three priority areas. As part of the reform implementation, the National Health Insurance (NHI) was established in 2001 which had as its goal to improve access to quality of health services through both public and private sector providers as well as provide an appropriate pooling mechanism that would decrease the amount that individuals were paying out of pocket for health care services. Fifteen years later, the Government of Belize is interested in assessing to what extent NHI has had an effect on health outcomes, outputs, and access; quality in outputs and processes; and efficiency. This assessment lays out the results from the assessment. On health outcomes, outputs, and access, the study found no difference in health outcomes, mixed results on health services outputs in NHI-contracted compared to non NHI-contracted Primary Care Providers (PCPs), initial improvements in access to care, but little difference in addressing financial barriers to access. On the quality of outputs and processes, the quality of NHI-contracted PCP services has improved over the years of NHI contracting. On efficiency, a simple efficiency analysis based on quality scores indicates that there is little difference between Belize City South Side and Southern Region PCPs. However, while NHI chose to provide services in the lower income South Side of Belize City and the rural Southern Region as a way to target the poor, membership registration has been on a first-come first-served basis rather than favoring the poorer within these geographical locations. These assessments provide the Government with the evidence to guide their decision-making on whether NHI is a sustainable model and whether it should be scaled up on a national level. The report includes a set of policy considerations and recommendations for the Government to review in light of the national discussions on NHI sustainability and scalability.

For some time now, health care services have been supported by government funds. As demands for improving health care services continue to increase additional demands will be placed on governments to respond. This, however, will not be easy. Slow economic growth and record budget deficits in the 1980's have forced reductions in public spending. Alternative approaches to finance health care services are needed. Such possible changes could involve: decentralization of federal government involvement; the promotion of nongovernment involvement; the imposition of user fees; and, establishing health insurance. Finally, the role of the Bank in pursuing new financing strategies is discussed.

It originally appeared in May, 1978 under the title "An Approach to the Study of Health Sector Financing in Developing Countries: A Manual." The present revised and updated version of the manual was completed in April, 1979. This manual presents an action-tested procedure for appraisal of health sector financing which may be used, with some local adaptations, to examine health sector financial resources. The guidance presented in the text of the manual, combined with the prototype data collection and tabulation arrangement in Appendix A, are sufficiently detailed to lead a host country health planner or financial specialist through the assessment process. For successful completion of such an evaluation, it is anticipated that a senior-level economist or public finance specialist would be available to assist the analyst, both in initial design and final interpretation of the results.

Throughout, recommendations and advice are specific to the different purposes of cost analysis and the different types of decisions commonly facing managers. The manual, which is intended for use as a training tool, was finalized following extensive field testing in workshops in Bangladesh, Egypt, and Zimbabwe. Methods of cost-finding and cost analysis are thoroughly explained and illustrated with practical examples and model step-by-step procedures for performing calculations. Since hospital accounting systems in developing countries may have gaps or inaccuracies, the manual gives particular attention to reliable methods for estimating costs when existing data are problematic. The manual opens with an explanation of the many advantages of using cost-finding and cost analysis as managerial tools. These include the provision of data needed for informed decisions on operations and infrastructure investment, the planning of future budgets, the establishment of charges for patient services, and the development of mechanisms for ensuring that costs do not exceed available revenues and subsidies. Against this background, the core of the manual is presented in three chapters. The first and most extensive chapter explains how to allocate costs to cost centres and how to compute unit costs. Information and examples are presented according to seven steps. Each is discussed in terms of the types of data needed, how component cost items should be treated, and how costs can be computed in particular situations or cases. Practical examples are used to illustrate the types of questions addressed in cost analysis and the value of this information in guiding decisions. Chapter two explains how cost data can be used to improve the management of an individual hospital. Information is intended to guide decisions at both the cost centre, or department, level and the hospital level. Managerial tasks covered include budgeting, profitability, efficiency improvements, contracting outside services or producing in-house, and assessing fiscal solvency. Chapter three considers the use of cost data in managing national and regional hospital systems. Specific applications include improvements in the referral system, the appropriate use of different providers of services, and the comparison of similar hospitals to identify inefficiencies or sources of waste. The manual concludes with a series of practical exercises, followed by explanations of their answers.
