The diagnosis, antibiotherapy, and prevention of the streptococcal diseases have improved considerably. However, the reemergence of severe streptococcal and enterococcal diseases constitutes a growing public health con cern, which remains open to scientific and medical debate. The XIII'h Lancefield International Symposium on Streptococci and Streptococcal Diseases, held at Institut Pasteur, Paris, France, September 16---2el, 1996, attracted 505 par ticipants from 43 countries. Twenty-two percent of the participants were students, a clear sign of the intense interest in this field. Of the 390 presentations made at the symposium, 260 were submitted as manuscripts for the Proceedings; we have included 249 of these in this volume. This symposium provided a forum for the presentation of the most recent findings and approaches to understanding several important fields, such as new aspects of infec tion, bacteria~host interactions, epidemiology, and molecular genetics of streptococci and enterococci. Over the last three years, the study of these subjects has expanded as increas ingly sophisticated methods of molecular analysis have been applied to investigate the bi ology of pathogenic streptococci and enterococci. Virulence, vaccine strategies, genetics, antibiotic resistance, epidemiology, and immunology are now being examined through the lens of molecular biology. The application of recently developed techniques to this field will continue to yield insight into the mechanism by which these organisms cause disease.

Book excerpt: Invading microbes are detected by cellular sensors, the consequences of which result in the production of potent anti-pathogen proteins such as type I interferon (IFN) as well as other cytokines capable of stimulating the adaptive immune response. Examples comprise the RIG-I-like helicase (RLH) and the Toll-like receptor (TLR) families which recognize non-self-pathogen derived molecules (PAMPs) including bacterial lipopolysaccharides as well as nucleic acids. In addition, an endoplasmic reticulum (ER) associated transmembrane protein referred to as STING (for stimulator of interferon genes) was established as being essential for triggering the production of innate immune proteins in response to the sensing of cytosolic DNA. Such DNA can be ‘self’-DNA produced from necrotic or apoptotic cells, or the actual genomes of DNA pathogens that become exposed following infection. Moreover, while STING appears essential for controlling innate signaling events triggered by DNA microbes, chronic STING activation also appears to be responsible for certain inflammatory diseases manifested by ‘self’-DNA. Thus, understanding STING function may lead to the design of new compounds that may facilitate vaccine development or conversely that may provide new therapies for the treatment of inflammatory disease.

The process by which essential poly unsaturated fatty acids (PUF As) stimulate prostaglandin biosynthesis and cancer development, and its interruption by COX-2 inhibition, is elucidated. This is followed by a section on the epidemiology of NSAIDs and cancers of the colon and breast, and other anatomic sites. These chapters reflect significant cancer protection owing to the regular use of common NSAIDs such as aspirin and ibuprofen. A section on animal models of carcinogenesis presents comprehensive evidence that general NSAIDs inhibit a variety of malignant neoplasms in vivo, and highlights recent findings which show that COX-2 blocking agents produce striking chemopreventive effects against colon cancer and breast cancer as well as other malignancies. Genetic models are presented confirming the critical role of COX-2 in carcinogenesis. Section IV then discusses the molecular biology of COX -2 vis-a-vis the role of COX -2 and, to a lesser extent, COX -1, in modulating a number of important processes in molecular carcinogenesis such as mutagen esis, cell division, angiogenesis, cell differentiation, and apoptosis.

Book excerpt: One of the major biomedical triumphs of the post-World War II era was the defmitive demonstration that hypercholesterolemia is a key causative factor in atherosclerosis; that hypercholesterolemia can be effectively treated; and that treatment significantly reduces not only coronary disease mortality but also all cause mortality. Treatment to lower plasma levels of cholesterol - primarily low density lipoprotein (LDL) cholesterol - is now accepted as best medical practice and both physicians and patients are being educated to take aggressive measures to lower LDL. We can confidently look forward to important decreases in the toll of coronary artery disease over the coming decades. However, there is still uncertainty as to the exact mechanisms by which elevated plasma cholesterol and LDL levels initiate and favor the progression of lesions. There is general consensus that one of the earliest responses to hypercholesterolemia is the adhesion of monocytes to aortic endothelial cells followed by their penetration into the subendothelial space, where they differentiate into macrophages. These cells, and also medial smooth muscle cells that have migrated into the subendothelial space, then become loaded with mUltiple, large droplets of cholesterol esters . . . the hallmark of the earliest visible atherosclerotic lesion, the so-called fatty streak. This lesion is the precursor of the more advanced lesions, both in animal models and in humans. Thus the centrality of hypercholesterolemia cannot be overstated. Still, the atherogenic process is complex and evolves over a long period of time.
