Book excerpt: Background: Type 2 diabetes mellitus (T2DM) remains a significant public health concern. Few lifestyle intervention trials have evaluated the effectiveness of a self-regulatory approach to an educational lifestyle program for adults with T2DM. Self-regulation encompasses mechanisms, which describe deliberate and subconscious methods individuals use to pursue and attain goals. Goal Setting Theory (GST) and the Health Action Process Approach (HAPA) examine specific theoretical constructs: what motivates a person to initiate behavior change, how they take action, and how they maintain action. Psychological self-regulation models like Hope Theory and Construal Level Theory (CLT) aim to expose the fluctuations in the mental process of behavioral change which lead to failure or success in goal pursuit and attainment. Educational interventions incorporating behavior change theories and self-regulation principles could provide more effective T2DM management. Objective: To evaluate theoretical determinants, and behavioral and clinical outcomes before and after a group-based lifestyle intervention targeting self-regulation strategies in adults with T2DM; the Pathways Lifestyle Program (PLP). Methods: PLP is a novel, interdisciplinary lifestyle intervention built upon Goal Setting Theory, Health Action Process Approach, and self-regulatory techniques. The 10-week intervention emphasized nutrition and physical activity education combined with strategies and support for overcoming obstacles faced during behavior change. Goals for the intervention included: (1) consume = 3 low glycemic index (GI) servings/day; (2) achieve = 150 minutes/week of moderate-to-vigorous intensity physical activity (MVPA); and (3) achieve or maintain an A1c of = 7.0% via fingerstick sample. Overweight or obese participants (n=19) aged >21 years old, diagnosed with T2DM >1 year, and not requiring insulin therapy were recruited. Three, 24-hour dietary recalls were used to assess dietary intake, and wrist-worn accelerometry assessed change in MVPA. Results: Significant changes for action and coping planning, action self-efficacy, hope, percentage of time spent sedentary, and diastolic blood pressure were observed from pre- to post-intervention (all p =0.05). Increases in MVPA trended towards significance (p=0.07). Mean (±SE) change in total GI (-2.67±1.92; p=0.19) and glycemic load (-23.5±14.93;p=0.14) indicated improvement although were not statistically significant. There were no significant changes in consumption of low GI servings, body mass index, or A1c. Conclusions: Feasibility and proof of concept regarding an integrated theoretical approach to lifestyle change was successfully demonstrated in this preliminary study. Additional research is needed to fully implement the intervention with a sample size adequately powered to evaluate intervention efficacy. Results from this study suggest that with modification, a larger, randomized control trial has high likelihood of success.

Book excerpt: Type 2 diabetes (T2DM) currently affects more than three million African American adults with double the number expected by 2025. The most effective and safest treatment for T2DM is lifestyle change therapy, including healthful eating, monitoring of blood glucose, and physical activity. However, current lifestyle change interventions are limited in their scope to alter the behaviors of individuals to more healthful ones. These limitations may be attributed, in part, to a lack of attention given to enhancing an individual's psychosocial process variables, such as resilience, coping skills, selfleadership, and empowerment. Incorporating resilience education into lifestyle change therapies is a novel approach that addresses the behavior modification limitations of current interventions by aiming to enhance psychosocial process variables. Therefore, the purpose of this project was to conduct a six-month pilot study to determine the feasibility of our resilience and diabetes self-management intervention, The Diabetes Coaching Program: Transforming Lives Through Resilience Education, in a convenience sample of African American adults (n=16) with T2DM. The intervention included four weekly resilience and diabetes education classes and eight bi-weekly support group sessions. Survey data and blood samples were collected at baseline and at six months. Twelve participants completed the study (75% retention). Results indicated that higher perceived stress scores were associated with less resilience, fewer adaptive coping skills, lower selfleadership, lower diabetes empowerment and greater depressive symptoms. However, diabetes empowerment was the only psychosocial process variable to be significantly enhanced by the intervention at six months. Weight, BMI, HbA1c, total cholesterol, LDL cholesterol, blood pressure, and IGF-1 levels were significantly decreased at six months, whereas, lymphocyte proliferation and physical activity were significantly increased. These data indicate that our intervention has the potential to improve diabetes selfmanagement among African Americans with T2DM and increase positive health outcomes, though further studies are needed to confirm these findings. Additionally, several lessons were learned from conducting the pilot study that may be useful for improving the intervention for future studies, including: recruitment and retention strategies; cultural competency issues; the use of complementary and alternative medicine practices by African Americans with T2DM; and approaches for increasing participant self-assessment and goal-setting.

Book excerpt: Type 2 diabetes mellitus (T2DM) is a chronic and progressive medical condition that has reached pandemic levels, currently affecting 368 million people worldwide. With previous research demonstrating that T2DM is largely irreversible, the focus is now on developing and disseminating structured diabetes self-management education and support (DSME) programs to help patients live as healthfully as possible without exacerbating their illness. In recent years, several DSME interventions have demonstrated clinically meaningful changes in T2DM patients; however, there appears to be little translation of these programs from research into real world settings because these settings often lack clear blueprints for how to create or implement DSME programs that best fit their organizational needs and structure. Thus, the current study aimed to address this research-practice gap through the development of a brief, theoretically-driven, and behaviorally-focused group-based DSME curriculum for use in real world settings. Phase 1 of the study entailed concentrated efforts to join behavioral health researchers, community health leaders, and T2DM experts in combining the Common Sense Model of Self-Regulation (a prominent health behavior theory), cognitive behavioral principles, and current guidelines from the American Diabetes Association into a novel DSME curriculum. Phase 2 of the study then focused on implementing the resulting four-session curriculum, An Active Approach to Diabetes Self-Management, in local community centers and examining its early acceptability, feasibility, and effectiveness in producing clinical and psychosocial outcomes of interest. A total of 22 adults with T2DM, ranging in age from 35 to 87 years, were recruited from four community organizations and entered into four separate groups. Results indicate statistically significant improvements in participants' glycemic control, diabetes knowledge, and diabetes self-efficacy over the course of the study. In addition, results indicate that both participants and community liaisons were highly satisfied with the intervention. These findings contribute to the field's growing knowledge of how best to capitalize on clinical, research, and real world expertise to design thoughtful and practical DSME programs.

Unfortunately, participants in these programs seldom receive adequate direction or strategies necessary to develop a commitment to regular activity and thus, are subject to high relapse and attrition rates. As a result, the quality of life they might enjoy is sacrificed. Therefore, the purpose for conducting this qualitative study was to design, implement and examine the impact of a strategy oriented education intervention on adults with Type 2 diabetes using the transtheoretical model of behavior change in conjunction with the empowerment ideology and Pratt's relational construct of andragogy. Based on this premise, the specific intent of the intervention was to guide adults with Type 2 diabetes from preparation into the action stage of behavior change to incorporate physical activity into their lifestyle using a pedometer for feedback as part of their diabetes self-management routine. The contemporary research paradigm using a case study approach was used to frame the examination of the strategy oriented education intervention and its effects on the behavior change process of adults with Type 2 diabetes. From a methodological viewpoint, this approach proved to be instrumental in allowing me as the practitioner researcher to: (1) explore in great detail each experience, including my own, individually and collectively; (2) develop a comprehensive understanding of the behavior change process and the impact of teaching and empowering adults as adult learners; and (3) the effect a change in physical activity might have on the perception of quality of life, well-being, and/or health status of those with Type 2 diabetes. The evolution of a behavior change/health promotion using andragogy in relation model guided me in this study to help four individuals with Type 2 diabetes improve their activity level an average of 2127 steps per day as well as improve a number of their health related measures. Concurrently, the same individuals reported a positive change in their quality of life; thus, suggesting the inclusion of physical activity in their daily routine had a positive impact and was a welcome addition to their diabetes self-management strategies.

The purpose of this evidence-based practice (EBP) change project was to implement Purpose: The purpose of this evidence-based change project was to provide diabetes self-management education (DSME) to participants in a federally qualified health clinic (FQHC) to increase patient knowledge of diabetes self-management behaviors and decrease fasting blood glucose levels. Methods: Thirteen adults with uncontrolled Type 2 Diabetes; able to read, write, and speak English; and who had access to a telephone and a computer or tablet with a stable internet connection were voluntarily recruited at a FQHC. Four bi-weekly DSME one-on-one virtual sessions lasting one hour occurred over a nine-week period, with a 30-minute follow-up phone session during weeks when DSME was not scheduled. Using a pretest/posttest approach, participants' diabetes self-management knowledge, using the Revised Michigan Diabetes Knowledge Scale (RMDKS), and fasting blood glucose were measured before and after the educational intervention session. Statistical analysis utilized a paired t-test for comparison. Results: The data showed a statistically significant post-intervention improvement in both patients' diabetes knowledge with a mean pretest knowledge score of 60% and mean posttest score of 77% (p
