Le taux d'efficacité de cette méthode est globalement satisfaisant (environ 92%), néanmoins il persiste un certain nombre d'échec. En étudiant les caractéristiques épidémiologiques des femmes réunionnaises ayant eu recours à l'IVG médicamenteuse, nous avons cherché à identifier des facteurs de risque d'échec de cette méthode. Méthodes : Étude rétrospective dans un centre d'orthogénie de l'ouest de La Réunion. Entre le 1er mai et le 31 décembre 2012, toutes les femmes ayant eu recours à l'IVG médicamenteuse jusqu'au 63e jour d'aménorrhée ont été inclues (226 patientes). Le protocole médicamenteux associait 600mg de Mifepristone à 400ug de Misoprostol. Nous avons comparé les caractéristiques épidémiologiques de ces femmes en fonction de l'"échec" ou du "succès". Résultats : L'efficacité globale de la méthode était de 63%. Les échecs étaient représentés par des rétentions intra utérines (31%), des grossesses évolutives (4,5%) et des curetages hémostatiques (1,5%). L'antécédent d'IVG chirurgicales était un facteur de risque d'échec, avec un risque multiplié par plus de 10 (OR : 11,5 ; IC95% 2,4-54,8) pour les femmes y ayant eu recours au moins 2 fois. Nous avons retrouvé une diminution de l'efficacité de la méthode pour les femmes d'âge avancé (27,5 +/- 7,0 vs 26,3 +/- 7,6) et les multipares (67% vs 59,5%). Les caractéristiques épidémiologiques étaient marquées par un nombre élevé de grossesses (3,0) et d'enfants (1,4) avant l'IVG, un taux de recours important aux IVG itératives (40%), et une grande part de mineures (9,5%). Conclusion : Il convient d'informer les patientes aux antécédents d'IVG chirurgicale qu'une aspiration assurerait un succès supérieur. De nouveaux travaux doivent être mis en œuvre pour évaluer et améliorer le succès de la méthode.

Book excerpt: Etude observationnelle rétrospective portant sur l'efficacité des interruptions volontaires de grossesse médicamenteuses (IVG) sur des grossesses précoces, réalisées sur 3 centres d'orthogénie entre janvier 2017 et juin 2022. Objectifs : L'objectif principal de ce travail est d'évaluer l'efficacité et la sûreté des IVG médicamenteuses sur des grossesses précoces, réalisées durant la période de l'étude. En 2020, 220 000 IVG ont été enregistrées en France, chiffre stable depuis plusieurs années. Autorisées jusqu'à 16 semaines d'aménorrhée, par voie médicamenteuse ou instrumentale, les IVG peuvent être pratiquées dans les établissements de santé, dans les centres de planification ou certains cabinets médicaux de ville pour la méthode médicamenteuse. Depuis avril 2021, en raison de la crise sanitaire, l'IVG par méthode médicamenteuse est réalisable jusqu'à 9 SA en établissement ou à domicile. La méthode consiste en la prise d'un antiprogestérone (mifépristone), puis 24 à 48 h après une prostaglandine (misoprostol). Par ailleurs, la suppression du délai de réflexion d'une semaine avant la réalisation d'une IVG a entraîné des demandes d'IVG pour des grossesses plus précoces et a probablement augmenté les situations d'IVG sur grossesse de localisation indéterminée. Ces GLI correspondent dans la plupart des cas à une grossesse intra-utérine (GIU) à un stade précoce mais peuvent aussi correspondre à une grossesse non évolutive ou encore à une grossesse extra-utérine (GEU). Méthodes : Pour répondre aux objectifs de l'étude, nous avons conçu une étude descriptive observationnelle rétrospective multicentrique. L'étude a été menée dans les centres d'orthogénie du CHU de Grenoble, du GHM et au Planning Familial de Grenoble, pour des IVG médicamenteuses réalisées entre janvier 2017 et juin 2022. Le critère de jugement principal est défini comme étant le taux de succès de l'IVG médicamenteuse, le succès étant défini comme l'obtention d'un avortement complet n'ayant pas nécessité d'intervention chirurgicale. Les critères de jugement secondaire sont les facteurs de risque d'échecs d'IVG et le taux de grossesse extra-utérine. Résultats : Cette étude menée sur 165 femmes et incluant 143 patientes a permis de mettre en évidence un taux de réussite de l'IVG de 90,21%, un taux de grossesses intra-utérines évolutives de 2,8%, un taux de grossesses extra-utérines de 3,5%, un taux de rétention trophoblastiques de 3,5%. Le taux de femmes perdues de vue est de 7,6%. Les facteurs prédictifs d'échec mis en évidence de manière statistiquement significatif sont le tabac chez la patiente, la gestité. L'étude montre qu'une IVG réalisée avec un taux de hCG inférieur à 1000 UI/L a plus de risque de donner une grossesse évolutive. Conclusion : Bien que la réalisation d'IVG sur grossesse précoce reste une pratique marginale, il semble intéressant que des recommandations nationales soient définies pour rassurer, d'une part les professionnels de santé et ainsi favoriser l'utilisation de cette pratique, et d'autre part, les patientes sur la sécurité de ces IVG. Cette étude sur les facteurs de risques des IVG sur grossesse précoce peut permettre d'aider les professionnels à adapter au mieux les informations à donner aux patientes ainsi que la surveillance de l'efficacité en post-abortum. La réalisation d'une analyse avec un échantillon plus large pourrait permettre d'affiner les résultats obtenus en y intégrant éventuellement une évaluation du vécu de l'IVG sur grossesse précoce.

Book excerpt: Introduction : Un trouble de stress post-traumatique (TSPT) est déclaré chez 3 à 6% des femmes en post-partum (PP) et jusqu'à 18,5% en cas de complications de la grossesse ou de l'accouchement. L'objectif de cette étude est d'évaluer la prévalence du TSPT après une césarienne code rouge et d'identifier les facteurs de risque parmi les facteurs de vulnérabilité prénataux, les facteurs de risque de la naissance et les facteurs d'entretien du PP. Méthode : Un questionnaire téléphonique ou informatisé incluant un Questionnaire de stress immédiat et le Posttraumatic Stress Disorder Checklist for DSM-5 a été proposé aux patientes ayant eu une césarienne code rouge entre le 12/05/2015 et le 28/02/2021 au CHU Sud de La Réunion. Résultats : Parmi les 555 césariennes retenues, 329 parturientes ont répondu. La prévalence du TSPT était de 20,1% et était stable dans le temps. Les 2 facteurs de risque retrouvés étaient l'expérience négative de l'accouchement et le vécu traumatique avéré. Les facteurs de vulnérabilité prénataux n'ont pas été retrouvés statistiquement significatifs. Près de 3 femmes sur 4 n'avaient pas été informées du risque de césarienne et plus d'1 femme sur 2 n'ont pas eu d'explications en PP. Conclusion : Les césariennes code rouge entraînent un TSPT chez 1 femme sur 5. Ce trouble pérenne peut durer jusqu'à 6 ans après l'accouchement. Cela signe la gravité de ce trouble et la nécessité de le prévenir. Le risque de le développer est 4 fois plus important en cas de vécu traumatique avéré au Questionnaire de stress immédiat. Proposer ce questionnaire en maternité pourrait être un élément important de prévention secondaire. Le rôle du personnel de santé est primordial.

Néanmoins, une grossesse de localisation indéterminée (GLI) n'est pas une contreindication à la réalisation d'une IVG. Et reporter une IVG pour s'assurer d'avoir une grossesse intra-utérine (GIU) peut créer un obstacle à l'accès à celle-ci. Méthode : Étude comparative, rétrospective et quantitative au CH d'Armentières entre janvier 2019 et mars 2021. Critère de jugement principal : comparaison du taux d'échec des IVG médicamenteuses entre les IVG sur GLI et les IVG sur GIU. Résultats : 50 IVG sur GLI ont été comparées à 100 IVG sur GIU, les caractéristiques de ces 2 groupes étaient comparables. Il n'y avait pas de différence significative du taux d'échec entre ces 2 groupes (p=1,00) 11,4% pour les GLI contre 10,6% pour les GIU. Nous avons aussi recherché dans notre étude les facteurs prédictifs d'échecs de l'IVG : seule la parité était associée à un risque plus important d'échec. Conclusion : la pratique de l'IVG médicamenteuse sur des GLI est aussi sûre et efficace que sur des GIU. Ces résultats vont dans le sens des recommandations de plusieurs pays où une GLI ne doit pas retarder l'accès à l'IVG. On retrouve néanmoins une disparité des pratiques, notamment dans la surveillance post-IVG sur GLI. L'élaboration de recommandations nationales claires pourrait harmoniser les pratiques et améliorer la sécurité des IVG sur GLI.
