Book excerpt: People with limited English proficiency (LEP) tend to be underserved and vulnerable in the United States health care system. Research has shown that LEP patients are more likely to have lower levels of education, be uninsured, have lower levels of income, and are more likely to experience discrimination during their health care encounters. This study seeks to find whether LEP patients face less favorable health care experiences than their non-LEP counterparts. Specifically, the study aims to examine the differences in health care utilization, access to health information, and satisfaction of health care services between LEP and non-LEP individuals. Based on previous use of LEP in health care literature, the limited English proficiency group in this study represents those whose primary household language is not English. The dataset used for this study was the 2007 National Survey of Children's Health. Linear and logistic regression results found that LEP patients are less likely to feel that their children's doctors and providers listen to them carefully and they are also less likely to report that they feel their providers respect their customs and values compared to non-LEP patients. Additionally, LEP patients are less likely to receive the specific health information they need and they utilize health care services less often than their non-LEP counterparts. These findings lead to the conclusion that the need for culturally diverse and culturally competent care remains in order to improve the health care utilization and encounters for LEP populations.

With healthcare being a vital aspect of most individuals’ lives, it is drastically affected by any gap in communication, especially when a language barrier is present. For this investigation, I conducted a research study to examine the experiences of limited English proficiency (LEP) patients in healthcare using anthropological methods. The aim was to understand how having LEP affects patients. The primary form of data collection for this project consisted of Semi-structured interviews with a sample of individuals with LEP. In addition to interviews, I analyzed documents that shed light on the current and future policies as well as the public's perception on this topic. The results indicate that those individuals that a patient speaks with prior to the physician, including nurses and staff, pose an additional barrier to their healthcare. This research contributes to the current body of scholarship on language barriers in healthcare, which have been significantly lacking in patient perspectives. Through interviews, participants had the opportunity to voice their experiences and opinions, which they may have otherwise not been able to do, that could contribute to the development of better policies related to overcoming language barriers in health care. Further, this research could also contribute to better education practices for health practitioners with regards to language and health.

The objective of this study was to demonstrate that patients with limited-English proficiency face more barriers which create equity and disparity in understanding their physician as compared to their English-speaking counterparts. Secondary data analysis was performed using the California Health Interview Survey (CHIS) 2011-2012. The CHIS is a population-based random-digit dial telephone survey conducted every two years of California's population. The survey consists of extensive questions in a variety of health related topics. For purposes of this study, specific questions were selected for statistical analysis. All the literature and data analyses strongly supported that patients with limited-English proficiency are more likely to face barriers in obtaining quality care throughout the health continuum.

Best Care at Lower Cost explains that inefficiencies, an overwhelming amount of data, and other economic and quality barriers hinder progress in improving health and threaten the nation's economic stability and global competitiveness. According to this report, the knowledge and tools exist to put the health system on the right course to achieve continuous improvement and better quality care at a lower cost. The costs of the system's current inefficiency underscore the urgent need for a systemwide transformation. About 30 percent of health spending in 2009-roughly $750 billion-was wasted on unnecessary services, excessive administrative costs, fraud, and other problems. Moreover, inefficiencies cause needless suffering. By one estimate, roughly 75,000 deaths might have been averted in 2005 if every state had delivered care at the quality level of the best performing state. This report states that the way health care providers currently train, practice, and learn new information cannot keep pace with the flood of research discoveries and technological advances. About 75 million Americans have more than one chronic condition, requiring coordination among multiple specialists and therapies, which can increase the potential for miscommunication, misdiagnosis, potentially conflicting interventions, and dangerous drug interactions. Best Care at Lower Cost emphasizes that a better use of data is a critical element of a continuously improving health system, such as mobile technologies and electronic health records that offer significant potential to capture and share health data better. In order for this to occur, the National Coordinator for Health Information Technology, IT developers, and standard-setting organizations should ensure that these systems are robust and interoperable. Clinicians and care organizations should fully adopt these technologies, and patients should be encouraged to use tools, such as personal health information portals, to actively engage in their care. This book is a call to action that will guide health care providers; administrators; caregivers; policy makers; health professionals; federal, state, and local government agencies; private and public health organizations; and educational institutions.
