Presque 15% de la population souffre d'une douleur de hanche, et selon le Collège des Enseignants en Rhumatologie, la prévalence de la coxarthrose après 55 ans, serait de 5%. Devant la fréquence de ces pathologies, le médecin de premier recours se doit de maitriser son examen clinique afin de proposer une prise en charge optimale. Une revue systématique de la littérature a été réalisée afin d'évaluer les performances diagnostiques de l'examen clinique d'une hanche douloureuse et proposer une série de 10 tests cliniques réalisables en médecine générale. Nous avons choisi de ne pas aborder les pathologies traumatiques aiguës relevant de la filière des urgences. L'anatomie descriptive et fonctionnelle a été abordée dans le but de permettre une bonne connaissance des différentes structures composant cette articulation ce qui permettra de mener à bien un examen clinique systématisé et reproductible. Les différents temps essentiels de l'examen clinique seront présentés: l'interrogatoire, l'inspection, la palpation et enfin les manœuvres cliniques orientées afin d'évoquer un diagnostic précis. Nous pouvons à présent diagnostiquer sur des critères de sensibilité et de spécificité : les tendinopathies glutéales avec les tests de Trendelenburg, Hip lag sign et d'abduction contre résistance ; la coxarthrose avec l'association du Scour test et du test du squat maximal ; les lésions du ligament rond par le Ligamentum teres tear test ; et enfin les pathologies labrales par le test du FADIR. En cas de doute diagnostique ou de persistance des symptômes, le recours à un autre spécialiste et/ou à des examens complémentaires sera de rigueur.
