Book excerpt: Introduction : La récidive locorégionale est associée à un risque de survenue de métastases à distance et de décès dans le cancer du sein. L'objectif de cette étude était d'évaluer l'impact du délai de récidive locorégionale sur la survie sans métastase dans une série de cancers du sein traités par chimiothérapie néoadjuvante. Méthodes : Entre 2002 et 2012, 1191 patientes de la cohorte NEOREP à l'institut Curie ont été incluses. Une analyse multivariée a été réalisée à l'aide d'un modèle statistique de type « Random Survival Forest ». Résultats : Le délai médian de suivi était de 59,8 mois. 7,1% des patientes ont présenté une récidive locorégionale. Le délai de récidive locorégionale et la récidive locorégionale étaient les facteurs les plus discriminants dans la survenue de métastase. Un délai de récidive locorégionale inférieur à 50 mois était associé à un mauvais pronostic en termes de survie sans métastase à 3 et 5 ans. L'analyse par sous-types moléculaires (Luminal, triple négatif et Her2 positif) retrouvait également un seuil pronostique à 3 ans de 50 mois pour les 3 sous-types. A 5 ans, le seuil restait de 50 mois pour les tumeurs triple négatives mais était de 75 mois pour les tumeurs luminales et HER2 positives. Conclusion : Une récidive locorégionale avant 50 mois est un facteur pronostique important dans la survenue de métastases dans le cancer du sein traité par chimiothérapie néoadjuvante. Passé ce seuil, la survie sans métastase est peu influencée par le délai de récidive.
