Book excerpt: One of the global emerging epidemics of non-communicable diseases is Type 2 Diabetes Mellitus (T2DM). The adverse consequences of T2DM are a lowered life expectancy of up to fifteen years and an increased risk of heart disease. T2DM is the leading cause of kidney failure lower limb amputations, and adult-onset blindness. As of 2010, eight percent of the United States population, or 24 million Americans, have T2DM and an estimated 57 million people have 'pre-diabetes'. In California, a University of California Los Angeles (UCLA) study found that T2DM has increased nearly 26 percent between 2001 and 2007. Orange county, California has an increase in the number of persons with T2DM from five percent in 2001 to seven percent in 2003. A solution in addressing the increase in T2DM cases is through and evidence-based system change implementing group visits led by a Certified Diabetic Educator (CDE) with a self-management tool for patients with pre-diabetes. The intended goal of such a program in a community clinic setting was a reduction or delay in conversion rates of pre-diabetics becoming T2DM. The Capstone system change took place in a North Orange County, California community health clinic. The Capstone system change involved the evaluation of implemented group visits with a self-management tool for patients with pre-diabetes. The classes were once a week, one hour in length, and taught over a four week period. The goal is to refer and provide timely, evidence-based, culturally appropriate, preventative health care for the pre-diabetic patients to delay the progression to T2DM. A six step framework developed by Larrabee was used to guide the development of the Capstone system change. The theoretical perspective of the Health Promotion Model designed by Pender was also used to emphasize the preventive health aspect throughout the Capstone system change. Researchers have shown educational group visits for T2DM and pre-diabetes can lead to improved clinical outcomes, is cost effective, improves patient satisfaction, and leads to greater self-efficacy for the patient. Incorporating a system change to include evidence-based group visits for T2DM and pre-diabetes is a recommended, feasible and affordable method in delaying the onset of T2DM for community clinics. For this Capstone system change, the group visits were an effective, culturally appropriate, efficient, and cost effective method of delivering preventive health care.

Book excerpt: Diabetes is linked to behavior and requires patients to engage in complex self management practices to attain glycemic control and prevent complications. Group medical visits (GMVs)--which are shared medical appointments used in primary care provide a model of care to help patients adhere to self-management behaviors. Developing, implementing, and translating this model of care in a clinical setting has proven challenging, and the underlying mechanisms related to improved outcomes found in participants of GMVs are undetermined. The purpose of this study was to implement and evaluate GMVs for diabetes care in a primary-care and residency-training facility and explore changes in behavioral intentions, self-management behaviors, and blood HbA1c levels. The study also explored whether behavioral intentions and self-management behaviors functioned as mediators of changes in HbA1c levels. A repeated measures design with 37 participants examined the behavioral intentions to diet, to exercise, and to adhere to medication; self-management behaviors (i.e., diet, exercise, and adherence to medication); and HbA1c levels of participants with Type 2 diabetes during and after the GMVs. Behavioral intentions and self-management behaviors were measured through self-report instruments at baseline, posttreatment, and 3-month follow-up; HbA1c values were measured at baseline and 3-month follow-up. Self-reported adherence to diet changed significantly from baseline to posttreatment and remained at 3-month follow-up. Self-reported exercise changed significantly from baseline to posttreatment; changes were not sustained at 3-month follow-up. Clinical and statically significant changes in HbA1c levels were seen at 3- month follow-up. A reduction of 0.7% in blood glucose levels was observed; the majority of the participants (59.5%) attained diabetes control at 3-month follow-up. No mediation relationship was found between behavioral intentions, self-management behaviors, and HbA1c levels. This study is the first reported examination of GMVs that found significance in biophysical outcomes without research-based funding. GMVs focused on health-behavior change can be executed and sustained in primary-care and residency-training facilities. This program modality is a promising model of care for motivated patients and may help patients reach self-care goals and diabetes control. Future research with a larger sample size and a control group is needed to enhance the current findings.

Introduction: Although group Diabetes Self-Management Education and Support (DSMES) programs have been attempted throughout Community Health Network, no evaluation of the programs had been conducted. Objectives: The primary objectives of this study were to evaluate attendance, patient satisfaction and change in disease state knowledge following participation in six sessions of a group DSMES program led by a multidisciplinary team. The secondary objective was to evaluate clinical outcomes. Methods: This retrospective, observational study focused on process improvement evaluating a standardized approach to a group DSMES program beginning in August 2017 within Community Health Network. Eleven courses ran six sessions lasting approximately 1.5 hours that occurred weekly or every other week. Patients with a history of Type 2 diabetes who had a primary care provider within Community Health Network were invited to participate. Attendance and clinical outcomes were evaluated by a retrospective chart review of the electronic medical record. Evaluation of the program, both patient satisfaction and knowledge, occurred through the voluntary completion of various paper surveys throughout the program. Results: To date, eleven clinic sites have completed a group DSMES program totaling 89 patient participants. Program attendance was evaluated to determine what topics, days, times, and clinic sites would allow future programs to reach the most patients. 47 patients (52.8%) were able to attend four or more of the six sessions; 14 patients (15.7%) attended all six sessions. Responses from the patient satisfaction surveys indicated that patients approved of the speakers well over 90 percent of the time, patients "Strongly Agree" in favor of the sessions at a rate of 88.5% (3.54/4), and patients favored topics focusing on nutrition. Knowledge-based surveys showed an average score increase of 8.3%. Frequently missed topics included goal fasting and fed blood sugar levels. Patients showed mastery of concepts related to the definition of diabetes as well as frequency of immunizations, eye exams, and hemoglobin A1c monitoring. The average reduction of hemoglobin A1c was 0.58% overall. Patients that attended all six sessions saw an average decrease of 0.96%. Patients lost an average of 0.9 kilograms. Systolic blood pressure average dropped by 2mmHg. Conclusion: Following participation in the group DSMES program patients were satisfied with their care while increasing their disease state knowledge leading to an improvement in hemoglobin A1c. The results observed from the review of this DSMES program supports its continued use and expansion throughout Community Health Network" --Unnumbered leaf 1.

Data were collected to determine the effectiveness of this program using the following methods: interviews, document reviews, observations, and a survey. Descriptive statistics in the form of frequencies and percentages were used to describe the results. Analysis of the qualitative data was conducted and themes emerged. Recommendations were provided as to how the program can be improved upon to make it more effective.
