Book excerpt: De nombreuses études ont rapporté que les sujets atteints de schizophrénie (SZ) souffrent de troubles de la communication d'ordre langagiers et cognitifs. Jusqu'alors, il n'existe que deux outils d'évaluation de ces troubles: la TLC (scale for the assessments of Thought Langage and Communication) par Andreasen (1986) et la SCD (Scale for the evaluation of Communication Disorders) par Bazin et al. (2005). Cependant, ces deux outils ne testent pas exhaustivement les déficits de la communication. Afin de combler ce manque, nous nous sommes intéressées à la batterie MEC (Montréal Evaluation Communication, Joanette et al., 2004) utilisée en neurologie par des orthophonistes. Celle-ci propose une évaluation exhaustive des troubles de la communication testant les aspects pragmatiques, discursifs, lexico-sémantiques ainsi que prosodiques, retrouvés dans la sémiologie langagière de la schizophrénie. Notre étude exploratoire a pour but de déterminer si la MEC, complétée par une évaluation neuropsychologique et une tâche d'attribution d'états mentaux, pouvait être adaptée à l'évaluation des troubles de la communication présents chez les sujets atteints de schizophrénie. Pour cela, 13 patients (SZ) et 13 sujets sains (CT), appariés en âge et années de scolarité, et ayant le français comme langue maternelle, ont été testés. Les principaux résultats ont montré des déficits communicationnels chez les participants SZ par rapport aux participants CT. En effet, certaines sphères de la communication sont intégralement touchées telles que la sphère discursive et une large partie de la sphère pragmatique. Seuls certains aspects des sphères pro sodiques et lexico-sémantiques sont aussi perturbés. L'évaluation neuropsychologique nous a permis d'objectiver certains déficits. La validation de la batterie MEC comme outil d'évaluation adapté, permettrait d'objectiver l'impact des déficits communicationnels dans la vie quotidienne du patient, et par là, laisserait envisager que ces troubles entrent dans les champs de compétences de l'orthophonie, avec la mise en place d'un projet rééducatif individualisé.

Ils ont des répercussions évidentes sur la communication dans la vie quotidienne et sont pourtant peu pris en charge en orthophonie. Le but de cette étude est d'évaluer les compétences pragmatiques de sujets schizophrènes et de les comparer à celles de sujets souffrant d'épisode dépressif majeur. Notre évaluation s'appuie sur 3 épreuves tirées du protocole Montréal Évaluation de la Communication : discours conversationnel, interprétation de métaphores et interprétation d'actes de langage indirect. Les résultats des sujets schizophrènes montrent de nettes difficultés au niveau de l'organisation du discours, des règles conversationnelles et des processus inférentiels. Ces résultats sont à nuancer au regard des difficultés cognitives de mémoire, d'attention et des fonctions exécutives. Une approche orthophonique de la sémiologie langagière de la schizophrénie représente un enjeu important pour la réhabilitation psychosociale.

Nous avons donc élaboré un outil d'évaluation orthophonique, de la pragmatique et du discours, à destination de patients atteints de schizophrénie. Le but de notre étude était d'objectiver leurs troubles via un nouvel outil. Pour se faire nous avons comparé les résultats obtenus par les patients à ceux obtenus par les sujets sains appariés en sexe, en âge, et en niveau d'études. Nos résultats ont révélé que les troubles langagiers des patients ont bien été mis en évidence par notre outil. De plus, l'analyse fine de nos résultats a permis d'établir des profils langagiers spécifiques à chaque patient. Ainsi, nous pensons que l'orthophonie devrait faire partie intégrante de l'évaluation et de la prise en soin de ces patients. En recherche, il serait nécessaire de valider nos épreuves sur un échantillon de patients plus important afin de corroborer nos conclusions. En clinique, l'évaluation et la prise en charge orthophonique permettrait une amélioration de la communication, une meilleure insertion socioprofessionnelle des patients, et une augmentation de la qualité de vie.

Résultats/discussion : Les résultats indiquent la présence de véritables troubles de la communication, mais n'ont pas pu permettre l'établissement de profils statistiquement significatifs en raison de la taille de l'échantillon. Cependant, des groupes de patients ayant des atteintes similaires des composantes communicationnelles ont pu être établis et des axes de prise en charge adaptés ont pu être proposés. Conclusion : Les troubles de la communication peuvent et doivent être évalués et pris en charge systématiquement par les orthophonistes dans la schizophrénie afin de limiter les conséquences fonctionnelles au quotidien.

Les psychiatres ayant lu notre outil d'information ont ensuite répondu à un questionnaire dans lequel il leur a été demandé d'une part, quel était leur avis sur ces troubles, et d'autre part, que pensaient-ils d'une prise en soin en orthophonie pour ces patients. Résultats Ces professionnels affirment que les troubles du langage et de la communication peuvent être invalidants pour leurs patients atteints de schizophrénie et ils présentent un intérêt quant au fait d'intégrer davantage l'orthophonie dans leur parcours de soin. Discussion : Il semble donc nécessaire de continuer à communiquer autour de cette problématique et d'expliquer les limites auxquelles les orthophonistes sont actuellement confrontés. Des recherches sur la prise en soin en orthophonie des patients souffrant de schizophrénie doivent également être poursuivies. Conclusion : Il s'agit, à l'avenir, de pouvoir accompagner pleinement les patients qui souhaitent être aidés dans leurs difficultés de langage et de communication.

Cette étude a pour but l'élaboration d'un livret d'information à destination des équipes soignantes des structures de psychiatrie adulte, pour les informer à ce sujet. Un questionnaire est élaboré et envoyé à cette population de professionnels afin de mesurer l'efficacité du livret par l'évaluation de leurs connaissances avant et après lecture de l'outil. Les résultats montrent une différence significative des deux mesures, mettant en avant une évolution positive de l'apprentissage concernant la pertinence et les moyens de l'orthophonie dans ces prises en soin suite à la lecture du livret. Cet outil d'information semble efficace dans les objectifs qu'il s'est fixés.

The sexual anatomy of the body also feels alien and inappropriate. Transsexualism that causes distress is treated with sex reassignment therapy. The aim is to alter body characteristics and anatomy closer to those of the target gender as well as to facilitate the integration into the social role of the opposite sex. Investigations and surgery are often only available in specialist centres.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
