Book excerpt: La maîtrise des apprentissages fondamentaux est indispensable pour une évolution scolaire favorable de l'enfant, pour la qualité de ses relations aux autres, au niveau de la confiance qu'il peut avoir dans ses compétences ; elle est la base de tous les acquis ultérieurs, et influencera également l'insertion socioprofessionnelle future de cet enfant. De nombreux élèves se trouvent confrontés à l'échec au niveau de ces apprentissages fondamentaux, notamment dans le domaine de la lecture. Pour tenter de palier ces difficultés, un dépistage des troubles au niveau des compétences pré-requises à l'apprentissage lexique est instauré lors du bilan de santé obligatoire de la 6ème année. Des outils permettant ce dépistage ont été créés, à destination des médecins, notamment les médecins scolaires. Le BSEDS 5-6 (Bilan de Santé Evaluation du Développement pour la Scolarité 5 à 6 ans) est un bilan médical qui comporte de nombreuses épreuves cognitives permettant de pointer finement ces troubles. Six médecins scolaires du Nord sont allés à Grenoble, se former à la passation de ce protocole. Après deux années de pratique, ils constatent qu'il est très long dans sa passation, et souhaiteraient qu'il soit restreint aux trois épreuves les plus prédictives (une épreuve visuelle, une épreuve métaphonologique, et une épreuve de langage). L'objet de ce mémoire d'orthophonie est la sélection, dans ce protocole, de ces épreuves. Pour les repérer, nous avons procédé à diverses études de corrélation : entre les épreuves du bilan et un score global calculé pour ce bilan, entre les épreuves elles-mêmes, entre le bilan dans son ensemble et une batterie de lecture administrée aux mêmes enfants un an plus tard ; nous avons également recherché si les facteurs "niveau socioculturel", "type d'école", et "âge" avaient pu influencer les résultats obtenus par les enfants de maternelle. Les différentes études menées sont présentées dans ce mémoire d'orthophonie, qui s'inscrit dans une démarche de prévention secondaire des troubles d'apprentissage de la lecture.

De telles difficultés étant majorées chez les enfants scolarisés en REP (Réseau d'Education Prioritaire), c'est auprès d'eux que nous avons choisi de mener notre étude. Empruntant les chemins tracés par Jean Piaget, enrichies par les théories qui s'y réfèrent, nous avons émis l'hypothèse qu'un retard de langage, observé chez des enfants de grande section de maternelle, scolarisés en REP, peut être mis en corrélation avec un décalage dans la psychogenèse du lire-écrire et un défaut de généralisation des schèmes représentatifs. Notre recherche nous a bien évidemment permis de mesurer la validité de notre hypothèse, mais aussi d'apprécier toute la richesse de l'approche psychogénétique dans une pratique orthophonique. Elle permet une évaluation explicative et non normative des difficultés de l'enfant à travers l'appréhension de son fonctionnement général. En ce qui concerne plus particulièrement le bilan de langage oral chez un enfant d'âge préscolaire, nous sommes convaincues de l'intérêt de l'utilisation des protocoles d'évaluation de la généralisation des schèmes représentatifs et de la psychogenèse du lire-écrire dans un souci de compréhension des troubles du langage oral et de prévention des troubles d'accès au langage écrit.

Cependant certains enfants présentent d'importantes difficultés dans l'apprentissage de leur langue maternelle qui peuvent être liées à une pathologie sous-jacente mais aussi à un environnement peu favorable. Comment aider ces enfants de manière efficace et adaptée? Est-ce à l'Education Nationale ou aux professionnels de santé de les prendre en charge? A quel moment faut-il agir? L'action «dépistage et suivi des enfants à risques de difficultés scolaires en moyenne section de maternelle» semble répondre à ces questions, par la recherche-action en 20012002 puis par la formation «Com'Ens »depuis 2004-2005, en créant un réel partenariat entre toutes les personnes gravitant autour de l'enfant (les professionnels de santé, ceux de l'Education Nationale et les parents) afin de venir en aide ensemble aux enfants en difficultés ou à risques de difficultés scolaires ultérieures. Ainsi, après un dépistage en moyenne section de maternelle est organisé, dans l'école, un suivi sous forme de petits groupes de 5 à 6 enfants rassemblés autour d'un album présenté par les professionnels formés. Les enfants présentant des troubles importants peuvent aussi être orientés vers des bilans et rééducations spécifiques. Notre travail présente un bilan de cette action depuis 2001-2002 en insistant particulièrement sur la formation « Corn 'Ens » et les différents dispositifs mis en place dans ce cadre ainsi que sur l'évaluation de son impact sur les professionnels engagés et les enfants suivis.

Chapter one has been largely rewritten to give an outline of current approaches to a model of comprehension of spoken language. Chapter two has a new initial section but otherwise remains as it was. Chapter three incorporates a new section on "pause" and how this interacts with rhythm, and rather more on the function of stress. Chapter four has an extended initial section but otherwise remains largely as it was. Chapter five on intonation contains several sections which have been rewritten to varying extents. Chapter six of the first edition has disappeared: in 1977, very little work had been published on "fillers" and it seemed worthwhile incorporating a chapter that sat rather oddly with the phonetic/phonological interests of the rest of the book. Not that there is a great industry of descriptions of the forms and functions of these and similar phenomena there seems no reason to retain this early but admittedly primitive account. The chapter on "paralinguistic vocal features", now chapter six, has some rewriting in the early part but considerable rewriting in the last sections. The final chapter on "teaching listening comprehension" has grown greatly in length. It still incorporates some material from the original chapter but most of it is completely rewritten.
