Book excerpt: Introduction: Les consultations pour chute dans les services d’accueil des urgences (SAU) représentent de l’ordre de 5 à 10 % des motifs de consultation des personnes âgées. En raison du vieillissement de la population, elles représentent (et représenteront de plus en plus) un problème de santé publique. Le but de ce travail est d’évaluer les chutes à domicile des patients de plus de 80 ans, se présentant au SAU. Méthodes et patients: Evaluation prospective, observationnelle et mono-centrique des caractéristiques des patients de plus de 80 ans admis pour chute à domicile, dans le SAU de l’Hôtel Dieu de Paris durant l’année 2009. L’objectif principal était une analyse des chutes chez les sujets de plus de 80 ans et d’en dresser un état des lieux, travail préliminaire à une analyse des facteurs de risque. Tous les patients admis dans un contexte de chute, exclusivement à domicile (ou maison de retraite) ou dans les parties communes de ce domicile, ont été inclus. Résultats: Parmi les 45 000 patients venus aux urgences en 2009, 12% étaient âgés de plus de 80 ans Les consultations dans un contexte de chute (n=200) représentaient 3,7% de ce collectif [(sexe féminin : 62%), (80-90 ans : 132 ; 90-100 ans : 50 ; > 100 ans : 18)]. 125 ont chuté à domicile (32 vivaient seuls et 102 avaient une aide à domicile) et 75 en institutions. L’état antérieur cognitif était bon dans 145 cas (72%), altéré dans 35 (18%) et 20 (10%) patients étaient déments. L’autonomie était bonne chez 132 patients (66%). Au moins une comorbidité était présente dans 146 cas (dont 38 maladies de Parkinson). 115 patients (58%) avaient fait au moins deux chutes dans l’année. La chute était accidentelle dans 68% des cas et liée à un malaise dans 32 % des cas. Le délai entre la chute et l’arrivée au SAU (connu pour 181 patients) était supérieur à 4 heures dans 63 % des cas. Les conséquences de la chute (plusieurs conséquences possibles) étaient traumatologiques (n=45, 23%), métaboliques (n=112, 56%), cardiovasculaires (n=82, 41%), respiratoires (n=48, 24%) ou neuropsychiques (n=63 ; 32 %). 135 patients (68%) ont nécessité une hospitalisation en MCO, 32 (16%) en gériatrie aiguë, 13 en moyen séjour (7%) et seulement 20 (10%) ont pu retourner directement à domicile (ou en institution). La mortalité à 7 jours est de 3,5% (n=7, dont 2 dans les 48 heures). Plus de la moitié des patients (57%) ont été réadmis aux urgences dans l’année suivante, dont 18 % dans un contexte de chute.Conclusion: Dans ce travail, 3,7 % des patients de plus de 80 ans qui consultent dans un SAU le font à la suite d’une chute à domicile. Plus d’une fois sur deux, le délai entre la chute et sa découverte est supérieur à 4 h. Les conséquences de la chute ne sont traumatologiques que dans 23% des cas. Seulement 10% des patients ont pu retourner à domicile le jour même. La mortalité à 7 j est de 3,5%. Ces résultats montrent que la survenue d’une chute à domicile, chez une personne âgée de plus de 80 ans, qui a nécessité une consultation dans un SAU, remet souvent en question à court terme, un mode de vie autonome. Dans ce contexte, ce travail devrait être préliminaire à une analyse des facteurs de risque mais également à celle des conséquences à moyen/long termes.

Les chutes des personnes âgées sont un enjeu majeur de santé publique, par leur fréquence et leur impact socio-économique. Chaque année, une personne âgée de plus de 65 ans sur trois chute. Cet évènement est responsable de comorbidités majeures et constitue une réelle entrée dans la perte d'autonomie. La prise en charge effective des chutes des personnes âgées reste limitée et la consultation médicale post chute recommandée par la Haute Autorité de Santé est peu réalisée. L'objectif de cette étude était d'évaluer l'impact d'un protocole remis aux patients chuteurs lors de leur passage aux urgences sur la réalisation de la consultation post chute chez le médecin généraliste. Les objectifs secondaires étaient d'évaluer l'impact de ce protocole sur la réalisation des autres examens recommandés (bilan biologique, kinésithérapie, ophtalmologue, pédicure/podologue). Méthodes. étaient inclus les patients de 65 ans et plus vivant à domicile et pris en charge aux urgences de l'Hôpital Foch suite à une chute. Pour évaluer l'impact du protocole d'étude, les patients ont été répartis en deux groupes (avec ou sans protocole) et un questionnaire téléphonique a été réalisé dans 15 jours suivant la consultation pour chaque patient inclus. Résultats. Au total, 50 patients ont été inclus de mai à juillet. Ils ont été répartis en deux groupes de population homogène. Dix patients n'ont pas pu être recontactés à distance (1 patient du groupe avec protocole et 9 du groupe sans protocole). Pour ceux recontactés, la consultation post chute chez le médecin généraliste a été réalisée dans 75% des cas dans le groupe avec protocole (24 patients) contre 32% des cas sans protocole avec une différence significative (p= 0.04). Pour les autres examens recommandés, il n'existait pas de différence significative entre les deux groupes excepté pour la réalisation du bilan biologique (p=0.04). Cependant, leur réalisation était plus fréquente lorsque les patients avaient reçu le protocole de conduite à tenir post chute. Conclusion. La mise en place du protocole de conduite à tenir post chute chez les personnes âgées lors de leur passage aux urgences pour chute permet d'augmenter la fréquence de réalisation de cette consultation et ainsi améliore la prévention du risque de récidive de chute. Les mesures de prévention passent avant tout par l'information des patients et la remise d'un document écrit semble permettre une meilleure adhésion de ces patients au projet de soins.
