L'objectif est d'évaluer le taux de pertinence des admissions en urgence des personnes âgées, les facteurs de risques associés. La pertinence a été évaluée par l'Appropriateness Evaluation Protocol. Cette étude épidémiologique, prospective a été conduite dans huit CHU français sur deux périodes de quatre semaines en 2010. Tous les patients, âgés de 80 ans et plus, admis dans un service hospitalier après un passage au SAU, ont été inclus. Sur 1577 admissions incluses, le taux de non-pertinence des admissions s'élève à 7,7% (121 admissions). En analyse multivariée, l'existence de pathologies cardiaques, l'origine institutionnelle, la sévérité clinique diminuent significativement la non-pertinence des admissions. Un diagnostic principal syndromique apparaît comme un facteur de risque de non pertinence. Les admissions non pertinentes en urgence des personnes âgées sont peu fréquentes dans cette étude.

Méthode : Etude prospective, descriptive, uni centrique, d’une cohorte de 685 patients admis aux urgences du CHU de Nantes sur 2 périodes de 15 jours en 2010. Détermination du taux d’admission non pertinente à l’aide d’un questionnaire inspiré de la grille Appropriateness Evaluation Protocol (AEPf) puis analyse statistique comparative entre les admissions jugées pertinentes et non pertinentes. Résultats : 81% des admissions ont été jugées pertinentes selon la grille AEPf et ce taux est similaire aux données de la littérature. La population étudiée est à prédominance féminine (65%) et vit principalement à domicile (68%). 51% des patients sont adressés par un médecin, sans que cela n’influence la pertinence de l’admission. Par contre, la régulation médicale par le Centre 15 avant l’admission apparait être un facteur de pertinence. Conclusion : Les plus de 80 ans semblent donc avoir un usage plutôt adapté des Urgences du CHU de Nantes. Une meilleure connaissance de la symptomatologie complexe de la personne âgée et des filières de soins existantes permettrait d’optimiser leur prise en charge et de limiter leurs passages inappropriés aux urgences.

La gestion des flux de patients, devenue prioritaire devant le nombre de lits limité des Centres Hospitaliers, impose de limiter au maximum le nombre d'hospitalisations non pertinentes qui sont délétères chez ce type de patient. Les hospitalisations non pertinentes qui sont un phénomène connu mais peu étudié en France, influencé par de nombreuses variables dont l'organisation locorégionale des soins. L'objectif principal de cette étude était d'évaluer la pertinence à l'aide de la grille AEPf (version française de l'Appropriateness Evaluation Protocol) et d'étudier les caractéristiques des admissions aux urgences des personnes âgées institutionnalisées, en Allier. Les 219 patients inclus dans l'étude ont été adressés aux services d'accueil des urgences (SAU) des Centres Hospitaliers de Montluçon, Vichy et Moulins du 15 juin au 19 juillet 2015. La population de l'étude était composée de 68% de femmes et la moyenne d'âge était de 87 ans et demi. Les motifs médicaux représentaient 53% et les motifs traumatologiques 29% des motifs d'admission au SAU. Le taux d'admissions non pertinentes aux SAU a été évalué à 9%, ce qui correspond aux données de la littérature récente. La régulation médicale semblerait améliorer le taux de pertinence des admissions aux urgences (92%) et le taux de pertinence des admissions des patients provenant des institutions du territoire de santé de Montluçon semblerait moins élevé (83%), mais ces résultats ne sont pas significatifs. La grille AEPf ne semble pas être l'outil optimal pour l'évaluation de la pertinence des admissions aux SAU des patients institutionnalisés en Allier étant donné les dossiers litigeux (13%) et le motif traumatologique absent de la grille. Ces résultats invitent à réaliser une étude épidémiologique de plus grande ampleur à l'aide d'une grille élaborée avec les médecins gériatres à fin d'analyser précisément les différents facteurs bio-psycho-sociaux et organisationnels intervenant dans les problématiques du parcours de soins des patients institutionnalisés, qui reste perfectible.

La population française est vieillissante, et parallèlement, le nombre de médecins généralistes en regard des besoins de la population décroit depuis plusieurs années. Existe-il un lien entre la difficulté d'accès aux soins primaires en ambulatoire et l'affluence des personnes âgées aux urgences ? Il s'agit d'une étude rétrospective concernant les patients âgés de 75 ans et plus admis dans le service d'accueil des urgences du Centre Hospitalier Lyon Sud au cours de l'année 2019. Un recueil de données a été réalisé au cours de 6 périodes d'une semaine afin d'évaluer l'impact d'évènements épidémiques, climatiques et scolaires sur l'affluence des personnes âgées. Dans un second temps, la pertinence des admissions a été évaluée à l'aide de l'outil Appropriateness Evaluation Protocol (AEPf) Le nombre hebdomadaire moyen de passages de personnes âgées de 75 ans et plus est globalement stable au cours de l'année et représente 20% du nombre total d'admissions aux urgences. Les personnes âgées sont adressées après un avis médical dans 23% des cas, par un professionnel de santé dans 59% des cas. Seulement 18% se présentent spontanément. Aucune variation significative des modalités d'admissions en fonction des évènements épidémiques, climatiques et scolaires n'a été constatée. La pertinence de l'admission des personnes âgées est de 73.5% lorsque l'admission est précédée d'un avis médical ou d'un professionnel de santé contre 49.4% lors d'une admission spontanée. Cette pertinence de l'admission a un impact sur le devenir des patients à l'issue de leur passage aux urgences. En effet, lorsque le passage est pertinent, il est suivi d'une hospitalisation dans 70,7% des cas. Contrairement à l'hypothèse de départ, cette étude a montré que l'affluence des personnes âgées aux urgences n'était pas impactée par la difficulté d'accès aux soins primaires en fonction des évènements épidémiques, climatiques et scolaires. Il ressort de cette étude un intérêt réel de réguler les admissions aux urgences afin qu'elles soient limitées aux admissions pertinentes à la fois pour diminuer le nombre d'admissions dans les services d'urgences mais aussi pour limiter les effets iatrogènes d'un passage aux urgences sur les personnes âgées. Plusieurs améliorations ont été proposées et mises en place par les autorités de santé ces dernières années.

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
