Résultats : 94 nouveau—nés ont été inclus, 2 nouveau-nés étaient douloureux et 67 étaient inconfortables. 5 facteurs influencent l’inconfort : l'âge gestationnel, le poids de naissance, le délai entre l'administration de corticoides et la naissance, le mode de déclenchement et la réalisation de gestes invasifs. Discussion : L’évaluation de la douleur prolongée doit être systématique chez tous les nouveau-nés. Le caractère multifactoriel de la douleur laisse penser que c’est l’association de plusieurs facteurs qui entraine l’inconfort.

Book excerpt: Objectif : L'objectif de cette étude était d'établir un état des lieux des pratiques concernant la traçabilité, le dépistage, l'évaluation et la prise en charge de la douleur à la naissance et en maternité, au CHU de Poitiers. Méthode : Étude rétrospective sur 391 nouveau-nés à terme en maternité. Les critères de jugements principaux étaient : les moments de l'évaluation de la douleur du nouveau-né ; les méthodes d'évaluation et les outils de mesure utilisés ; la fréquence de réalisation ; les moyens mis en oeuvre pour y pallier ; les thérapeutiques utilisées ; leurs posologies et la durée d'installation ; et secondairement, les motifs pouvant expliquer la douleur chez les nouveau-nés. Résultats : Notre étude a montré qu'aucune échelle de cotation de la douleur n'a été utilisée lors des examens cliniques. Un score DAN a été côté durant le séjour avec un taux de renseignement jusqu'à 97,9 % les trois premiers jours. Une évaluation de la douleur a été tracée pour 33 nouveau-nés lors de leur examen clinique initial. L'utilisation du paracétamol a été relevée pour 38 nouveau-nés et six d'entre eux avaient un examen clinique initial à priori normal. Les motifs de douleurs retrouvés étaient la présence d'une bosse sérosanguine, des marques de forceps, ventouse et lésions du scalp. Conclusion : La mise en place d'un protocole et l'adaptation des supports de suivi du nouveau-né seraient à étudier, afin de sensibiliser d'avantage les professionnels de santé au traçage régulier de l'évaluation de douleur, et d'optimiser sa prise en charge.

The critique of ‘essentialism' (i.e. there is no specifically feminine essence) proposed by both gender theory and deconstruction is just one more twist in the ontological negation of the feminine.
