L'Internet est aujourd'hui un outil communément utilisé pour rechercher et échanger tout type d'information, y compris dans le champ de la santé. L'analyse de la littérature permet d'estimer qu'environ un patient consultant sur cinq a recherché de l'information en santé sur Internet. Dans ce contexte, une rencontre en consultation de médecine générale avec un patient internaute nous a incité à nous intéresser aux perceptions qu'ont les médecins généralistes sur la relation médecin-malade avec un patient internaute. Pour étudier ce phénomène social et humain émergent qu'est la relation médecin-malade avec un patient internaute, nous avons réalisé une enquête qualitative exploratoire. Trois "focus groupe" (ou groupes de discussion) de médecins généralistes, focalisés sur leurs expériences vécues en consultation avec un patient internaute, ont été enregistrés. Les discours retranscrits ont été analysés selon une méthode d'analyse qualitative de contenu d'approche phénoménologique, dans une démarche compréhensive. Les résultats obtenus caractérisent les perceptions de médecins généralistes sur la relation médecin-malade avec un patient internaute (objectif principal) mais aussi sur les patients internautes et sur l'Internet (objectifs secondaires). L'approche phénoménologique met en lumière l'existence d'une signification "ici et maintenant" des recherches sur l'Internet et des habitus des patients internautes, pour une prise en charge globale du patient. Cette approche phénoménologique, compréhensive, permet de restituer en synthèse que les modifications de la relation médecin-malade dans la médiation avec l'Internet dépendent des positions intersubjectives de chacun des acteurs de cette relation, au moment de la rencontre (en consultation), mais n'affectent pas les fondements de la relation médecin-malade installée

Il constitue une importante source d'informations médicales pour les patients devenant des interlocuteurs informés. Régulièrement confrontés à ce phénomène, quel est le ressenti des médecins sur leur relation avec des patients-internautes ? Une étude qualitative par entretiens semi-dirigés auprès de treize médecins généralistes du Cher a été réalisée entre Juillet et Octobre 2016. L'échantillon était raisonné en fonction du sexe, de l'âge, des lieux et du mode d'exercice. Une retranscription écrite intégrale et une analyse thématique de contenu ont été réalisées. Les médecins ont reconnu un usage régulier d'internet dans leur pratique professionnelle via des sites référencés. Ils ont considéré qu'internet était un outil d'informations rapide et facile d'accès au point de le banaliser. Le profil des patients à la recherche d'informations n'a pas été modifié avec l'arrivée d'internet. Selon les médecins, les motivations sont multiples : implication dans la démarche de soins, curiosité, effet de société ou par manque de médecins. La communication est devenue plus sincère augmentant la relation de confiance. Le patient est devenu plus autonome. Il est responsabilisé dans son projet de soins par partage du savoir. La diffusion de fausses informations est restée un risque majeur remettant en cause l'autorité médicale. Le rapport de force créé par une justification des prises en charge a entrainé une augmentation du temps de consultation selon les médecins. Les médecins ont souhaité pouvoir orienter leurs patients vers des sites fiables et contrôlés. La relation médecin-patient serait certainement améliorée si les médecins maitrisaient davantage l'outil internet et étaient formés à la gestion des patients-internautes.

Introduction : L’étude LOMBALNET vise à vérifier la faisabilité d’une démarche d’éducation thérapeutique via internet par le médecin généraliste en soins primaires. L’objectif principal était de mesurer le taux de visite à un site d’éducation thérapeutique sur proposition de leur médecin généraliste. Méthode : Neuf médecins généralistes ont invité cent dix patients à visiter un site d’éducation thérapeutique du patient concernant la lombalgie créée à cet effet, et nous avons enregistré le nombre de connexions. Le taux seuil de validité était fixé à 20%. Dans un deuxième temps un questionnaire de satisfaction était proposé aux visiteurs. Résultats : Parmi les 110 personnes incluses, 73 % ont visité le site. La partie du site la plus visionnée est celle dédiée aux conseils pratiques d’hygiène lombaire. La satisfaction concernant l’utilisation du site Internet est évalué à 8,15 (+/- 1,61) sur 10. Se rendre sur un site web à l’invitation de son médecin est jugé normal par 81 % des participants, et 92 % donnent plus d’importance à un site conseillé par leur médecin traitant. Conclusion : Les technologies de l’information et de la communication sont utilisables dans l’éducation thérapeutique du patient lorsque celle-ci est initiée par leur médecin généraliste.

Les sites de santé ont connu un développement important et constituent pour le patient une importante source d'informations médicales. L'un des premiers témoins de ce phénomène est le médecin soumis à l'information que peut lui présenter son patient. Du point de vue du généraliste, quels sont les facteurs facilitateurs et freinateurs influençant la relation médecin-malade lors de l'évocation par le patient d'informations de santé collectées sur internet? Nous avons mené une étude qualitative par entretiens semi-dirigés auprès de treize médecins généralistes charentais. Les médecins interrogés reconnaissent un usage régulier d'internet dans leur pratique professionnelle avec la consultation de sites médicaux spécialisés principalement. Ils perçoivent sans difficulté qu'internet est devenu pour le patient une source d'informations de santé au quotidien facile et rapide d'accès, pouvant peut-être répondre parfois à la difficulté d'obtenir un rendez-vous avec leur praticien. Notre étude a permis d'identifier, que l'évocation d'informations de santé par le malade en consultation, pouvait faciliter la communication verbale et non verbale au sein de la relation médecin-patient et favoriser la responsabilisation du malade dans son projet de soin par partage du savoir. Les médecins ont également identifié plusieurs freins dans ce contexte : sentiment de justification, remise en question de l'autorité médicale et perte de temps générée par la multiplication des explications et des échanges autour de ces informations de santé en ligne. Afin de mieux gérer l'intrusion d'internet dans la consultation et in fine dans la relation médecin-malade, les médecins de notre étude ont émis la nécessité de développer des sites de santé dédiés au patient, aux sources fiables et validées, et d'accompagner le patient sur internet dans une démarche pédagogique pour l'aider dans le choix de ses informations. La participation à des formations sur l'internet santé et la gestion du patient internaute, est également un élément identifié dans notre étude comme pouvant contribuer à améliorer la gestion du patient internaute dans le cadre d'une relation médecin-malade optimale. Notre étude a permis de percevoir le sentiment partagé des médecins vis à vis de l'internet santé sur la relation avec son patient en identifiant les facteurs facilitateurs et freinateurs influençant le lien soignant-soigné lors de l'évocation par le patient d'informations de santé en ligne. Se former à l'internet santé et à la gestion du patient internaute, pour mieux orienter le patient sur des sites de santé en ligne, fiables et validés, sont les moyens, identifiés par les médecins de notre étude, pour mieux gérer la consultation à l'avenir et la relation médecin-patient internaute.

L'objectif principal de l'étude était d’évaluer les conséquences de l’outil internet sur la relation médecin/malade, puis d’évaluer la place d’internet en matière de santé. Méthodes : Étude observationnelle descriptive et analytique auprès des patients majeurs et internautes de cabinets de médecine générale dans l’Oise et la Somme. Résultats : 456 patients ont répondu au questionnaire entre le 1er mai et le 1er août 2015. 76% s'estimaient en bonne santé et 92% en bonne entente avec leur médecin. Lorsque l'entente était mauvaise, la confiance était moins importante envers le médecin, et la recherche sur internet était plus importante.(p=0,0046) Une approbation de l'avis du médecin (p=0,003) ou une alternative (p=0,05) étaient recherchés. Donc, la confiance en son médecin s'altérait.(p=0,01) comme le désir de moins consulter.(p=0,0067) Discussion/Conclusion : Internet intervenait donc comme informateur en matière de santé tout comme le médecin ou le pharmacien. Il fragilisait les mauvaises relations médecin/malade. La reconnaissance des informations du net était nécessaire et confortait la nécessité d'éducation thérapeutique.
