Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.

Years ahead of its time, his profound critique became the foundation for studies of imperialism and economic development. Spain, in particular, has been transformed from an immigrant-exporting country to one receiving hundreds of thousands of new immigrants. Today, almost 13 percent of the country’s population is foreign-born. Spanish Legacies, written by internationally known experts on immigration, explores how the children of immigrants—the second generation—are coping with the challenges of adaptation to Spanish society, comparing their experiences with those of their peers in the United States. Using a rich data set based on both survey and ethnographic material, Spanish Legacies describes the experiences of growing up by the large population of second-generation youths in Spain and the principal outcomes of the process—from national self-identification and experiences of discrimination to educational attainment and labor-market entry. The study is based on a sample of almost 7,000 second-generation students who were interviewed in Madrid and Barcelona in 2008 and then followed and re-interviewed four years later. A survey of immigrant parents, a replacement sample for lost respondents in the second survey, and a survey of native-parentage students complement this rich data set. Outcomes of the adaptation process in Spain are systematically presented in five chapters, introduced by real-life histories of selected respondents drawn by the study’s ethnographic module. Systematic comparisons with results from the United States show a number of surprising similarities in the adaptation of children of immigrants in both countries, as well as differences marked by contrasting experiences of discrimination, self-identities, and ambition.

Book excerpt: In 1570's New Kingdom of Granada (modern Colombia), a new generation of mestizo (half-Spanish, half-indigenous) men sought positions of increasing power in the colony's two largest cities. In response, Spanish nativist factions zealously attacked them as unequal and unqualified, unleashing an intense political battle that lasted almost two decades. At stake was whether membership in the small colonial community and thus access to its most lucrative professions should depend on limpieza de sangre (blood purity) or values-based integration (Christian citizenship). A Tale of Two Granadas examines the vast, trans-Atlantic transformation of political ideas about subjecthood that ultimately allowed some colonial mestizos and indios ladinos (acculturated natives) to establish urban citizenship alongside Spaniards in colonial Santafé de Bogotá and Tunja. In a spirit of comparison, it illustrates how some of the descendants of Spain's last Muslims appealed to the same new conceptions of citizenship to avoid disenfranchisement in the face of growing prejudice.
