Book excerpt: Introduction En 2009, les anticoagulants oraux directs (AOD) font leur entrée sur le marché français. Les AOD sont une alternative aux antivitamines K (AVK). Connaissant déjà les différents déterminants de la prescription, nous avons voulu identifier les déterminants associés à un changement de prescription d'une anticoagulation orale. Méthode Nous nous sommes appuyés sur les données de l'étude CACAO, une étude de cohorte nationale, prospective, menée en 2014, réalisée auprès d'une population de patients sous traitement anticoagulant oral et bénéficiant d'un suivi par un médecin généraliste. Les données ont été collectées à l'aide d'un questionnaire auprès du médecin généraliste et d'un questionnaire électronique réalisé lors de l'inclusion de chaque patient. L'objectif est de décrire les facteurs associés au changement de prescription d'une anticoagulation orale. Résultats 406 médecins généralistes ont inclus 7 154 patients sous traitement anticoagulant oral. 5 699 patients éraient sous AVK et 1 455 sous AOD. Les facteurs associés à un changement de prescription sont : l'âge, le sexe féminin, l'absence d'antécédent d'IDM, un antécédent d'hémorragie hospitalisée, l'absence de traitement pas fibrate, avoir pour indication une FA, ne pas avoir pour indication une prothèse valvulaire .Discussion Notre échantillon de patients, semble être représentatif de la population générale des patients traités par anticoagulants oraux en France. Notre population de médecins est plus jeune, plus impliquée dans la recherche clinique et l'enseignement. Notre étude permet de mettre en évidence plusieurs facteurs représentatifs associés au changement de prescription d'une anticoagulation orale.

Book excerpt: Les recommandations sur l'utilisation des anticoagulants oraux directs évoluent et les particularités de prescription propres à chaque AOD peuvent rendre leur utilisation complexe en médecine générale. L'objectif principal de l'étude était d'évaluer les connaissances et pratiques sur leur utilisation en cabinet de médecine générale à la Réunion. Il s'agit d'une étude qualitative, prospective de type enquête déclarative via un questionnaire adressé aux médecins généralistes Réunionnais de Juin à Novembre 2016. 74 médecins généralistes ont répondu au questionnaire. 90% des médecins prescrivent des AOD et 51% en initient. La maladie thrombo-embolique veineuse et le maintien de l'INR difficile chez un patient sous AVK sont les deux indications majeures retenues pour l'initiation. 34% des médecins contreindiquent les AOD si le patient est âgé (>75ans). Dans les indications prévention et traitement des événements thrombo-emboliques veineux, le rivaroxaban a été le plus sélectionné. 12,5% des médecins interrogés n'envisagent pas d'examen complémentaire à l'initiation du traitement. Les situations complexes rencontrées au cabinet sont le plus souvent des complications hémorragiques. 11 médecins ont arrêté un traitement par AOD du fait d'un effet secondaire grave. Enfin 64% des médecins interrogés n'ont pas connaissance d'antidote. Les modalités d'initiation, les contre-indications et les indications des AOD sont connues par les médecins généralistes. Bien que les médecins ne soient pas réticent à la prescriptions d'AOD, les particularités propres à chaque anticoagulant et l'évolution des recommandations ne sont pas évidentes. Cela rend leur utilisation complexe au cabinet.

Book excerpt: Introduction : Les anticoagulants oraux occupent une place importante dans l'arsenal thérapeutique du médecin de par leurs nombreuses indications. L'objectif de notre travail était d'établir un état des lieux descriptif de la prescription des anticoagulants oraux dans des cabinets de médecine générale. Méthode : Etude observationnelle descriptive basée sur un recueil de données, effectuée entre février et mai 2016 dans des cabinets de médecine générale en Ariège et dans le Gers. Résultats : Les AVK sont les anticoagulants oraux les plus prescrits, la FANV est l'indication principale de prescription. Les traitements anticoagulants oraux sont majoritairement prescrits par les spécialistes. Il n'y a pas de différences significatives entre AVK et AOD en fonction des caractéristiques du patient telles que le genre, l'âge, l'indication de prescription et la spécialité du prescripteur initial. Les patients ayant changé de traitement anticoagulant oral sont en majorité des hommes, de plus de 75 ans, porteurs d'une FANV. L'apixaban est l'AOD le plus prescrit en cas de changement de thérapeutique. Les spécialistes ont une place prépondérante dans les switches médicamenteux. Conclusion : Nous n'avons pas retrouvé de différences significatives entre les profils des patients sous AVK et sous AOD. Les patients ayant changé de thérapeutique anticoagulante ont un profil similaire aux patients en primoprescription.
