Il constitue un enjeu de santé publique majeur. Il représente la 3e cause de mortalité en France (10% des décès), la 2e cause de démence, et la 1e cause de handicap non traumatique de l'adulte dans les pays économiquement développés. Grâce à une meilleure connaissance du mécanisme des AVC et aux progrès considérables de l'imagerie cérébrale, la prise en charge a été fondamentalement améliorée. Une partie des AVC ischémiques peut bénéficier d'un traitement par fibrinolyse, dans un délai de 3 heures suivant l'apparition des symptômes. L'AVC doit donc être considéré comme une urgence thérapeutique. Le but de cette étude prospective, réalisée sur le Centre Hospitalier de Vichy, était d'étudier les conditions d'accès aux soins des patients présentant une suspicion d'AVC. 56 patients ont été inclus sur une durée de 7 semaines. Dans l'effectif de patients à AVC ischémique (n=35), 2 ont bénéficié d'un scanner cérébral dans les 3 heures. Cette thèse analyse les sources de retard dans la prise en charge médicale. En conclusion, il ressort que l'amélioration de la prise en charge de ces patients passe par une meilleure information du grand public, et par une amélioration structurelle et fonctionnelle des centres hospitaliers.
