Statistics indicate that it is exacting a great and relentlessly increasing toll of human suffering. It is clear that the problem is not being dealt with in any effective manner. At the invitation of the. Canadian Hepatic Foundation, many of the world's experts gathered in Toronto, May 14-15 1976, to focus attention on one of the most important aspects of the alcohol problem -alcohol induced liver damage. The epidemiology of alcohol induced liver disease was discussed and current views on the pathogenesis of the problem were reviewed. New insight into the pathological alterations of the liver was presented and some of our current therapeutic capabilities were discussed. Dr. Hans Popper summarized the Symposium and presented some of his views on those aspects of the problem which will require early attention by the research community. The Symposium achieved its immediate objective -that of bringing together the committed experts of various disciplines for an updating of our understanding of alcohol and the liver and for a discussion of new approaches to the problem. As a backdrop to the Symposium, however, was large writing on the wall to the effect that we are expending our research talents and efforts on a totally unnecessary problem. Right now we probably know enough and have sufficient resources at our disposal to solve the problem.

Human interest in alcohol dates back to the Neolithic period (circa 10,000 BC). Only a small amount of alcohol is found naturally in food. Therefore, humans have yet to adapt to the high quantities of alcohol consumed daily by alcohol abusers. Alcohol is a simple carbohydrate and is metabolised as a fuel. However, unlike more naturally occurring carbohydrates such as glucose, the pathways for its metabolism are not subject to the same checks and balances. Ethanol oxidation reduces NAD+, which starves reactions that depend on this substrate. This accounts for the accumulation of fat in alcoholic steatosis. Alcohol metabolism generates significant amounts of reactive oxygen species (ROS), which damages mtDNA and subsequently mitochondrial integrity. The authors provide several lines of evidence that link the clinical picture of alcoholic liver disease with mitochondrial damage. Other important topics highlighted in this book include histo-pathological aspects of the disease. Clinical assessment of nutrition and vitamin supplementation is addressed as well. The book concludes with the highly topical and controversial aspect of liver transplantation in patients with alcoholic liver disease.

Social, religious, and gender-related barriers to alcohol consumption are loosening, and the financial improvement of some segments of populations now enable them to buy alcoholic beverages. Thus the greatest percentage rise in the United States has recently been in black women. Adolescents and young people drink more alcoholic beverages than ever, and growing alcohol abuse by pregnant women has let to an increase of the incidence of the fetal alcohol syndrome. While the social and behavioral, including psychiatric, consequences of alcoholism are staggering, the gastrointestinal and, particularly, hepatic manifestations are the most widespread somatic effects, and chronic hepatic disease in alcoholics appears to cause the greatest cost to society. Indeed, mortality from liver cirrhosis is considered a reliable index of alcohol consumption in a country.

This effect is due to its toxicological influence on the central nervous system with the resulting functional disturbances. For many years, the concept that alcoholic liver disease was of nutritional origin and only indirectly related to alcohol consumption and metabolism was accepted. Opinion has changed gradually in recent years and tends now to regard alcohol itself through its combustion, as responsible for many metabolic disorders. Interest in this problem has increased during the past decade, and numerous papers bearing on this subject have appeared. It seems that the oxidation of alcohol in the liver interferes in many ways with the intermediary metabolism of lipids, carbohydrates and proteins, including enzymes and hormones, and exerts damaging influence on the liver, the musculature, the heart, the brain and the kidneys. The "Workshop Symposium" brought together a limited number of the scientists involved in the new development, biochemists, physiologists, pharmacologists, pathologists and clinicians from eight European countries and from the United States and allowed a very lively and informal exchange. The symposium was honored by the presence of Sir Hans Krebs, whose vast experience stimulated the discussion. It is a great pleasure to dedicate this volume to Sir Hans on the occasion of his 70th birthday on August 25th.
