Book excerpt: Les travaux présentés dans cette thèse s'inscrivent dans le contexte du traitement des images cardiaques et l'étude de la fonction contractile du coeur, pour une meilleure compréhension de physiopathologie cardiaque et l'aide au diagnostic. Nous avons implémenté une méthode de segmentation des parois endocardiques sur des ciné-IRM non marquées. Nous avons utilisé une approche fondée sur les ensembles de niveaux, avec une formulation basée-région qui donne des résultats satisfaisants sur des cas sains et pathologiques. Nous avons proposé une méthode pratique pour la quantification des déformations segmentaires afin decaractériser la contractilité myocardique. La méthode a fait l'objet d'une validation clinique par l'expertise de médecins et par comparaison à la méthode HARP sur des IRM marquées. Pour améliorer la précision des mesures, nous avons proposé un algorithme de recalage iconique multimodal IRM/TDM, utilisant la maximisation de l'information culturelle. Nous l'avons appliqué à la localisation de coupes petit-axe dans des volumes TDM avec des résultats encourageants. Ce travail a pour perspective son application à l'obtention de séquences TDM de hautes résolutions spatiale et temporelle.

A ce jour, l'étude des mouvements pariétaux du ventricule gauche par imagerie ultrasonore, est essentiellement qualitative. Nous proposons, dans ce manuscrit, des solutions automatisées à ce problème, afin d'être en mesure d'estimer quantitativement, pour chaque instant d'un cycle cardiaque, les contributions de chacun des mouvements pariétaux du ventricule gauche par imagerie ultrasonore selon la vue petit axe. Dans un premier temps, nous avons développé une interface de segmentation conviviale qui permette d'isoler la paroi myocardique de chaque image d'une séquence d'échocardiographie. Notre méthode exploite les caractéristiques et les spécificités d'acquisition des images ultrasonores. Une validation sur vingt trois séquences d'images de patients sains est venue corroborer l'intérêt de notre méthode. Cette segmentation pariétale a permis la détermination automatique des épaisseurs régionales de la paroi myocardique pour chaque image d'une séquence cardiaque, ainsi que le calcul automatique des épaississements systoliques régionaux. Dans un deuxième temps, à partir d'une approche basée sur la combinaison des informations de mouvements issues issuesà la fois des images d'échocardiographie et des images Doppler tissulaires, nous présentons une méthode de quantification des trois composantes de vitesses du ventricule gauche en vue parasternale petit axe : rotation, contraction et translation. Afin de faciliter la lisibilité des résultats, nous proposons des images reconstruites représentant la contraction régionale du myocarde pour un cycle cardiaque. Les résultats prometteurs obtenus chez des patients sains, tant au niveau des vitesses de contraction que des vitesses de rotation pariétales, appellent d'autres validations sur un plus grand nombre de cas, mais aussi des applications de cette méthode à la caractérisation quantitative de pathologies.

Book excerpt: La segmentation des structures cardiaques en Imagerie ultrasonore est un problème qui reste ouvert et particulièrement difficile. En effet le processus physique de formation des images échographiques (interaction de l'onde ultrasonore avec les tissus biologiques explorés) conduit à des Images bruitées (phénomène de "speckle') et à un faible contraste. Du tait de ces difficultés la plupart des recherches ont été consacrées à la segmentation de l'endocarde (interface interne entre le sang du muscle cardiaque) et très peu d'études abordent la segmentation de l'épicarde (interface externe). Afin de réaliser une segmentation complète du myocarde, nous avons proposé une méthode intégrant l'information de mouvement dans le processus de segmentation. De ce fait la première partie de ce travail est consacrée à l'estimation de mouvement dans des séquences d'images échocardiographiques. La plupart des techniques d'estimation souffrent de limitations dues à la complexité de la formation des images ultrasonores. Nous avons caractérisé ces limitations pour des méthodes d'estimation de mise en correspondance de blocs, à partir de simulations réalistes d'images ultrasonores et sur des données in vivo du cœur. Cette étape nous a permis de choisir une méthode d'estimation fiable et exploitable sur des données réelles. La segmentation que nous avons développée appartient aux méthodes de type contours actifs. Notre approche est basée sur une "formulation par ensemble de niveaux variationnels, évoluant à partir des statistiques locales de l'image et intègre un a priori de forme simple (elliptique). L'originalité de la méthode repose sur l'utilisation de l'information de mouvement pour propager la contrainte de forme par un filtre de Kalman. Cette méthode a été évaluée sur des données réelles acquises in vivo et représentant 120 images traitées. L'évaluation a été réalisée en comparant les contours obtenus avec ceux tracés par un cardiologue.

The excep tional achievements of mountaineers who have climbed nearly all peaks over 8,000 m without breathing equipment raise the question of maximal adaptation ca pacity of man to low oxygen pressures. More importantly, the increase in tourism in the Andes and the Himalayas brings over 10,000 people to sites at altitudes above 4,000 and 5,000 m each year. At such heights several kinds of high alti tude diseases are likely to occur, and these complications require detailed medical investigations. Medical authorities need to inform both mountaineers and tourists as to how great a physical burden can be taken in the mountain environment without risk to health. Physicians need to know what kind of prophylaxis is to be employed at high altitudes to prevent the development of diseases and what therapeutic measures should be used once high altitude diseases have occurred. Moreover, the physical condition of the indigenous population living at higher altitudes such as the Andes and the Himalayas, who are exposed continuously to the stress of high altitude, requires our attention. We have become familiar with symptoms characteristic of chronic high-altitude disease: under special conditions this popu lation has a tendency to develop pulmonary hypertension, which is associated with pulmonary edema, pulmonary congestion, and right heart failure.

Book excerpt: The partition of fluid between the vascular and interstitial compartments is regulated by forces (hydrostatic and oncotic) operating across the microvascular walls and the surface areas of permeable structures comprising the endothelial barrier to fluid and solute exchange, as well as within the extracellular matrix and lymphatics. In addition to its role in the regulation of vascular volume, transcapillary fluid filtration also allows for continuous turnover of water bathing tissue cells, providing the medium for diffusional flux of oxygen and nutrients required for cellular metabolism and removal of metabolic byproducts. Transendothelial volume flow has also been shown to influence vascular smooth muscle tone in arterioles, hydraulic conductivity in capillaries, and neutrophil transmigration across postcapillary venules, while the flow of this filtrate through the interstitial spaces functions to modify the activities of parenchymal, resident tissue, and metastasizing tumor cells. Likewise, the flow of lymph, which is driven by capillary filtration, is important for the transport of immune and tumor cells, antigen delivery to lymph nodes, and for return of filtered fluid and extravasated proteins to the blood. Given this background, the aims of this treatise are to summarize our current understanding of the factors involved in the regulation of transcapillary fluid movement, how fluid movements across the endothelial barrier and through the interstitium and lymphatic vessels influence cell function and behavior, and the pathophysiology of edema formation. As far as my own research was able to ascertain, the last comprehensive work dealing with this subject was writ ten in Germany by the Dresden internist, F. Lickint, whose Tabak und Organismus was published in 1939 by the Hip pokrates-Verlag. All subsequent monographs in this field have tended to focus on detailed aspects, and there has been no shortage of publications on subjects such as how smokers can quit smoking, healthy eating for smokers etc. Friends and colleagues abroad have urged me to prepare an English language version of Tabakabhiingigkeit. In gladly complying with this suggestion, I have intentionally prepared an up dated and slightly enlarged new edition, taking account of the rapidly proliferating literature on the subject up to the start of 2002. The harmful sequelae of smoking are played down by politicians in many industrialised countries, including Ger many.
