Book excerpt: Excerpt from Essays Clinical Medicine: Being the I Being Reprints of Papers Published at Various Times in the "American Journal of the Medical Sciences" The use of creasote in tlie treatment of phthisis pulmonalis dates back to 1830, the year in which its was discovered by Keichenbach, of Blausko, in Moravia. Later on, it fell into disuse, like some other valuable medicaments, and for nearly thirty years previous to 1877 it was practically of little or no importance in the therapeutics of pulmonary disease. At the date just referred to, Drs. Bouchard and Gimbert again revived interest in this drug by publishing a very complete article in the Gazette IMdomaduire, of Paris," on its beneficial effects in consumption. They claimed for it excellent results, and for careful, learned, and honest observers, their account, was as nearly enthusiastic as thorough work is apt to be. Shortly after reading of the observations 01 Bouchard and Gimbert, I began to use creosote in the treatment of pulmonary phthisis, and in a clinical lecture delivered at the Bellevue Hospital Medical College, and published id the Medical Record of September 21,1878, page 223,1 mention in what manner I have used it and with what results in the following terms: "For several months, both in the Out-door Department of the New York Hospital, at Charity Hospital and elsewhere, I have given dessert-spoonful doses of the mistura creasoti of our Pharmacopoeia to lessen the quantity of sputa in phthisis. I am very much pleased with this remedy, and believe it merits a very extensive trial. No doubt, in lessening the abundance of expectoration, it also diminishes the frequency of cough." At that time, I would add, I regarded creosote aa being only a good anti-catarrhal agent, to be ranked high amongst some analogous remedial substances whose main action consisted in the diminution of the amount, of purulent sputa, but which had little or no curative effect upon the essential lesions of pulmonary phthisis, aa we understand them. The favor with which they have been received both at home and abroad, and the wish expressed in many quarters that they might be arranged in a more permanent form, have led to their collection and republication in one volume. When the rebellion broke out, there were but few works treating of medical subjects from a military point of view which were attainable by the thousands of physicians who entered the army. That much good has been accomplished by the essays contained in this volume, scarcely admits of a doubt. The demand for them has been immense, and all of them have been reprinted several times. Though at the present day, when through the large experience winch our army medical officers have acquired, such monographs are scarcely necessary for their use, there are continually new candidates for military service coming forward to whom they cannot be too highly recommended. Not only are individuals improvident of health, but public communities neglect precautions that would avert many attacks of disease; and even governments, having control of armies and navies, are unmindful of preventive measures which would diminish the expense and promote the efficiency of these bodies. It ought to be unnecessary to urge the importance of naval hygiene. If it be so requisite to study what to do and what to leave undone on shore, where everything demanded for the healthy maintenance of the body is in abundance, how much more strictly ought the laws of health to be observed on board ship, where human beings are crowded together in violation of all these laws, breathing a scanty supply of air vitiated by the retention of their own excretions, subsisting upon an unwholesome diet, their sleep always interrupted, and their minds continually disquieted by passions called into operation by the unnatural circumstances of their lives. Yet no sanitary code has ever been promulgated in our own service, nor, until recent years, has it been attempted elsewhere. The young medical officer is without a guide. As much confused by the manners of those around him as by the maze of rigging overhead, he credits whatever he is told and accepts, "it is the custom of the service," as palliating whatever appears barbarous and unnatural.
