Book excerpt: We study the costs of hospitalizations on patients’ earnings and labor supply, using the universe of hospital admissions in Denmark and full-population tax data. We evaluate the quality of treatment based on its ability to mitigate the labor market consequences of a given diagnosis and propose a new measure of hospital quality, the "Adjusted Earning Losses" (AEL). We find a 4 percentage points difference in lost earnings between the best and worst large Danish hospitals, all else equal. We show that AEL contains significant additional information relative to traditional measures and does not suffer from worse selection issues. We also document a large decline in the labor cost of hospitalizations over time, with large variations across diseases. We find that the average post-hospitalization reduction in labor earnings declined by 25 percent (50 percent) on the intensive (extensive) margin between 1998 and 2012.

It contains a selection from almost 150 papers presented at the Second World Congress on Health Economics, held at the University of ZUrich, Switzerland, 10-14 September 1990. This is ten years after the last Congress (held in Leiden, The Netherlands) and more than 17 years after the Tokyo meeting organized by the International Economic Associ ation (which might be called the First World Congress were it not for its small size and symposium-like character). Out of the 16 papers appearing in this book, only four were invited, and they were refereed like all the others. The remaining contributions are survivors of stiff competition. However, in spite of the large sample of papers we were able to draw from, certain gaps of coverage persist. In particular, environmental impacts on health were designated as a field of priority interest in the call for papers to this Congress. Yet, not even half a dozen contribu tions were submitted, which is remarkable in view of the intense interest of the public in the health effects of environmental degradation, ranging from noise level to ozone concentration. Another specially designed field was the political economy of health, dealing with the behavior of policy makers when deciding about changes in the health care systems. It did not attract as many contributions as we had hoped for or expected.

It may be considered an implementation of S, an award-winning language initially - veloped at Bell Laboratories since the late 1970s. The R project was initiated by Robert Gentleman and Ross Ihaka at the University of Auckland, New Zealand, in the early 1990s, and has been developed by an international team since mid-1997. Historically, econometricians have favored other computing environments, some of which have fallen by the wayside, and also a variety of packages with canned routines. We believe that R has great potential in econometrics, both for research and for teaching. There are at least three reasons for this: (1) R is mostly platform independent and runs on Microsoft Windows, the Mac family of operating systems, and various ?avors of Unix/Linux, and also on some more exotic platforms. (2) R is free software that can be downloaded and installed at no cost from a family of mirror sites around the globe, the Comprehensive R Archive Network (CRAN); hence students can easily install it on their own machines. (3) R is open-source software, so that the full source code is available and can be inspected to understand what it really does, learn from it, and modify and extend it. We also like to think that platform independence and the open-source philosophy make R an ideal environment for reproducible econometric research.

The Applied Econometrics Association has devoted to the topic two symposia in less than three years (Lyons, February 1983; Rotterdam, December 1985), without experiencing any difficulties in getting valuable papers: on econometrics of risks and medical insurance, on the measurement of health status and of efficiency of medical techniques, on general models allowing simulation. These were the themes for the second meeting, but other aspects of medical-economic problems had presented themselves already to the analyst: medical decision making and its consequences, the behaviour of the actors - patients and physicians -, regional medicometrics and what not: some of them have been covered by the first meeting.
