Book excerpt: With the American military withdrawal, the Taliban's seizure of control, and a developing humanitarian crisis, the United States faces a question of what policy it should pursue toward the Islamic Emirate of Afghanistan. To inform U.S. policymakers, the authors of this Perspective identify the remaining American interests in Afghanistan-principally counterterrorism and humanitarian relief-and propose a framework to evaluate three different U.S. overall policy approaches: to engage with the Taliban, to isolate the regime, or to oppose the Taliban by seeking to remove them from power. The authors identify the conditions under which these policies may be most appropriate and how they would best serve U.S. interests. They conclude that engagement offers the only prospect of advancing American interests in the country. They caution, however, that isolation is the more usual U.S. response to an unwelcome change in regime. With its embassy closed and a comprehensive sanctions regime in place, this will become the default U.S. policy toward Afghanistan in the absence of contrary decisions.

Isolate or Engage systematically examines the challenges to and opportunities for U.S. diplomatic relations with nine intensely adversarial states—China, Cuba, Iran, Libya, North Korea, U.S.S.R./Russia, Syria, Venezuela, and Vietnam: states where the situation is short of conventional war and where the U.S. maintains limited or no formal diplomatic relations with the government. In such circumstances, "public diplomacy"—the means by which the U.S. engages with citizens in other countries so they will push their own governments to adopt less hostile and more favorable views of U.S. foreign policies—becomes extremely important for shaping the context within which the adversarial government makes important decisions affecting U.S. national security interests. At a time when the norm of not talking to the enemy is a matter of public debate, the book examines the role of both traditional and public diplomacy with adversarial states and reviews the costs and benefits of U.S. diplomatic engagement with the publics of these countries. It concludes that while public diplomacy is not a panacea for easing conflict in interstate relations, it is one of many productive channels that a government can use in order to stay informed about the status of its relations with an adversarial state, and to seek to improve those relations.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

This book analyses how the Bush administration understood and pursued its interests in the Central Asia states, namely Kyrgyzstan, Tajikistan, Kazakhstan, Uzbekistan, Turkmenistan. It discusses the shift in US interests after September 11 and highlights key ideas, actors and processes that have been driving US policy in Central Asia. The author examines the similarities between the Bush and Obama administrations’ attitudes towards the region, and he points to the inadequacy of the personality focused, partisan accounts that have all too often been deployed to describe the two presidential administrations. To understand US Central Asian policy, it is necessary to appreciate the factors behind its continuities as well as the legacies of the September 11 attacks. Using case studies on the war on terror, energy and democracy, drawing on personal interviews with Americans and Central Asians as well as the fairly recent releases of declassified and leaked US Government documents via sources like the Rumsfeld Papers and Wikileaks, the author argues that the US approached Central Asia as a non-unitary state with an ambiguous hierarchy of interests. Traditionally domestic issues could be internationalised and non-state actors were able to play significant roles. The actual relationships between its interests were neither as harmonious nor as conflicted as the administration and some of its critics claimed. Shedding new light on US relations with Central Asia, this book is of interest to scholars of Central Asia, US Politics and International Relations.

This model enables a comprehensive analysis of international engagement's scope, areas, and methods. Further, the book also explores the context of engagement in de facto states, which has a significant impact on its implementation and results. In this way, the book also advances our understanding of the opportunities, obstacles, and limitations of engagement without recognition. The analysis is based on the current EU engagement in Abkhazia and draws from other cases in the Caucasus, Eastern Europe, and beyond and finds that international engagement with de facto states is more comprehensive and multifaceted than previously known. However, it also faces some distinct challenges and produces modest results. Finally, the book provides practical recommendations on how to better utlilise the peacebuilding potential of engagement without recognition. Civic engagement, social cohesion, and other dimensions of social capital affect social, economic and health outcomes for individuals and communities. Can these be measured, and can federal surveys contribute toward this end? Can this information be collected elsewhere, and if so, how should it be collected? Civic Engagement and Social Cohesion identifies measurement approaches that can lead to improved understanding of civic engagement, social cohesion, and social capital - and their potential role in explaining the functioning of society. With the needs of data users in mind, this report examines conceptual frameworks developed in the literature to determine promising measures and measurement methods for informing public policy discourse. The report identifies working definitions of key terms; advises on the feasibility and specifications of indicators relevant to analyses of social, economic, and health domains; and assesses the strength of the evidence regarding the relationship between these indicators and observed trends in crime, employment, and resilience to shocks such as natural disasters. Civic Engagement and Social Cohesion weighs the relative merits of surveys, administrative records, and non-government data sources, and considers the appropriate role of the federal statistical system. This report makes recommendations to improve the measurement of civic health through population surveys conducted by the government and identifies priority areas for research, development, and implementation.
