GAO noted that: (1) VA health care has evolved from a system primarily providing hospital care to veterans injured during wartime to a system focused on the treatment of low-income veterans with medical conditions unrelated to military service; (2) the eligibility provisions of the VA health care system are vague, provide uneven services, do not guarantee services, and cannot provide the total care that low-income veterans need; (3) while VA health care eligibility provisions should be reformed to better suit veterans' health care needs, none of the proposed legislation would be budget neutral; (4) present eligibility provisions do not cause inappropriate hospitalizations; (5) several legislative proposals to reform VA eligibility provisions would eliminate the restrictions on outpatient care and create a uniform benefit package, but inappropriate use of resources, uneven access, and some restrictions would continue to exist; (6) the proposed legislation to reform VA eligibility provisions would increase service availability, demand, and program costs; and (7) approaches for developing budget-neutral eligibility reforms include limits on covered benefits, limits on the number of veterans eligible, an increase in veteran cost-sharing, the authorization of recoveries from Medicare, VA retention of a portion of third-party recoveries, and reinvestment of savings from efficiency improvements.

GAO noted that: (1) veterans' eligibility for VA health care has evolved over time; (2) VA has gone from a system that provides hospital care for veterans with war-related injuries to a system that covers a wide array of medical services for all veterans; (3) eligibility for VA health care has grown complex and includes multiple categories for veterans based on period of service, seriousness of service-connected disability, and income; (4) veterans' eligibility continues to be based on their need for hospital-related care; (5) unlike other public and private insurance programs, there are no guaranteed benefits under the VA health care system; (6) the provision of VA health services is conditioned on the availibility of space and resources; and (7) alternative approaches for restructuring VA health care eligibility include eliminating the restrictions on access to ambulatory care, creating an uniform benefit package, expanding veterans' entitlement to free care, and changing the VA funding structure.

In Generation Unbound, Isabel V. Sawhill offers a third approach: change "drifters" into "planners." In a well-written and accessible survey of the impact of family structure on child well-being, Sawhill contrasts "planners," who are delaying parenthood until after they marry, with "drifters," who are having unplanned children early and outside of marriage. These two distinct patterns are contributing to an emerging class divide and threatening social mobility in the United States. Sawhill draws on insights from the new field of behavioral economics, showing that it is possible, by changing the default, to move from a culture that accepts a high number of unplanned pregnancies to a culture in which adults only have children when they are ready to be a parent.

Book excerpt: Adolescence is a distinct, yet transient, period of development between childhood and adulthood characterized by increased experimentation and risk-taking, a tendency to discount long-term consequences, and heightened sensitivity to peers and other social influences. A key function of adolescence is developing an integrated sense of self, including individualization, separation from parents, and personal identity. Experimentation and novelty-seeking behavior, such as alcohol and drug use, unsafe sex, and reckless driving, are thought to serve a number of adaptive functions despite their risks. Research indicates that for most youth, the period of risky experimentation does not extend beyond adolescence, ceasing as identity becomes settled with maturity. Much adolescent involvement in criminal activity is part of the normal developmental process of identity formation and most adolescents will mature out of these tendencies. Evidence of significant changes in brain structure and function during adolescence strongly suggests that these cognitive tendencies characteristic of adolescents are associated with biological immaturity of the brain and with an imbalance among developing brain systems. This imbalance model implies dual systems: one involved in cognitive and behavioral control and one involved in socio-emotional processes. Accordingly adolescents lack mature capacity for self-regulations because the brain system that influences pleasure-seeking and emotional reactivity develops more rapidly than the brain system that supports self-control. This knowledge of adolescent development has underscored important differences between adults and adolescents with direct bearing on the design and operation of the justice system, raising doubts about the core assumptions driving the criminalization of juvenile justice policy in the late decades of the 20th century. It was in this context that the Office of Juvenile Justice and Delinquency Prevention (OJJDP) asked the National Research Council to convene a committee to conduct a study of juvenile justice reform. The goal of Reforming Juvenile Justice: A Developmental Approach was to review recent advances in behavioral and neuroscience research and draw out the implications of this knowledge for juvenile justice reform, to assess the new generation of reform activities occurring in the United States, and to assess the performance of OJJDP in carrying out its statutory mission as well as its potential role in supporting scientifically based reform efforts.

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.
