Available in PDF, EPUB and Kindle. Book excerpt: Access to health care services for American Indians and Alaska Natives has been a long standing concern.1 The Indian Health Service (IHS), an agency within the Department of Health and Human Services (HHS), is charged with providing health care to the approximately 1.9 million American Indians and Alaska Natives who are members or descendants of federally recognized tribes.2 These services are provided at federally or tribally operated health care facilities,3 which receive IHS funding and are located in 12 geographic regions overseen by IHS area offices.4 These IHS-funded facilities vary in the services that they provide. For example, some facilities offer comprehensive hospital services, while others offer only primary care services. When services are not available at these facilities, the agency's contract health services (CHS) program may pay for services from external health care providers, including hospital- and office-based providers. The CHS program is administered at the local level by individual CHS programs generally affiliated with IHS-funded facilities in each area. These individual CHS programs may be federally or tribally operated. These federal and tribal CHS programs determine whether or not to pay for the referral of a patient to an external provider or pay an external provider for a service already provided. IHS requires that patients meet certain eligibility and administrative requirements to have the services paid by the CHS program. In addition, the CHS program, which is funded through the annual appropriations process, must operate within the limits of its appropriations. Therefore, committees associated with each CHS program meet at least weekly to review cases and approve payment based on the relative medical need of each case. When the requirements have not been met or funds are not available, CHS programs defer or deny requests to pay for services. Services for which patients otherwise meet necessary requirements, but for which CHS program funds are not available for payment, are known as unfunded services. Limits on available resources have affected the specific types of services available to American Indians and Alaska Natives through the CHS program. For example, in a 2005 report examining 13 IHS-funded health care facilities, we reported that primary care services were generally offered at the facilities, but certain specialty and other services were not always directly available to American Indians and Alaska Natives.5 These facilities also generally lacked funds to pay for all of these services through their CHS programs. We also noted that, in some cases, gaps in services resulted in diagnosis or treatment delays that exacerbated the severity of a patient's condition and required more intensive treatment.

Book Health Care Utilization as a Proxy in Disability Determination

This book was released on 2018-04-02 with total page 161 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Social Security Administration (SSA) administers two programs that provide benefits based on disability: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

Available in PDF, EPUB and Kindle. Book excerpt: Pursuant to congressional requests, GAO reviewed the availability of health care services to American Indians and Alaska Natives residing in five Indian Health Service (IHS) areas, focusing on: (1) the differences in health care delivery in the five areas; (2) the availability of basic health care services; and (3) major health care needs warranting additional attention. GAO found that: (1) two IHS areas provided most health care services through large medical centers, two areas relied on public or private contractors to provide essential services, and one service area provided hospital and contractor-provided care; (2) health care services, including basic medical and surgical treatment and diagnostic services, are generally available in all five areas, but the availability of preventive and dental care services is limited; (3) Indians do not utilize available preventive care services; (4) IHS officials are concerned that contract-care problems including funding limitations, procedural burdens, and restrictive eligibility criteria lessen the availability of some nonemergency services; (5) geographic isolation of the service population and the existence of alternate health programs such as Medicaid has affected the availability of services in many areas; (6) alcohol and substance abuse services and related mental health services are the greatest unmet health care needs in IHS service areas; (7) Congress has expanded IHS authority and increased funding for alcoholism and substance abuse prevention and treatment services; (8) IHS has no comprehensive data on the rates of alcoholism and substance abuse in Indian communities, tribal initiatives, or the effectiveness of IHS and tribal prevention and treatment programs; and (9) IHS is seeking funding for a study of alcohol and substance abuse in Indian communities and has discussed research needs with the National Institutes of Health (NIH) and the Substance Abuse and Mental Health Services Administration (SAMHSA).

Book Indian Health Service

Download or read book Indian Health Service written by Isaac Dubois and published by Nova Science Publishers. This book was released on 2014 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Indian Health Service (IHS) is an agency in the Department of Health and Human Services (HHS) that provides health care to American Indians and Alaska Natives. When care at an IHS-funded facility is unavailable, IHS's Contract Health Service (CHS) program pays for care from non-IHS providers if the patient meets certain requirements and funding is available. The Patient Protection and Affordable Care Act requires the Government Accountability Office (GAO) to study the administration of the CHS program, including a focus on the allocation of funds. IHS uses three primary methods to determine the allocation of CHS funds to the 12 IHS geographic area offices: base funding, which accounts for most of the allocation; annual adjustments; and program increases, which are provided to expand the CHS program. This book examines (1) the extent to which IHS's allocation of CHS funding varied across IHS areas, and (2) what steps IHS has taken to address funding variation within the CHS program.

Download or read book Leadership by Example written by Institute of Medicine and published by National Academies Press. This book was released on 2003-05-21 with total page 206 pages. Available in PDF, EPUB and Kindle. Book excerpt: The federal government operates six major health care programs that serve nearly 100 million Americans. Collectively, these programs significantly influence how health care is provided by the private sector. Leadership by Example explores how the federal government can leverage its unique position as regulator, purchaser, provider, and research sponsor to improve care - not only in these six programs but also throughout the nation's health care system. The book describes the federal programs and the populations they serve: Medicare (elderly), Medicaid (low income), SCHIP (children), VHA (veterans), TRICARE (individuals in the military and their dependents), and IHS (native Americans). It then examines the steps each program takes to assure and improve safety and quality of care. The Institute of Medicine proposes a national quality enhancement strategy focused on performance measurement of clinical quality and patient perceptions of care. The discussion on which this book focuses includes recommendations for developing and pilot-testing performance measures, creating an information infrastructure for comparing performance and disseminating results, and more. Leadership by Example also includes a proposed research agenda to support quality enhancement. The third in the series of books from the Quality of Health Care in America project, this well-targeted volume will be important to all readers of To Err Is Human and Crossing the Quality Chasm - as well as new readers interested in the federal government's role in health care.
