Book excerpt: Objectif: La rééducation cardiovasculaire est un moyen thérapeutique recommandé et validé en cas de maladie cardiovasculaire. Elle a un effet bénéfique bien établi sur les capacités fonctionnelles et sur la qualité de vie des patients, qui est lié à des adaptations périphériques vasculaires, musculaires, et respiratoires. L'existence d'un effet sur la fonction cardiaque elle même est moins évident. La plupart des études utilisant l'échographie 2D standard n'ont pas retrouvé d'amélioration significative de la fraction d'éjection ventriculaire gauche (FEVG) après rééducation. La nouvelle technique d'échographie en Speckle Tracking Imaging (STI) permet d'évaluer les anomalies subtiles de la fonction cardiaque en étudiant la déformation des fibres myocardiques. Notre hypothèse principale était que chez les patients atteints de maladie cardiovasculaire, une rééducation cardiovasculaire permettrait une amélioration de la contractilité myocardique du ventricule gauche (VG), évaluée en STI par la mesure du strain longitudinal global (SLG). Méthode: Cette étude prospective réalisée au CHU de Nîmes a inclus 30 patients d'âge moyen 5 5± 10 ans, dont 73 % de sexe masculin, avec une FEV G moyenne de 5 3± Il %, hospitalisés pour une rééducation cardiovasculaire de 4 semaines dans les suites d'un syndrome coronarien aigu (63%), d'une chirurgie cardiaque (30%), ou d'une insuffisance cardiaque chronique (7%). Les paramètres d'échographie standard ainsi que le SLG étaient mesurés et comparés, avant et après la rééducation. Nous avons également comparé la charge maximale de l'épreuve d'effort et les résultats du questionnaire de qualité de vie, réalisés par les patients avant et après la rééducation Résultats: Nous n'avons pas retrouvé, de modification significative de la FEVG ou du SLG après la rééducation. Il existe par contre une amélioration significative de la charge maximale de l'épreuve d'effort (p=0,027) et de la qualité de vie (limitations dues à l'état physique p=0,009, et limitations dues à l'état psychique p=0,042). Conclusion: Dans notre étude, la rééducation cardiovasculaire n'a pas entraîné d'amélioration de la fonction contractile du VG, mais a permis une amélioration significative de la tolérance à l'effort des patients, avec un effet bénéfique sur leurs capacités physiques et leur qualité de vie
