A treatment option for correcting moderate asymmetries is asymmetric extractions. This study evaluated post-treatment dental arch symmetry in patients treated with unilateral premolar extractions. Post-treatment casts of 60 patients were divided into four treatment groups based on the history of occlusal asymmetry and the treatment plan. DesignCAD3000 software (Upperspace Corporation, Pryor, OK) was used to evaluate asymmetrically treated arches for symmetry. The four treatment groups were compared to identify differences in arch asymmetry based on treatment. The lateral incisor and canine were found to be more palatal on the extraction side in patients treated with unilateral extractions (P .001). Book excerpt: Introduction: Anchorage and vertical control are some of the limiting factors in orthodontics, and their control is essential for successful orthodontic treatment. The purpose of this retrospective cephalometric study was to quantify the anchorage and vertical control capabilities of the double TPA by comparing Class II patients treated with full fixed appliances (Roth prescription - GAC 022x028 Slot), extraction of maxillary first bicuspid and utilizing double TPAs on first and second molars to a control sample of matched untreated Class II patients. Methods: Pretreatment and post treatment lateral cephalograms were analyzed of 20 patients (10 male and 10 female) from the same orthodontic practice with skeletal Class II malocclusions, characterized by hyperdivergent growth patterns. All of the 20 patients had maxillary first premolar extractions and double TPAs on the maxillary first and second molars. Space closure was done without headgear, utilizing only double key-hole closing loop archwires. The experimental sample was matched to controls from the Michigan and Bolton Brush growth studies. 12 cephalometric variables were identified to evaluate the effect of double TPAs on various skeletal and dental parameters including anchorage and vertical control. Results: Upper first molar horizontal movement in TPA experimental group: U6-PT Vertical=0.9mm, 6-PTV UMKC=1.2mm and H Perp to mestal U6-0.6mm, moved less forward compared to controls. Upper first molar vertical movement in TPA experimental group: U6-PP measurement in the TPA group was 1.9mm less compared to controls. U6-SN measurement was statistically insignificant between the two groups. Conclusions: Double TPAs were shown to effectively hold the anchorage and control the vertical in the treatment of skeletal Class II hyperdivergent patients.

Book excerpt: Lingual arches are frequently used in orthodontic extraction treatment, with the intended purpose of achieving maximum anchorage in the mandibular arch; however, little evidence exists in the literature that supports this anchorage approach. The lingual arch is widely supported in the literature for space maintenance and sagittal correction. It has been proven effective at minimizing any decreases in arch depth and perimeter when there is early loss of primary teeth. Theoretically, the lingual arch as a passive maintenance appliance should exert no forces on the teeth, yet studies have shown both the molars and incisors to move. To examine its anchorage efficiency further, we compared incisor and molar position in extraction treatment with and without the use of a lingual arch. Pre and post-treatment cephalograms that included lower premolar extractions were recruited from a depository of images. Canine retraction with power chain or NiTi closing coils with or without the use of a lingual arch for maximum anchorage were compared for incisor and molar position. Angular and millimetric measurements for IMPA, L1-NB, L1-APog and molar mesialization were measured and compared using a t-test. The greatest difference between pre and post-treatment was the IMPA with 5.19° more uprighting of the incisors in the group with no lingual arch, followed by 4.38° more uprighting in the L1-APog measurement. However, none of the differences between the groups were significant: IMPA (p=0.129), L1-NB (angular p=0.161, millimetric p=0.205), L1-APog (angular p=0.197, millimetric p=0.196) and mesialization of the molar (p=0.308). The change in incisor and molar position does not significantly differ with or without the use of a lingual arch in extraction treatment. Clinically, this suggests that the lingual arch does not provide maximum anchorage. Another modality, such as TADs, may provide a better source of anchorage for space closure.

Statistical data analysis included descriptive statistics of the data, analysis of the matrices, Wilcoxon Signed Rank tests and Kruskal-Wallis tests of the changes which occured during treatment. The study concluded that extraction treatment does not necessarily lead to the narrowing of the dental arches in the canine region.
