Muybridge, it is almost possible to hear the horse galloping. The pounding sound of the hoofs hitting the ground -like a drum- can also echo the rythmic beating of the human heart. That sound, that visceral rhythm, reminds us of the link between motion and performance: the perfectly executed stride of the horse, the incredible coordination of multiscale phenomena behind a heart beat. Furthermore, the decomposed sequence in Muybridge's photograph has become a well-known example of breaking motion into its components over time, and as such is reminiscent of those images that are routinely acquired in clinical practice, where the heart appears dilating and shirnking in a sequence of snapshots. The investigation of this motion and its subtleties is essential for refining our understanding of cardiac function, and the appreciation of how and when this motion is no longer perfectly executed can lead us to understand functional impairments and provide insight into the unfolding of pathology. In the presence of congenital heart disease (CHD), cardiac mechanics are altered: from single ventricle physiology to conduction abnormalities to different cardiomyopathies, it is important to both capture and interpret biomechanical changes that occur in the presence of a congenital defect. This special issue in Frontiers in Pediatrics, now an e-book, focuses on 'Ventricular mechanics in congenital heart disease' and looks at current knowledge of phenomena such as systolic/diastolic dysfuction and current methods (chiefly in cardiovascular magnetic resonance imaging and echocardiography) to evaluate cardiac function in the presence of CHD, and then presents a series of original studies that employ both medical imaging and computational modelling techniques to study specific CHD scenarios.

Book excerpt: Many noteworthy advances in our knowledge of the pericardium, its functions and diseases and their relation to heart failure have been made since the first edition of this book appeared in 1981; and no other book that covers in detail the physiology and pathophysiology has since been published. The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.

Book excerpt: The heart and lung are intricately linked. When the heart is affected by disease, the lungs will often show some related pathological or clinical conditions and vice versa. Pulmonary heart disease is by definition a condition when the lungs cause the heart to fail. The left ventricle in combination with the other structures in the “left heart” pumps blood throughout the body. The right ventricle (and structures of the “right heart”) pumps blood to the lungs where it is oxygenated and returned to the left heart for distribution. In normal circumstances, the right heart pumps blood into the lungs without any resistance. The lungs usually have minimal pressure and the right heart easily pumps blood through. However when there is lung disease present, like emphysema, chronic obstructive lung disease (COPD) or pulmonary hypertension- the small blood vessels become very stiff and rigid. The right ventricle is no longer able to push blood into the lungs and eventually fails. This is known as pulmonary heart disease. Pulmonary heart disease is also known as right heart failure or cor pulmonale. The chief cause of right heart failure is the increase in blood pressure in the lungs (pulmonary artery).
