Hospital acquired infections (HAIs) cause more than 98,000 deaths annually in the United States, according to Infection Control and Hospital Epidemiology. (HAIs) are avoidable and can be prevented with proper hand hygiene compliance. Hospitals and health facilities have focused greater attention on hand hygiene in the last few years because of growing concern about HAIs. Staff education is a key element of the implementation of an effective hand hygiene improvement program. Improvements in hand hygiene practices have a clearly demonstrable positive impact on nosocomial infection rates.

Definitions of health-care settings are proposed in Appendix 1. These Guidelines and the associated WHO Multimodal Hand Hygiene Improvement Strategy and an Implementation Toolkit (http://www.who.int/gpsc/en/) are designed to offer health-care facilities in Member States a conceptual framework and practical tools for the application of recommendations in practice at the bedside. While ensuring consistency with the Guidelines recommendations, individual adaptation according to local regulations, settings, needs, and resources is desirable. This extensive review includes in one document sufficient technical information to support training materials and help plan implementation strategies. The document comprises six parts.

With non- compliance of proper hand washing technique from staff, nosocomial infections may occur. Nosocomial infections are infections that have developed in a patient while under medical care. These infections were not there or had been developing before the patient had been admitted into the healthcare facility, but the infection did arise within the seventy two hour time frame after being admitted into the healthcare facility. These infections that have developed in the hospital can be detrimental to a patient's recovery and many of these infections are produced by microorganisms that have become resilient to antibiotic therapy. Research has indicated that in many hospitals across the United States, shockingly hand washing compliance from healthcare staff fell below fifty percent. However, these numbers are not entirely valid due to the fact that many of the statistics are gathered by auditors which work in the healthcare facility and the workers are conscious of their existence. These figures were compared against nosocomial infections in the hospital setting and validated that with simple hand washing technique, many infections could have been avoided. Hospital acquired infections are detrimental in that they delay a patient's recovery process, extend hospital stay, development of resistance to antibiotic therapy, financial burden to the patient and to the healthcare system, disability, and even death. Hand hygiene is the most effective and basic way to prevent the rise of nosocomial infections. The way to reduce the occurrences of nosocomial infections is to provide education from current evidence based practice, skill training, mandatory in-services, and compliance with behavioral change from staff (Mehta, Y., Gupta, A., Todi, S., Myatra, S., Samaddar, D., Patil, V., Bhattacharya, P., and Ramasubban, S., 2014). This paper will discuss many options that healthcare workers may follow in order to be compliant with proper hand hygiene technique.

A quantitative cross-sectional survey using primary data collected via a WHO validated self-administered questionnaire has been distributed to students at the Solwezi College of Nursing. 167/206 participants were recruited via stratified random sampling. Descriptive statistics highlighted potential relationships between demographic/training factors and students hand hygiene knowledge which reduces HAI. Moreover, potential relationships were analysed using Fishers exact test and a multinomial logistic regression. The low compliance to effective hand hygiene has continued to fuel the high prevalence of Hospital Acquired Infections in Africa. The large number of nursing students has a potentially high impact at reducing the HAI public health problem in Zambia, however, there is insufficient information on the nursing student’s demographic/training factors affecting their hand hygiene knowledge which reduces the HAI risk.

Book excerpt: Based on the research studies, Hospital Acquired Infections (HAI) according to the World Health Organization (WHO,) review of scientific data related to hand hygiene (2009), in the industrialized countries 5-15% is claimed by HAI of inpatients and affects 9-37% of those admitted to intensive care units (ICUs). HAI's are responsible for prolonged hospital stays due to complications such as MRSA leading to costly treatments. Most of the deadly pathogens are carried by the healthcare from one patient to another or from one place to another if proper hand hygiene is not observed. However compliance of washing hands remains low or poor among healthcare workers. In published studies suggest that, on average, compliance with hand hygiene is around 40% (WHO Guidelines on hand hygiene in health care). Education about hand hygiene remains the main factor to reinforce the prevention of hospital-acquired infections. The process of education is based on Health Belief Model a conceptual framework that describes a person's behavior as an expression of health beliefs. This is where one focuses on individual's perception of risks and benefits that can change in health related behavior such as washing hands. There are several components of the model including susceptibility, severity, benefits and barriers. An education program to motivate health workers should be initiated to improve washing hands to prevent HAI. Providing regular training on the importance of hand hygiene using " My 5 Moments for hand hygiene" approach and the correct procedures of hand rubbing and hand washing to all health workers. (WHO, 2005) By providing the tools to support health care workers and others responsible for improving patient safety at the national and local levels WHO Patient Safety hopes to see compliance increase in each country of the world from its current Impact of Hand Washing Education on Hospital Acquired Infection baseline. The goal is to promote hand hygiene and increase awareness of Hospital Acquired Infections through education Evidenced Based programs.

Increasing readmission rate associated with hospital or healthcare acquired infections (HAI) affect millions every year resulting loss of lives in addition to extra cost of care and time. Research has shown that lack of hand hygiene compliance among health care workers such as nurses, doctors, nursing assistants and other allied health workers is directly linked with health care-associated infections. While infection can affect people of all ages, the most vulnerable are the elderly, the very young, people with chronic illnesses such as end stage cancer, surgical patients, as well as people undergoing invasive procedures such as dialysis. Health care-associated infection (HCAI), also referred to as hospital acquired infection(HAI), is an infection occurring in a patient during the process of care in a hospital or other health care facility which was not present or incubating at the time of admission. Prolonged hospital stays, long-term disability, increased resistance of microorganisms to antimicrobials, massive additional costs for health systems, high costs for patients and their family, and unnecessary deaths are some the outcomes of HCAIs (World Health Organization) (WHO).Implementing structured education and training will improve healthcare workers compliance with hand hygiene in order to prevent the occurrence and spread of deadly infections before they occur.

An implementation plan as well as an evaluation plan is included in the proposal. Getting all the stakeholders involved is also described in the dissemination plan. Finally, there will be patient surveys and a hand hygiene questionnaire to proceed with the proposal. Studies have shown that hand hygiene, whether its soap and water or alcohol based hand rubs help to decrease bacteria on a health care provider's hands or family members' hands, which helps to stop the spread of infection post operatively and during a stay at a hospital. Something so small can save patients, families, healthcare facilities, time and money.

Book excerpt: Hospital acquired infections (HAI) are a major public health problem today, resulting in increased morbidity, mortality, and increased health care cost. According to Centers for Disease Control and Prevention (CDC), there were approximately 1.7 million HAIs which caused 99,000 deaths in 2002. Hospitalized patients carry infectious microorganisms which are transmitted from one patient to other by the hands of health care workers (HCW). Hand washing is the best way to remove these microorganisms and to prevent transmission of diseases. According to Takahashi and Turale (2010), regular and timely hand washing practices can reduce the incidence of infection in health care facilities. However, hand washing compliance remains unacceptably low among HCWs. A study conducted by Knoll and others (2010), revealed that compliance with hand hygiene can be positively influenced by education, knowledge, motivation and relevant training courses; and also be negatively impacted by external factors such as increased workload. Implementing an evidence-based hand hygiene compliance program to improve hand hygiene practice and to promote compliance is paramount important in preventing HAIs among hospitalized patients. Disseminating the results of the compliance program targeting the intended audience would share knowledge about the program to improve practice and to maximize its impact on patient outcome. Evaluating the HCWs knowledge and compliance with hand hygiene practices using a monitoring tool and competency check list will determine the effectiveness of the program. Compliance with recommended hand hygiene guidelines reduces transmission of microorganisms and decreases the incidence of HAIs which in turn will decrease mortality, morbidity, and health care cost.

Intensely troubled by the tragic and baffling loss of so many young mothers, Semmelweis sought answers. The Etiology was testimony to his success. Based on overwhelming personal evidence, it constituted a classic description of a disease, its causes, and its prevention. It also allowed a necessary response to the obstetrician's already vocal, rabid, and perhaps predictable critics. For Semmelweis's central thesis was a startling one - the fever, he correctly surmised, was caused not by epidemic or endemic influences but by unsterilized and thus often contaminated hands of the attending physicians themselves.
