Book excerpt: Problem: Obesity among preschool-aged children is a major public health concern as obese children are more likely to be obese as adults, and obesity is a risk factor for many chronic diseases. Diets high in fruits and vegetables (FV) may decrease the risk of many chronic diseases as well as contribute to obesity prevention. However, many preschool-aged children in the US are not meeting recommended intakes of FV. Therefore, the purpose of this study is to examine previous interventions conducted to increase FV consumption among preschool-aged children. Methods: The search for published articles was conducted using PubMed through the Weill Cornell Medical College library. The search terms "fruit AND vegetable AND consumption AND intervention" with specific filters were used to find studies that had been conducted on preschool-aged children, had full-text versions available online, were published in English, and were of rigorous design. Results: Thirty-four articles resulted, but only 12 were eligible to be included and further examined in this paper. Conclusions: The studies showed promising results for interventions conducted among preschool-aged children to increase the consumption of FV at least in the short-term. One common theme of many of the interventions was repeated exposure to and tastes of FV. Increased fruit consumption was typically achieved through less rigorous strategies, which may be due to children's natural taste preference for sweet food items. Further, many of the interventions involved multiple components.

As seen by the various findings and recommendations, not only is more work in this area required but the translation of this work to practice and policy is imperative if we are to address the challenges impacting the nutrition, physical activity, and health of young populations.

"Raised on Veggies" provides the knowledge and tools necessary to help parents increase their child's fruit and vegetable intake and set the stage for a lifetime of healthy eating habits.

Book excerpt: Schools are an attractive entry point to improve children’s diets, as their eating habits can be shaped during childhood and the information disseminated from school can reach adults through children. We implemented a cluster-randomized trial in 12 schools in peri-urban Viet Nam to assess if two school-based interventions increased knowledge of healthy diets among children and their parents, as well as children’s consumption of healthy foods. First, children were given lessons about food before school lunch and encouraged to share the lessons with their parents. Second, children were provided with healthy snacks for five weeks to reinforce messages about healthy eating. We found that in the short term, the nutrition lessons raised the knowledge index score of the children by 0.35 standard deviation. After six months, this intervention retained its effectiveness only for the children who also received free access to fruit, emphasizing the linkage between knowledge and practice. By itself, free access to fruit at school increased the children’s daily fruit consumption by half a portion, but not at the expense of home fruit consumption. Access to healthy foods at school can therefore be an effective measure to raise children’s healthy consumption. Child-parent communication was not a reliable channel for knowledge dissemination in our setting.

Book excerpt: Background: The intersection of health care, food insecurity and diet is increasingly a focus of policies and programs targeting the related crises of high rates of obesity and other forms of malnutrition, associated chronic disease risks, impaired performance at work and school, and soaring health care and other economic costs, specifically in low-income families. Nutrition incentive programs may mitigate the severity of food insecurity and low fruit and vegetable (FV) consumption by augmenting federal nutrition benefits and improving access to and affordability of FV with financial incentives at their point of purchase. Fruit and vegetable prescription programs, an innovative nutrition incentive model, aim to increase access to health-supporting foods for patients at risk of chronic disease. Only a few prescription program studies have been published in the peer-reviewed literature, demonstrating decreased hemoglobin A1c levels and BMI, and increased FV consumption, and important gaps remain. No prescription study analyzes pediatric prescription programs; most incentive studies focus on changes in FV purchasing or sales rather than consumption or food security; and incentive studies rarely account for measured and unmeasured confounders in the largely self-selected, convenience samples of participants. The two papers that comprise this dissertation assess changes in household food security and children’s fruit and vegetable consumption for low-income families participating in a pediatric fruit and vegetable prescription program. Methods: The Wholesome Wave FVRx program is a 4-6 month intervention offered since 2011 in select cities and states across the country. In pediatric programs during 2012-15, health care providers enrolled children and youth aged 2-18 years (one per household) who were clinically obese or overweight. Participants received nutrition education by a clinician, nutritionist, or trained health educator at each clinical visit or in a class setting (approximately monthly), including guidance on FV consumption and replacement of unhealthy foods with fresh FV. Providers distributed prescriptions allocated by household size ($0.50-$1.00/per person per day) and shared details of partnering farmers markets, where prescriptions were redeemed for produce. Change in food security was assessed among 578 households from 2013-15, using five measures from the USDA Household Food Security Supplement. Change in fruit and vegetable consumption was calculated for 1024 children between their first and last visits in the years 2012-2015 using an adapted National Cancer Institute screener. Paired t-tests and McNemars paired tests were used to compare study variables between first and last visits, and multiple linear regression analyses, including propensity dose-adjusted models, were used to model the change scores of each as a function of important socio-demographic and program-specific covariates. Results: The percentage of households experiencing very low FS was greatly reduced and those with high/marginal FS increased more than 30%, demonstrating the desired program impact, consistent with SNAP-based incentive programs. Moreover, highest program exposure (specifically, 5 or 6 clinical visits) was associated with a higher food security change score, suggesting that increased levels of participation could indicate a larger degree of change in household food security from the beginning to end of program participation. Households whose primary caretaker had attained higher than high school education also had a greater mean change score. We found both an unadjusted mean increase in children’s FV consumption of 0.33 cups between first and last visit, as well as a dose propensity-adjusted increase in the mean change score of more than a quarter-cup for each 1-unit change in total visits while holding predicted number of visits and site constant. Contrary to common findings in children’s nutrition interventions, a greater portion of the change score increase was attributed to vegetable consumption as compared to fruit consumption. In all outcomes, select clinical sites had higher change scores than those in the reference site, suggesting implications for program design and implementation. Conclusion: Fruit and vegetable prescription programs in clinical settings have the potential to increase food security and children’s fruit and vegetable consumption in low-income households. Future research should utilize a comparison group, complete validated measures, and include qualitative analysis of site-specific barriers and facilitators to success. Comparative effectiveness research could help build the business case for weaving food-based programming into usual care. Integrating food security screenings and referral to local food resources are important steps in improving food access for all and should be adopted by health care providers regardless of an accompanying fruit and vegetable prescription program.

Book excerpt: Food insecurity and low resources continue to be a burden influencing the health, well-being, growth, and development of millions of U.S. children and adults. Groups and individuals experiencing restrained access to food are our neighbors, individuals we may see each day, and individuals who we may not interact with or see because of their isolated situations. They include the elderly, those experiencing mental illness, veterans, certain race/ethnic groups, adolescents, young women with children, those living in rural areas, and those using food pantries, among others. Many of these groups, both hidden and visible, have rates of food insecurity above the national average that are resistant to national improvements in food security. Yet, attention to these subsets of the population is imperative to improve U.S. health and nutrition and to reduce rates of chronic disease. Many groups face specific barriers to maintaining sufficient food, for example, rural populations may find it difficult to access federal food assistance or other resources such as food pantries and nutrition education because of distance or lack of consistent internet access separating them from these resources. Further, their remote locations may make it difficult to obtain the types of foods that they prefer. Other specific barriers may include limited facilities and equipment for food preparation, access to culturally appropriate foods and preparation supplies, and foods that complement the foods that they already have. Tailored approaches to quantify access to food, the nutrition environment, dietary intake, and other barriers are necessary to build successful interventions and to quantify the needs of these populations.

Book excerpt: Data from the National Health and Nutrition Examination Survey (NHANES) shows that the prevalence of childhood overweight has almost tripled in the past three decades. In 2006, data from the Pediatric Nutrition Surveillance System (PedNSS) showed the prevalence of overweight among low-income children aged 2 - 5 years was 14.8%, compared with 13.9% for all U.S. children of a similar age. The Pennsylvania WIC Program set the long-term objective of reducing the incidence of childhood obesity among participants enrolled in the WIC Program. By January 2001, the Pennsylvania WIC Program implemented the Obesity Prevention Series, an on-going nutrition education plan for parents and caregivers of children over the age of two. Key educational components of the series include: physical activity promotion, parenting skills related to child feeding, choosing lower fat fast foods and convenience foods, role modeling, limiting sweetened beverages and snack foods and increasing the consumption of water, fruits and vegetables. The purpose of this study was to evaluate the effectiveness of the Obesity Prevention Series Module "Increasing Fruits and Vegetables" at promoting knowledge of the health benefits of and positive attitude towards consumption of fruits and vegetables, as measured by a change in parental knowledge and beliefs, and their intention to change behavior. The study was conducted at three Pennsylvania WIC sites located in Delaware County, Pennsylvania. Parents or guardians of children two to five years of age were asked to complete a survey to measure knowledge, attitudes and self-efficacy prior to participating in a brief nutrition education session using the Obesity Prevention Services Module "Increasing Fruits and Vegetables". The nutrition education session is designed to encourage families to eat more fruits and vegetables and manage barriers. Following the brief nutrition education session, parents and caregivers were asked to complete a survey similar to the survey they had completed previously. The post survey provided a space for parents and caregivers to write in goals related to increasing the fruit and vegetable consumption of their child or themselves as a means to measure intention to change behavior. Twenty-nine paired pre and post surveys were collected. The Obesity Prevention Series Module "Increasing Fruits and Vegetables" was effective in positively changing knowledge, attitudes and self-efficacy as measured by change in mean response pre to post survey. Interestingly, 14%(4) of participants in this study did not agree that their consumption of fruits and vegetables could influence their child's consumption of fruits and vegetables, at both pre-survey and post survey. Fifty-two percent of parents and caregivers intended to change behavior following the intervention by setting fruit and vegetable related goals. The most popular goals set by parents and caregivers included serving fruits and vegetables with every meal, hiding vegetables in foods, trying new fruits and vegetables and serving more fruit salad, mixed vegetables, frozen fruit bars, or stir fries. Childhood overweight and obesity is a complex issue with many related causes such as genetics, environment, diet and level of activity. There is some research that shows an inverse association between the consumption of fruits and vegetables and the incidence of childhood overweight and obesity. A key educational message for parents and caregivers moving forward would be to eat and serve more fruits and vegetables as part of a low-fat diet. Nutrition education needs to address attitudes related to modeling behaviors, parent/child feeding roles, the use of food as a reward and increase knowledge regarding the purchase, storage, and preparing of fruits and vegetable. WIC Nutritionists can work with parents and caregivers to explore their perceptions of fruits and vegetables, to reduce barriers and to increase their children's access to fruits and.
